Random or predictable?: Adoption patterns of chronic care management practices in physician organizations.

BACKGROUND: Theories, models, and frameworks used by implementation science, including Diffusion of Innovations, tend to focus on the adoption of one innovation, when often organizations may be facing multiple simultaneous adoption decisions. For instance, despite evidence that care management practices (CMPs) are helpful in managing chronic illness, there is still uneven adoption by physician organizations. This exploratory paper leverages this natural variation in uptake to describe inter-organizational patterns in adoption of CMPs and to better understand how adoption choices may be related

Preventing Mistransfusions: An Evaluation of Institutional Knowledge and a Response.

BACKGROUND: Blood product mistransfusions occur when a process error causes transfusion of incompatible blood products. These events are known sources of negative patient outcomes. One such event demonstrated an institutional knowledge gap and an opportunity to reduce this source of transfusion errors. The focus of this study was to evaluate the application of point of care cognitive aids to bridge potentially lethal knowledge gaps in blood product to patient compatibility. METHODS: A patient-donor ABO antigen compatibility grid for red blood cells (RBC) and fresh frozen plasma (FFP) was deve

Characterization of academic cardiothoracic surgeons who started as attendings in private or community practice.

BACKGROUND: Surgeons are traditionally categorized as working either in academic or private/community practice, but some transition between the two environments. Here, we profile current academic cardiothoracic surgeons who began their attending careers in private or community practice. We hypothesized that research activity may distinguish cardiothoracic surgeons who started in non-academic versus academic practice. METHODS: Publicly available data regarding professional history and research productivity were collected for 992 academic cardiothoracic surgeons on faculty at the 77 cardiothora

Applying molecular immunohematology discoveries to standards of practice in blood banks: now is the time.

Lessons from more than 100 years of immunohematology exemplify that many critical discoveries were made serendipitously and their more rapid implementation could have benefited transfusion recipients and pregnancies. Constituents of blood that are not essential for the attempted therapeutic benefit of a transfusion are largely removed from today's blood products. We are now moving on to avoid unnecessary exposure to potentially harmful constituents of the therapeutically required cells, like blood group antigens that are foreign to the patient. Cost efficacy needs to be kept in mind but may ev

Developing evidence-based health education resources: a participatory and iterative approach applied to the one health concept.

INTRODUCTION: The development of health education resources frequently relies on a top-down approach centered on scientific or institutional expertise, which may not always reflect the needs of end users. In a context marked by the emergence of the One Health concept and its forthcoming integration into French school curricula, the development of appropriate educational resources represents a major challenge. This study aimed to describe a participatory, iterative and co-constructive approach to the development of evidence-based educational resources within the e-Bug program. METHODS: Resourc

Group battles its way to anesthesiology department.

This is a story of the evolution of anesthesiology services in a small Mid-America town in which there is a developing multispecialty group. The location and author are not identified, because to do so would almost certainly intensify the hostility that still remains some years after the effort by the group to establish an anesthesiology department was successfully completed. The account is offered in the hope that other groups may recognize their situations and reach similar solution, or be encouraged to attempt such an approach to anesthesiology services.

A logic model for alternative response programs to guide public health research and practice.

Policing and incarceration in the United States are well-documented drivers of health inequities, disproportionately harming marginalized communities. Alternative response programs-unarmed, non-police response-are promising public health strategies to reduce these harms. This study presents the development of a logic model to guide the design and evaluation of such programs. Through semi-structured interviews with 11 staff members across six independent alternative response programs and a focus group with seven expert practitioners and scholars, we identified pathways by which these programs c

Navigating Complexities: Vitamin D, Skin Pigmentation, and Race.

Vitamin D plays a critical role in many physiological functions, including calcium metabolism and musculoskeletal health. This commentary aims to explore the intricate relationships among skin complexion, race, and 25-hydroxyvitamin D (25[OH]D) levels, focusing on challenges the Endocrine Society encountered during clinical practice guideline development. Given that increased melanin content reduces 25(OH)D production in the skin in response to UV light, the guideline development panel addressed the potential role for 25(OH)D screening in individuals with dark skin complexion. The panel discov

Assessing Tumor-Infiltrating Lymphocytes in Solid Tumors: A Practical Review for Pathologists and Proposal for a Standardized Method from the International Immuno-Oncology Biomarkers Working Group: Part 2: TILs in Melanoma, Gastrointestinal Tract Carcinomas, Non-Small Cell Lung Carcinoma and Mesothelioma, Endometrial and Ovarian Carcinomas, Squamous Cell Carcinoma of the Head and Neck, Genitourinary Carcinomas, and Primary Brain Tumors.

Assessment of the immune response to tumors is growing in importance as the prognostic implications of this response are increasingly recognized, and as immunotherapies are evaluated and implemented in different tumor types. However, many different approaches can be used to assess and describe the immune response, which limits efforts at implementation as a routine clinical biomarker. In part 1 of this review, we have proposed a standardized methodology to assess tumor-infiltrating lymphocytes (TILs) in solid tumors, based on the International Immuno-Oncology Biomarkers Working Group guideline

ABO-incompatible kidney transplantation with high isoagglutinin titres: practice pattern and problem solving: a German multi-centre survey.

INTRODUCTION: ABO-incompatible kidney transplantation (ABOi-KT) expands the donor pool, yet baseline anti-ABO isoagglutinin titres ≥ 1:512 remain challenging and practice is highly variable. We surveyed the German transplant centres to capture current management of very high-titre ABOi candidates and centre responses to insufficient titre decline. METHODS: Thirty-six centres were invited to complete an anonymised 19-item web-based questionnaire (January-March 2025) on (i) centre activity, (ii) isoagglutinin testing and isoagglutinin titre limits (iii) desensitisation protocol, (iv) ABO abort

Determinants of adherence towards tuberculosis guidelines among Indonesian private practitioners: a qualitative study.

INTRODUCTION: In high tuberculosis (TB) burden countries, the private sector manages a large proportion of initial visits by presumptive patients with TB. In Indonesia, the second largest contributor of TB cases globally, private practitioners (PPs) often do not adhere to national TB guidelines. A district public-private mix programme to mitigate this issue was started in 2019, yet engagement remains low. This study examines what affects the decisions of Indonesian PPs regarding TB management. METHODS: We performed semistructured in-depth interviews and focus group discussions (FGDs) with 24
