In 42 insulin-dependent type-I diabetics (age: 25.1 +/- 6 years; history of diabetes: 6.7 +/- 6.6 years) and in 118 control patients (age: 26.1 +/- 5.7 years) the oral health status including a periodontal examination was assessed. In addition, diabetic patients served to study the microflora in gingival pockets and on the mucosa of the cheek. The dental examination comprised the assessment of oral hygiene (Approximal Plaque Index, API), gingival inflammation (Sulcus Bleeding Index, SBI, and Gingival Index, GI) and of loss of attachment. Special medical history forms were used to gather inform

BACKGROUND: The authors examined the tongue bacteria associated with oral halitosis (bad breath originating from the oral cavity), focusing on noncultivable bacteria-bacteria that cannot be identified by bacterial culture techniques.
METHODS: The authors took samples from the dorsal tongue surface of eight adult subjects with halitosis and five control subjects who did not have halitosis. They identified the bacteria in these samples by using both anaerobic culture and direct amplification of 16S ribosomal DNA, a method that can identify both cultivable and noncultivable microorganisms. They

Lactobacilli are Gram-positive, facultatively aerobic, rod-shaped bacteria that are a normal part of the human microbiota and rarely cause infections in immunocompetent hosts. Lactobacillus casei and Lactobacillus rhamnosus are the most common human pathogens within this genus and are also frequently used in probiotics. Both species are inherently resistant to vancomycin, and penicillin-sometimes in combination with aminoglycosides-remains the primary treatment for infections caused by these organisms. Management becomes particularly challenging in patients with severe penicillin allergies, su

Erysipelothrix rhusiopathiae is a thin, non-sporulating, gram-positive rod found worldwide and hosted by many domestic or savage animals, birds, fish, and shell/fish. It is extremely resistant in the external environment (soil, surface waters). Human infection is prevalently related to occupational exposure to sick or infected contaminated animals (anthropozoonosis). Frequently, the disease manifests itself as an acute cellulitis on the skin around the gate of entry. Systemic infection is uncommon, most commonly involving the endocardium or joints. We present a case of subacute E. rhusiopathia

Erysipelothrix rhusiopathiae is a Gram-positive rod, carried by many domestic and pet animals and very resistant in the environmental habitat, causing an anthropo-zoonosis infection in humans. It can determine, most frequently, a skin infection and may cause also septic arthritis or systemic infection, usually associated with aortic endocarditis. Bacteremia without endocarditis is a very rare presentation, generally seen in immunocompromised patients. We report such an unexpected diagnosis in a 75-years old woman, with mitral regurgitation, who was investigated for a persistent febrile syndrom

BACKGROUND: Despite its well-established limitations, culture remains the gold standard for microbial identification in periprosthetic joint infection (PJI). However, there are no benchmarks for the time to positivity (TTP) on culture for specific microorganisms. This study aimed to determine the TTP for pathogens commonly encountered in PJI.
METHODS: This retrospective, multicenter study reviewed prospectively maintained institutional PJI databases to identify patients who underwent hip or knee revision arthroplasty from 2017 to 2021 at 2 tertiary centers in the United States and Germany. On

Blood culture remains the gold standard for identifying bloodstream infections caused by bacteria and fungi. Isolation of the culprit microorganism onto agar plates also facilitates antimicrobial susceptibility testing. The purpose of this study was to determine the contamination rates, pathogen profile, and antimicrobial resistance in a secondary healthcare setting in a two-year timeframe. In this study, data regarding blood cultures of the years 2023 and 2024 were retrospectively analyzed to address the above questions. Blood cultures were incubated for seven days before being discarded as n

Whipple's disease is caused by a ubiquitous Gram-positive bacillus, Tropheryma whipplei. The disease is extremely rare, with only 1,000 cases reported worldwide. Classic Whipple's disease is characterized by a multisystemic involvement with joint (arthralgias) and gastrointestinal (abdominal pain, diarrhea, and weight loss) symptoms. We present a case of a 48-year-old male who had a constitutional syndrome associated with an exuberant bilateral pleural effusion. The small bowel biopsy identified a rod-shaped bacterial cologne in the macrophage cytoplasm, positive for periodic acid-Schiff (PAS

Bacillus cereus is a Gram-positive aerobic or facultatively anaerobic spore-forming rod, which usually causes food poisoning. Its recognition as a pathogen in neonates has increased over the past two decades. The clinical course of a neonate (gestation 24 weeks) with B. cereus infection refractory to therapy is described. Death occurred after withdrawal of support following persistently positive blood and bone marrow cultures despite therapy with vancomycin, gentamicin, imipenum, clindamycin, ciprofloxacillin, immunoglobulin and granulocyte colony stimulating factor over a period of 49 days. In a controlled clinical comparison, three commercial blood culture systems--the standard aerobic BacT/Alert bottle (STD), the aerobic BacT/Alert FAN bottle (FAN) and the Isolator system (ISO; Wampole Laboratories, USA) were compared for their ability to detect aerobic and facultatively anaerobic microorganisms. A total of 945 BacT/Alert (STD and FAN) blood culture sets were compared. Of these, 110 blood culture sets (11.6%) yielded growth of 116 clinically significant bacterial and fungal isolates. Microorganisms were recovered from 10.7% (101/945) of the FAN bottles compared to 8.9% (84/945)

The Herbaspirillum species are gram-negative bacteria that inhabit soil and water. Infections caused by this pathogen are an uncommon clinical entity. We describe a rare case of septic shock and bacteremia caused by Herbaspirillum huttiense in an immunocompetent adult female. The patient, a 59-year-old female, presented to the hospital with circulatory shock, fever, chills, and cough. Chest x-ray revealed right lower lobe lung consolidation consistent with pneumonia, and blood cultures with a positive concerning gram-negative curved rod which was later identified as H. huttiense. The patient w

A pink pigment-producing bacterial strain was isolated from wastewater and identified as Paenibacillus sp. BPW19. The motile bacterial strain was Gram-positive, acid fermenting, glucose, sucrose utilizing and rod-shaped with an average cell length of 1.55 μm as studied under the Environmental Scanning Electron Microscope. Even though being psychrotolerant, the cell growth condition of BPW19 was optimized as 25 ºC along with pH 8, and 2.25% inoculum concentration considering the operational ease of the production. Sonication assisted solvent extraction produced 5.41% crude pigment which showed

Pasteurella multocida, a small gram-negative rod, is a domestic animal saprophyte that occasionally causes disease in humans. Infectious arthritis may develop from a superficial animal bite or scratch. Nine previous cases of infectious arthritis due to this organism have been documented in the literature, and a tenth case is reported here. Most patients had recent animal exposure, and half the patients had underlying chronic rheumatoid arthritis. Clinical signs of inflammation were consistently present; however, systemic infection was infrequent. The lack of positive synovial fluid gram-stain
