[Pathological fracture in primary malignant bone tumors].

Retrospective analysis of 30 patients with pathological fracture out of 336 patients with primary malignant bone tumors should demonstrate the influence a pathologic fracture and the form of surgical therapy have on the survival rate. In 25 out of 30 patients a fracture led to diagnosis of the disease. Pathological fractures occurred cumulatively by malignant fibrous histiocytoma of the bone and in tumor stages IIb and III. Surgery was performed on 26 out of 30 patients (12 ablative therapies, 14 reconstructive therapies). The mortality risk for patients with pathological fractures was more th

Fracture and bone mineral density in hemodialysis patients.

AIM: The aim of this cross-sectional study was to determine in hemodialysis patients the pattern of low trauma fracture, the ability of dual X-ray absorptiometry (DXA) to discriminate between patients with and without fracture, and the magnitude, distribution and mechanism of bone loss. PATIENTS AND METHODS: Eighty-eight patients were studied. Bone mineral density (BMD) was measured by DXA at lumbar spine (LS), femoral neck (FN) and 3 radius sites (UD, MID and 1/3R). In 11 patients (12.5%), 16 fractures occurred and were predominant at the distal forearm and ribs. RESULTS: Patients with frac

Facial reanimation after temporal bone fracture.

Fracture of the temporal bone is usually associated with skull injury and the patient is initially seen in the emergency room. As a result the diagnosis of facial paralysis may be delayed. The aim of this study is to emphasize the importance of early diagnosis and treatment of facial paralysis in such cases. Between 1990 and 1996, sixteen patients with facial paralysis due to temporal bone fracture were operated on within one month of trauma. The surgical technique was selected on the basis of the location of fallopian canal involvement and the patients hearing status. Six patients had grade V

[Bone tissue regeneration after a gunshot fracture].

A general histological analysis of regularities of the osseous tissue reparative regeneration was performed in dops with gunshot fracture of long tubular bones. Methods of light and electron microscopy, cytophotometry of summary proteins optic density, historadioautography with 3H-thymidine were used. On the site of the osseous defect forms a complicated in tissue composition regenerate in the course of proliferative stage of regeneration and in interaction of diverse cellular differons. Viable osteogenic elements of bony fragments are the accessory source of the osseous regenerate forming. A

The epidemiology of fragility fractures: is there a role for bone quality?

Fragility fractures, particularly those of the hip, vertebrae, and distal forearm, constitute a major public health problem. The two ultimate determinants of fracture are bone strength and propensity to trauma. Bone strength depends not only upon bone mass but also upon a variety of qualitative aspects of bone structure. These include its architecture, the amount of fatigue damage it has sustained, and changes in its bulk material properties, indices that are collectively subsumed into the term "bone quality." Fragility fractures show differences in their patterns of incidence by age, sex, eth

Fracture of temporal bone with exsanguination: pathology and mechanism.

Eight cases of basal skull fracture with transverse fracture of the petrous temporal bone with medial extension to the internal carotid artery and lateral extension of the structures of the middle ear are described. Injuries in all cases were due to major blunt impact to the head usually occurring in a motor vehicle accident. General autopsy revealed major blood loss without any obvious external or internal site of hemorrhage suggesting that exsanguination was a complication of the head injury. The internal carotid arteries at the most medial extension of the fractures were lacerated or transe

[Surgical tactics in pathological fractures of the long bones].

In the article is presented an experience of treatment of 201 patient with benign tumours and tumour-like bone affections, aggravated with pathologic fractures. The principal conclusion of the work--early operative treatment of the given pathology (with the exception of dystrophic bone cysts of some types) in contrast to the rooted wait-and-see surgical tactics.

[Clinical studies on treatment of fractures of the zygomatic bone].

The author has made clinical studies on treatment of fractures of the zygomatic bone in terms of frequency of fractures according to sex, age, fracture type, main manifestations and treatment methods from 106 patients with zygomatic bone fractures among 969 patients with maxillofacial bone fractures. The results obtained were as follow: 1. The frequency of malar bone fracture was 4 times more in male than that in female. 2. The most prevalent age of malar bone fracture was 21-30 years of age, and the nexts were followed 11-20, 31-40, 0-10, 41-50, 51-60, 61-70, and over 71 in the orders. 3. Amo

[Fractures of the petrous bone. An evaluation of 123 cases].

Out of 2,888 blunt head injuries that were treated at the Hermann Hospital Trauma Center of the University of Texas at Houston, only 123 temporal bone fractures were diagnosed in 110 patients. The clinical diagnosis was confirmed with high resolution computerized tomography of the temporal bones. Three main categories of fractures were identified: oblique in 74%, longitudinal in 13% and transverse in 13%. The clinical manifestations included hemotympanum in 83%, cerebrospinal fluid otorrhea in 21% and Battle's sign in 10.6%. One third of the patients had conductive hearing loss, 1/2 had mixed

Colles' fracture and bone density of the ultradistal radius.

To determine whether Colles' fracture, generally considered a manifestation of postmenopausal osteoporosis, is associated with a decrease in bone density at the site of fracture, we measured bone mineral density of the ultradistal radius (UDR-BMD) by single-photon absorptiometry with computer-assisted image processing. In 119 normal women (ages 22-92 years), UDR-BMD decreased by 17% between ages 30 and 75 years. From UDR-BMD measurements in these normal women and in 40 women (ages 53-80 years) with Colles' fracture alone, a population-based analysis was made to estimate fracture risk at differ

Bone mineral density and fracture in postmenopausal women.

This retrospective study examined bone mineral density (BMD) for discrimination of female patients with fractures. Bone densitometry was done in 146 patients over the age of 50 years at radius, lumbar spine, and proximal femur sites using single and dual photon absorptiometry. The patients were divided into three groups: (A) no osteoporotic fractures (n = 92); (B) mild spine fractures with greater than 15% compression (n = 38); and (C) hip fractures (n = 16). Groups B and C did not differ significantly from each other in BMD, but these groups differed significantly from group A for spine and f

[Surgery of facial paralysis in fractures of the petrous bone].

Follow-up of 38 patients with facial nerve paralysis in fractures of the petrous bone. Transmastoidal approach was performed in 23 cases. 15 patients underwent either translabyrinthine or transtemporal approach. The clinical and intraoperative findings and procedures are demonstrated. The need for surgical treatment in facial paralysis caused by petrous bone fractures is stressed.

Bone loss and forms of tibial condylar fracture.

Bone loss and fracture type were determined in 358 tibial condylar fractures. Degree of osteoporosis at the knee was defined by analogy to Singh's grades of osteoporosis at the hip and to Jhamaria's classification of osteoporosis at the calcaneum: grade V-normal; grade IV-slight reduction of trabecular bone; grade III-osteoporosis, predominantly of trabecular bone; grade II-osteoporosis of trabecular and cortical bone. Five fracture types were distinguished, with the component of trabecular bone compression increasing from a to e: a-pure split fractures; b-split fractures with displacement of

Trabecular bone architecture in the pathogenesis and prevention of fracture.

Rapid loss of trabecular bone (as after menopause) occurs by complete removal of some structural elements, leaving those that remain more widely separated and less well connected. The most likely cellular mechanism is an increase in the number of resorption cavities deep enough to lead to focal perforation of trabecular plates, either as a non-specific consequence of increased remodeling activation, or as a specific consequence (direct or indirect) of estrogen deficiency. Disruption of the connections between structural elements produces a disproportionate loss of strength, for which the incre
