Some clinical aspects of geriatric surgery.

The relative frequency of surgical problems in the aged is discussed and some statistics based on experience are given. General rules in the management of the aged are suggested and the situation is analysed with stress on biological age and past influences. The increased importance in surgery of fluid loss as well as shock is discussed. A regimen for fluid replacement therapy is suggested. Problems in history-taking and the investigation of the elderly surgical patient are debated, and the value of ice as an anaesthetic agent for geriatric patients is discussed. Points about postoperative car

Sex counselling for your geriatric patients.

The geriatric population has traditionally received less attention by family physicians than any other segment of society. Sexual counselling is of necessity an important sphere of family medicine. The geriatric patient is entitled to and should receive empathy and support in this area. The physician must use basic sexual advice tailored to the special needs of this group. These needs are reviewed and certain truths and fallacies are discussed.

Primary-care-focused interpretable machine learning model for depression screening in geriatrics: A comparative study of multiple algorithms.

BACKGROUND: The prevalence of depression among older adults places a considerable strain on healthcare systems due to a shortage of psychiatrists for professional evaluations. This study reports a machine learning (ML) model to assist in screening for geriatric depression, enabling primary care practitioners to detect and respond to cases effectively and on time. METHODS: Data from the 2011-2018 National Health and Nutrition Examination Survey (NHANES) were used. To identify relevant variables for depression screening, features were selected using Least Absolute Shrinkage and Selection Operat

Management of overactive bladder: consensus statements from the Hong Kong Urological Association and the Hong Kong Geriatrics Society.

Overactive bladder (OAB) is a common urological disease with a high prevalence in older adult populations. Antimuscarinic drugs have been the most common treatment for OAB for more than a decade, but their anticholinergic side-effects and potential impact on cognitive function among older patients are usually underestimated. This consensus aimed to provide practical recommendations concerning OAB management, with a particular emphasis on older patients. A joint consensus panel was formed by representatives of the Hong Kong Urological Association and the Hong Kong Geriatrics Society. Literature…

Associations of the geriatric nutritional risk index and simplified geriatric nutritional risk index with chronic obstructive pulmonary disease.

PurposeThis study aimed to investigate the associations of the Geriatric Nutritional Risk Index (GNRI) and the simplified Geriatric Nutritional Risk Index (sGNRI) with Chronic Obstructive Pulmonary Disease (COPD).MethodsWe assessed 161 patients with COPD and 947 healthy controls. The sGNRI was derived using the formula: serum albumin (g/dL) + 0.1 × body mass index (BMI, kg/m2). Primary outcomes included the BODE (body mass index, airflow obstruction, dyspnea, exercise capacity) index and hospital length of stay (LOS). The secondary outcome was the prevalence of COPD.ResultsMalnourished patient

Atrial Fibrillation as a Geriatric Syndrome: Why Are Frailty and Disability Often Confused? A Geriatric Perspective from the New Guidelines.

Atrial Fibrillation can be considered a geriatric syndrome for its prevalence and incidence, its impact on patients' quality of life, and Health Systems' economy. The European Society of Cardiology 2024 guidelines introduce a recommendation for maintaining vitamin K antagonist therapy over switching to direct oral anticoagulants in clinically stable elderly patients with atrial fibrillation. This article explores the implications of this indication for the geriatric clinical context. The focus will also be devoted to the need for the stratification of older patients with atrial fibrillation, m

Geriatric assessment in older adults with acute myeloid leukemia: A Young International Society of Geriatric Oncology narrative review.

The therapeutic landscape of acute myeloid leukemia (AML) in older adults has been transformed by the advent of targeted therapies, including venetoclax (a B-cell lymphoma-2 inhibitor), gilteritinib (a FMS-like tyrosine kinase 3 inhibitor), ivosidenib, and enasidenib (isocitrate dehydrogenase 1/2 inhibitors). These agents, in combination with hypomethylating agents, have significantly improved outcomes among patients aged 60 years and older, however, overall survival remains very poor. Hence, the management of AML in this population requires a nuanced approach to balance overall survival, trea

Comprehensive Geriatric Assessment (CGA) Screening of Geriatric Patients in a Tertiary Care Hospital: a Cross-sectional Study in India.

BACKGROUND: Within the next few decades, India will witness a staggering increase in its elderly population owing to advancements in health care. However, an increase in life span is not mutually inclusive with high quality of living. Old age is associated with multiple ailments which decreases functionality and increases dependency, several of which go undiagnosed while catering to primary complaints. This prompts the need for a comprehensive approach to the healthcare of the elderly for which Comprehensive Geriatric Assessment (CGA is widely practiced in the west. OBJECTIVES: This study was

Geriatric Mental Healthcare Training: A Mini-Fellowship Approach to Interprofessional Assessment and Management of Geriatric Mental Health Issues.

INTRODUCTION: The population in United States aged 65 and older has rapidly grown and is projected to grow faster than any other segment of the population. Despite this demographic shift, the nation's geriatric workforce is shrinking. AIM: The primary goal of the fellowship was to form a learning collaborative that would help trainees in medicine, nursing, social work, pharmacy and occupational/physical therapy understand the roles of each discipline involved in the provision of geriatric mental healthcare and to enhance basic knowledge of common geriatric syndromes. METHODS: Faculty from th

Geriatric rescue after surgery (GRAS) score to predict failure-to-rescue in geriatric emergency general surgery patients.

BACKGROUND: Geriatric-patients(GP) undergoing emergency-general-surgery(EGS) are vulnerable to develop adverse-outcomes. Impact of patient-level-factors on Failure-to-Rescue(FTR) in EGS-GP remains unclear. Aim of our study was to determine factors associated with FTR(death from major-complication) and devise simple-bedside-score that predicts FTR in EGS-GP. METHODS: 3-year(2013-15) analysis of patients, age≥65y on acute-care-surgery-service and underwent EGS. Regression analysis used to analyze factors associated with FTR and natural-logarithm of significant odds-ratio used to calculate estim

[Vaccination of Older Adults in Portugal: Recommendations from the Geriatrics Study Group of the Portuguese Society of Internal Medicine].

Older persons are more susceptible to infections and have a higher risk of serious complications, with a worse functional and vital prognosis. Vaccination is an effective strategy with a favorable safety profile for preventing infections and promoting healthy aging. In view of the clinical evidence and the vaccines available in Portugal in the first half of 2025, the Geriatrics Study Group of the Portuguese Society of Internal Medicine presents a proposal for vaccination of adults aged 65 years or older. The experts also point out the need to create a national lifelong vaccination program that…

[S3 guidelines on the diagnostics, treatment and aftercare of peripheral arterial occlusive disease : Key points and implications for geriatrics].

The updated S3 guidelines on diagnostics, treatment and aftercare of peripheral arterial occlusive disease differ from previous versions in one key respect. While catheter-supported interventional and surgical treatment methods were previously covered in separate chapters, they are now evaluated jointly, depending on the location of arterial obstructions and the clinical stages. The stage of critical leg ischemia is specified in a prognostically relevant manner by the classification of local wound and infection conditions. Geriatric aspects are addressed in a separate chapter. The degree of fr
