Colorectal cancer screening is now proven to be effective and is widely endorsed by guidelines committees. Several forms of screening have merit, but the screening tests used in individual patients are often dictated by insurers. The use of screening colonoscopy is expected to increase. Gastroenterologists will play the central role in ensuring the safe and cost-effective application of colonoscopy to screening and surveillance.

Since Selecting Therapeutic Targets in Inflammatory Bowel Disease-II (STRIDE-II), new evidence has become available across inflammatory and patient-centred outcomes. We aimed to update the STRIDE recommendations for Crohn's disease and ulcerative colitis. We searched MEDLINE, Embase, and Cochrane CENTRAL for studies published after the STRIDE-II search period. Findings were integrated with surveys of International Organization for the Study of IBD (IOIBD) members and paediatric gastroenterologists from the Porto group of the European Society for Paediatric Gastroenterology, Hepatology and Nutr…

BACKGROUND: Colonoscopy is commonly used to screen for neoplasia. To assess the performance of screening colonoscopy in everyday practice, we conducted a study of the rates of detection of adenomas and the amount of time taken to withdraw the colonoscope among endoscopists in a large community-based practice.
METHODS: During a 15-month period, 12 experienced gastroenterologists performed 7882 colonoscopies, of which 2053 were screening examinations in subjects who had not previously undergone colonoscopy. We recorded the numbers, sizes, and histologic features of the neoplastic lesions detect…

Otolaryngologists are unique … tolaryngologists (0.04% of practicing) performed manometry testing annually, compared to 493 (69.3) gastroenterologists (3.3%). Less than 1% of practicing otolaryngologists (37.8 (8.0) (0.04%)) and gastroenterologists (51.6 (8.4), 0.35%) performed 24-h pH testing annually. FEES testing was most commonly performed by otolaryngologists; however, only 48 (6.3) providers …

BACKGROUND AND AIMS: Inflammatory bowel disease (IBD) causes significant morbidity, frequently resulting in hospital admission and resection surgery. However, little is known about: 1. how IBD patients' inpatient healthcare utilisation compares to other inpatients and 2. whether there are potentially modifiable factors which may influence this.
METHODS: Over five months a cohort of admitted IBD patients were acquired and each assigned five admitted, age and gender matched controls at a single tertiary center. Data compared over 15 months included: total cumulative length of stay (TLoS), numbe

BACKGROUND: Transoral endoluminal gastroplasty (EG) by the Bard Endocinch device is available for the treatment of gastroesophageal reflux disease (GERD). This study assessed the early (<or=12 months) outcomes in patients undergoing EG performed by one gastroenterologist compared with another set of patients referred by the same gastroenterologist for laparoscopic antireflux surgery (LAS) at a foregut surgery center.
METHODS: From June 2000 to July 2002, 87 consecutive patients cared for by a single gastroenterologist were diagnosed with refractory GERD and underwent either EG (n = 47) or ref

Family history of cancer (FHC) is important in the context of cancer prevention and risk counselling, but there is a lack of information about its consideration in medical routine. We aimed to characterize how FHC is assessed and taken into account in the primary care setting in Germany. We conducted a mail survey among 285 office-based physicians in south-west Germany. We sent a questionnaire to randomly selected general practitioners, dermatologists, gastroenterologists, gynaecologists, urologists and pulmonologists, asking about collection of information on FHC and implications for preventi…

BACKGROUND AND AIMS: Alcohol use disorder (AUD) and alcohol associated liver disease (ALD) commonly co-occur, and integrated approaches to treatment are associated with better outcomes. This study aimed to develop an online module, the Program for Advanced Training of Hepatologists in Alcohol Use Disorder Management (PRATHAM), to enroll and train hepatologists, and to evaluate changes in participants' knowledge, attitudes, practices, and self-perceived competence pre- and post-training.
METHODS: This included identification of specific needs, establishing learning objectives, designing a curr

Hepatocellular carcinoma (HCC) is a common complication in patients with chronic liver disease and leads to significant morbidity and mortality. Liver disease and liver cancer are preventable by mitigating and managing common risk factors, including chronic hepatitis B and C infection, alcohol use, diabetes, obesity and other components of the metabolic syndrome. The management of patients with HCC requires treatment of the malignancy and adequate control of the underlying liver disease, as preserving liver function is critical for successful cancer treatment and may have a relevant prognostic

Barium granulomas are rare complications of the barium enema. They pose diagnostic problems to the gastroenterologist, who may suspect a carcinoma, and to the pathologist, who may have difficulty in determining the precise nature of the foreign body. From four cases we suggest a simple and quick diagnostic method: paraffin-block roentgenography.

Cervical esophageal perforation is rare, but it is associated with high mortality. We describe two patients with cervical esophageal perforation that required surgical treatment. In both cases, good outcomes were evenly achieved, despite the presence of risk factors. A prompt diagnosis and treatment with collaboration between a surgeon and a gastroenterologist are important.

This paper, the second in the series, will build on the first and explore the importance of liver and pancreatic manifestations of cystic fibrosis (CF) and the effect on morbidity and mortality of this multifaceted genetic condition. It will also further develop the critical role of the gastroenterologist as part of the multidisciplinary group of clinicians and allied health staff in the effective management of patients with CF.

Laryngopharyngeal symptoms (LPS) are increasingly common yet challenging presentations to gastroenterologists' office due to heterogeneous causes, with reflux accounting for a minority of patients.1,2 Given the suboptimal efficacy of empiric acid suppression therapy, accurately identifying patients with reflux-related symptoms remains crucial for targeted management strategies. Previous studies showed limitations of existing tests, with no clear gold standard for diagnosing laryngopharyngeal reflux disease (LPRD).3-5.

This Position Statement from the European Society of Gastrointestinal Endoscopy (ESGE) and the European Society of Gastroenterology Nurses and Associates (ESGENA) sets standards for the reprocessing of flexible endoscopes and endoscopic devices used in gastroenterology. An expert working group of gastroenterologists, endoscopy nurses, chemists, microbiologists, and industry representatives provides updated recommendations on all aspects of reprocessing in order to maintain hygiene and infection control.

Although variation is the rule with pancreatic duct morphology, certain anomalies occur as a result of altered embryological development. These anomalies can produce obvious clinical symptomatology or may be implicated as a cause of symptoms when noted in the evaluation of patients with gastrointestinal complaints of obscure origin. Gastroenterologists and radiologists should be familiar with these anomalies for more accurate interpretation of pancreatograms and their clinical implications.

This report presents the case of a 22-year-old male with recurrent genital ulcers, oral aphthae, and gastrointestinal ulcerations initially diagnosed as Crohn's disease by gastroenterologists. Despite overlapping clinical and histological features between Crohn's disease and intestinal Behçet's disease, a dermatological consultation raised suspicion for Behçet's, leading to a revised diagnosis. This case highlights the crucial role of dermatologists in preventing misdiagnosis and ensuring effective treatments for complex inflammatory conditions.

Pancreatic cancer is a highly aggressive disease with a poor prognosis. In advanced stages, symptoms such as biliary and gastric outlet obstructions frequently occur and can be managed through techniques such as advanced endoscopy, interventional radiology, and surgery. A multidisciplinary team, including oncologists, gastroenterologists, surgical specialists, and palliative care providers, is crucial to tailor and select treatment strategies that align with the individual goals and preferences of each patient.
