The use of dorsal distraction plating for severely comminuted distal radius fractures: A review and comparison to volar plate fixation.

INTRODUCTION: Optimal fixation for highly comminuted distal radius fractures remains a major treatment challenge for orthopaedic surgeons. Dorsal distraction plating can serve as an improved fixation technique by allowing reduction under ligamentotaxis, providing a dorsal buttress, addressing proximal comminution, and allowing for early weightbearing in polytrauma patients. The aim of this study was to review current literature regarding treatment of distal radius fractures treated with dorsal distraction plating. METHODS: We performed a literature search in Pubmed and EMBASE databases to ide

Smoking, medial comminution, and single fixation as predictors of failure in the fixation of native distal femur fractures: a multicenter retrospective study over 109 fractures.

INTRODUCTION: Native distal femur fractures (DFF) pose a significant challenge. The rates of fixation failure and nonunion range from 10 to 50%. This study aims to identify patient characteristics, injury types, or treatment factors that may predict failures in the surgical management of native distal femur fractures. METHODS: This retrospective study was conducted by reviewing all distal femur fractures treated consecutively from January 2015 to December 2022 at three orthopedic trauma referral centers. Adult patients with acute 33 A2–A3, and 33 C distal femur fractures who were treated with

The use of adjuvant anteromedial support plate fixation in the treatment of comminuted proximal humerus fractures: A retrospective observational study.

Proximal humerus fractures (PHFs) are clinically common and are usually treated with proximal lateral locking compression plate fixation. However, for partial comminuted PHFs, lateral plate fixation cannot restore the support of the proximal humerus medial column, so there are more postoperative complications and even surgical failure. To improve the success rate of proximal humeral comminuted fracture surgery and reduce the complications, we used anteromedial support plate combined with lateral locking compression plate for the treatment of proximal humeral comminuted fracture. From January 2

Primary Total Ankle Replacement for Acute Comminuted Pilon Fractures in Older Adults: Proposed Indications and a Report of 29 Cases.

BACKGROUND: Management of severe distal tibia pilon fractures (AO/OTA 43-C3) in older adults is exceptionally challenging. Open reduction and internal fixation is associated with a high incidence of posttraumatic arthritis, whereas primary arthrodesis results in significant functional limitations. We investigated primary total ankle replacement (TAR) as a definitive, single-stage, motion-preserving solution. MATERIALS AND METHODS: We retrospectively reviewed 29 older adult patients (age ≥60 years) treated with primary TAR since 2016 for comminuted pilon fractures. Inclusion required severe ar

Industrial Plasticine-Assisted Physical-Model Reconstruction Versus Virtual Surgical Planning for Comminuted Le Fort I Fractures: A Retrospective Cohort Study.

Reconstruction of comminuted Le Fort I fractures is difficult when the tooth-bearing maxilla is divided into multiple fragments and stable skeletal landmarks are limited. The authors describe a hybrid physical-digital planning strategy that uses industrial plasticine as a reversible provisional stabilizer during assembly of 3-dimensionally printed fracture fragments. The dentate fragments are first oriented according to dental arch form, intercuspation, fracture-line morphology, and residual stable anatomy. Small amounts of plasticine are then applied only to the printed models, permitting rep

Management of Comminuted Midshaft Clavicle Fractures with Interfragmentary Screws, Non-Invasive Screw Fixation Knot Technique and Plating in 83 Patients.

INTRODUCTION: Midshaft clavicle fractures are common, with comminuted variants posing a higher risk of non-union, often necessitating surgical intervention. Anatomical repair is critical due to the clavicle's sole osseous connection between the shoulder and trunk, impacting functional outcomes. CASE SERIES: This retrospective study evaluated 83 patients (74 males, nine females, aged 18-65 years) surgically treated for midshaft clavicle fractures (2010-2023). Dominant-side fractures accounted for 87% of cases. Severe comminution was managed using 2.7 mm cortical interfragmentary screws and the

OCCS Classification and Treatment Algorithm for Comminuted Mandibular Fractures Based on 109 Patients and 11 Years Experiences: A Retrospective Study.

Comminuted mandibular fractures (CMFs) pose significant challenges to surgeons for their serious complications and poor outcomes. We aimed at proposing a classification with treatment algorithm of each category for CMFs. Patients with CMFs were retrospectively reviewed and classified into five categories: Type I: relatively good occlusion, no or slightly displaced fragments, no continuity destruction or bone defect; Type II: relatively good occlusion, damaged morphology, low comminution degree but intact continuity without bone defect; Type III: damaged morphology and higher comminution degree

Clinical results of using minimally invasive long plate osteosynthesis versus conventional approach for extensive comminuted metadiaphyseal fractures of the radius.

INTRODUCTION: The minimally invasive plate osteosynthesis (MIPO) technique has been introduced recently. The extensive comminuted fractures of the distal radial metaphysis with diaphyseal involvement are probably good indications for MIPO technique because of less extensive dissection and soft-tissue stripping. The purpose of this retrospective study was to compare the clinical results of MIPO technique to those of conventional open reduction in extensive metadiaphyseal fractures of distal radius. MATERIALS AND METHODS: Of 34 patients treated for comminuted metadiaphyseal fractures of the dis

Loss of correction in unstable comminuted distal radius fractures with external fixation and bone grafting--a long term followup study.

Over the years, management of complex distal radius fractures by closed means has often failed leading to late collapse. We have chosen the principle of ligamentotaxis using external fixation and bone grafting in this study to prevent late complications. Eighty one patients with complex distal radius fractures belonging to Type IV A, IV B, IV C of Universal classification were treated with an AO external fixator between 1995 and 2001. Mean age group was 38. 47 years with longest follow up of 7 years. Bone grafting was done primarily in 20 patients and early grafting (within 3 weeks) in 5 patie

Use of the volar fixed angle plate for comminuted distal radius fractures and augmentation with a hydroxyapatite bone graft substitute.

Treatment of distal radius fractures with a volar fixed angle plate achieves sufficient stabilisation and permits early physical exercise. However, secondary displacement after surgery sometimes occurs in elderly patients with a metaphyseal comminution and/or cases in which the subchondral support pegs were not placed immediately below the subchondral zone. We treated elderly patients suffering from distal radius fractures with metaphyseal comminution, using both volar fixed angle plate with or without augmentation with a hydroxyapatite bone graft substitute to investigate the benefit of augme

Clinical experience with reamed locked nails for close and open comminuted tibial diaphyseal fractures: a review of 50 consecutive cases.

We present the results of our experience in treating comminuted tibial shaft fractures with reamed interlocking intramedullary nail from September 1993 to December 1995. In this retrospective study, there were fifty patients with an average follow-up of 14.3 months (range six to twenty-eight months). Ninety-eight percent of the fractures were due to motor-vehicle accident with majority of the patients being motorcyclist (96%). Thirty-eight fractures were closed and twelve were open (Gustilo grade I--8; grade II--4); 44% of them had additional fractures or other injuries. According to Winquist-

Comminuted fractures of the basilar joint of the thumb: combined treatment by external fixation, limited internal fixation, and bone grafting.

Displaced, comminuted fractures of the thumb carpometacarpal joint were treated by intermetacarpal external fixation, anatomic reduction of the joint surfaces, bone grafting and adjunctive internal fixation in thirteen patients. Retrospectively, at an average follow-up time of 35 months, nine patients showed a good result and one patient had a fair result. All fractures had united without secondary displacement, but focal irregularities of the joint surface were commonly seen. As compared with the uninvolved side, axial rotation averaged 79%, radial abduction 89%, key pinch 88%, and grip stren

Posterior Plating Through a Single Posterolateral Approach for a Comminuted Thurston-Holland Fragment in a Pediatric Triplane Ankle Fracture.

We present the case of a 12-year-old boy who sustained a three-part triplane fracture of the left distal tibia following a torsional fall, with a small, comminuted posterior Thurston-Holland fragment. Triplane fractures are transitional injuries unique to mid-adolescence. While displaced fractures are conventionally managed through an anterior approach with indirect reduction of the posterior fragment, this technique relies on the posterior fragment being a single, congruent piece. In the present case, comminution of the posterior fragment precluded reliable indirect reduction, prompting a pos

Closed Reduction Internal Fixation with Titanium Elastic Nailing System for a Proximal Humerus Shaft Comminuted and Segmental Fracture.

Segmental fractures of the proximal humeral shaft are rare and challenging to manage, particularly in elderly osteoporotic patients with multiple co-morbidities. Traditional open fixation methods carry risks of soft tissue complications, neurovascular injury, and delayed recovery. Minimally invasive techniques such as Closed Reduction and Internal Fixation (CRIF) with Titanium Elastic Nailing System (TENS) offer an alternative, though their use in segmental proximal humeral fractures remains underreported. We report the case of a 72-year-old female with diabetes mellitus, hypertension, and cor

In situ fluid-to-solid converting injectable wet adhesive for comminuted fracture repair via water-activated bonding.

Comminuted fractures, characterized by multiple irregular bone fragments, present significant challenges for traditional fixation methods like plates and screws, often resulting in complex surgeries and delayed healing. Injectable hydrogel adhesives have been applied for comminuted fractures, but face challenges such as excessive fluidity before curing and curing processes that involve high temperatures or ionic release, which can result in material leakage, localized inflammation and reduced adhesion. To address these challenges, we developed a humidity-responsive hydrogel adhesive system, co
