[Gilles de la Tourette's syndrome. Current status of tic disease].

Two French physicians, J.M.G. Itard (1774-1838), then G. Gilles de la Tourette (1857-1904), have individualized a syndrome which has now been universally recognized for almost a century. Modern clinical studies conducted in the United States on a large number of patients have confirmed and completed Gilles de la Tourette's excellent description based on 9 cases. Interpreted at first as psychological, these "caricatures of natural acts" (J.M. Charcot) have given rise to numerous experimental studies in search of a possible imbalance of neurotransmitters. The inconstant or contradictory results

Tonic, phasic and cortical arousal in Gilles de la Tourette's syndrome.

This study explored the hypothesis that Gilles de la Tourette's syndrome involves a disturbance in arousal modulation. The experimental group consisted of 20 unmedicated subjects with the Gilles de la Tourette's syndrome, and the control group of 20 subjects with chronic medical illnesses (haemophilia, von Willebrandt's disease and diabetes). There were differences between groups in the manner in which log conductance level changed over time during sound and light habituation experiments involving mild levels of stimulation with the Gilles de la Tourette group showing less change in arousal le

Gilles de la Tourette syndrome and the neurological basis of obsessions and compulsions.

Gilles de la Tourette syndrome (GTS) is a chronic neurological disorder manifested by involuntary motor tics and vocalizations. Many GTS patients also suffer from obsessions and compulsions. The clinical similarities between GTS and obsessive-compulsive disorder (OCD), their occurrence among members of the same families, and the fact that both can be observed as symptoms of known basal ganglia disturbances suggest that GTS and OCD share common neurological mechanisms. It is hypothesized that the tics and vocalizations of GTS are aberrant manifestations of simple motor programs that are spontan

A clinical study of Gilles de la Tourette syndrome in the United Kingdom.

The clinical features of 53 British-born patients with Gilles de la Tourette syndrome are described. The mean age at onset of body tics was seven years and for vocalisations 11 years. Coprolalia was present in 39%, copropraxia in 21%, echolalia in 46% and echopraxia in 21%. Complicated antics and mannerisms were also common, often involving the compulsive touching of objects or self-injurious behaviour. Forty-six per cent of cases had a family history of tics in a single close relative and in two individuals a further member of the family had Gilles de la Tourette syndrome. Focal dystonia was

Carbon monoxide poisoning with features of Gilles de la Tourette's syndrome.

A previously healthy 58-year-old man had severe carbon monoxide poisoning. Following a comatose state, tics of the head, coprolalia, fits of shouting, and abnormal vocal utterances developed. In addition to the signs of diffuse encephalopathy, he had some of the features associated with idiopathic Gilles de la Tourette's syndrome. The computed tomographic scan showed ventricular enlargement and low-density areas in the basal ganglia.

Neuroanatomy of Gilles de la Tourette's syndrome. Possible midbrain involvement.

Alterations of the dopaminergic system are well documented in Gilles de la Tourette's syndrome (TS). Dopamine (DA)-receptor blockers often relieve symptoms, whereas DA agonists acutely exacerbate them. The cluster of symptoms and known localization of lesions in encephalitis lethargica (EL), together with studies on the anatomy of vocalization, suggest that damage to the periaqueductal gray and midbrain tegmentum may be involved in TS. Pharmacologic findings in patients with TS and EL suggest that oculogyric crises and obsessions are associated with diminished DA levels and the development of

Gilles de la Tourette syndrome. Interactions with other neuropsychiatric disorders.

Gilles de la Tourette syndrome is a neuropsychiatric disease with a childhood onset below age 16, characterized by chronic involuntary movements, obsessions, compulsions, utterances, echolalia, coprolalia and aggressive behavior symptoms. Two unique case histories are described in this report, one with an intercurrent history of primary anorexia nervosa and the other with rheumatic encephalitis. They were successfully treated; the former with chlorimipramine, and the latter with combination of L-tryptophan, nicotinic acid, and pyridoxine HCl. These two cases illustrate the possibility that one

Hypoxanthine guanine phosphoribosyltransferase (HGPRT) in Gilles de la Tourette syndrome.

Hypoxanthine guanine phosphoribosyltransferase (HGPRT) and adenosine phosphoribosyltransferase (APRT) were examined from 11 individuals with Gilles de la Tourette syndrome, 10 of their first- or second-degree relatives, and 3 normal controls. It has been suggested that in some self-mutilating Tourette patients, HGPRT shows a time-related loss of activity at 4 degrees C, and an unusual isoelectrofocusing pattern. Although 3 patients experienced self-mutilation, no consistent abnormalities were found in the temperature-stability of their HGPRT at 4 degrees C and 70 degrees C, or in isoelectrofoc

A comparison of pimozide and haloperidol in the treatment of Gilles de la Tourette's syndrome.

In a double-blind placebo-controlled study, both pimozide and haloperidol significantly decreased tic frequency in nine patients with Gilles de la Tourette's syndrome. These findings are consistent with the catecholamine hypothesis of tic generation. Pimozide was associated with significantly fewer complaints of lethargy. Follow-up 4-20 months later showed that six of seven patients receiving pimozide and one of two receiving haloperidol had had greater than 75% improvement in symptoms.

Significance of genetic factors in Gilles de la Tourette syndrome: a review.

Observations suggesting a genetic basis for Gilles de la Tourette syndrome are reviewed with particular emphasis on the finding of familial aggregation. Studies of both Tourette syndrome and simple tic have found that approximately 30% of patients have a positive family history of tic. The significance of this figure depends on a number of factors, in particular the prevalence of positive tic histories in the population. If the latter figure is 10%, which the best available evidence suggests is a reasonable estimate, approximately 30% of families in the general population would be expected to

Comparison of lithium and haloperidol therapy in Gilles de la Tourette syndrome.

Three patients suffering from Gilles de la Tourette Syndrome were initially treated with haloperidol. Depressive side effects and symptom breakthrough necessitated the search for another agent. The efficacy of lithium carbonate in treating stereotyped hyperkinetic behavior (such as is seen in Gilles de la Tourette syndrome) prompted the evaluation of lithium carbonate. An objective behavioral observation technique, clinical ratings and the patient's subjective reports were used to systematically record the response to treatment during the entire course of the study. Initially blood plasma Li+

The use of methylphenidate and imipramine in Gilles de la Tourette's disease in children.

The authors treated two children with Gilles de la Tourette's disease and one child with a probable combination of Gilles de la Tourette's disease and minimal brain dysfunction with methylphenidate, which resulted in exacerbation of symptoms. One child was briefly treated with imipramine, with similar results. The authors suggest that these pharmacologic agents should be used cautiously in treating patients with possible movement disorders (tics) and conclude that their findings lend support to the catecholamine hypothesis of the etiology of Gilles de la Tourette's disease.

A study of endogenous dopamine metabolism in Gilles de la Tourette's disease.

This longitudinal, blind study of Gilles de la Tourette's disease in a 44 year old male patient who was nonresponsive to Haloperidol therapy indicates that dopamine excretion is related to clinical response. An increased urinary excretion of dopamine and some of its metabolites was associated with the failure of Haloperidol therapy. Imipramine, administered to treat the patients' depressive mood that emerged in the course of treatment, decreased the urinary excretion of dopamine and moderately alleviated the symptoms of Tourette's syndrome. The results suggest that monitering urinary dopamine

Two cases of Gilles de la Tourette's syndrome treated with haloperidol.

Two cases of Gilles de la Tourette's syndrome occurring in Sri Lanka are reported. Both patients had the characteristics of the syndrome as described by Fernando (1967): (1) Childhood onset (below 16 years of age); (2) Multiple motor tics; (3) Unprovoked vocal utterances which may progress to coprolalia. Both responded to haloperidol, withdrawal of medication being followed by relapse, and reintroduction by remission. The literature on the aetiology of the condition has been reviewed. The weight of evidence favours an organic cause, although psychological precipitation cannot be ruled out.
