Revisiting subcallosal cingulate deep brain stimulation for depression: Long-term safety and effectiveness outcomes from a pooled analysis of 172 implanted patients.

OBJECTIVE: Deep brain stimulation (DBS) of the subcallosal cingulate (SCC) has been investigated clinically for treatment-resistant depression (TRD). Although the initial BROADEN study, a randomized controlled trial, was halted after an interim futility analysis, observation of long-term follow-up (LTFU) data from this and other cohorts demonstrated sustained improvement in depressive symptoms, prompting further investigation of DBS as a therapeutic option. METHODS: Data from 5 studies, including BROADEN, were used to evaluate the long-term safety and effectiveness of SCC DBS for TRD. Effecti

Neural correlates of self-advantage effect in mental time travel in individuals with high levels of schizotypal traits: A resting-state functional connectivity study.

AIM: Mental time travel (MTT) is a cognitive capacity to mentally re-experience past events and anticipate future possibilities. Self-related MTT events have stronger vividness and sense of experience than events related to others, i.e., the "self-advantage effect". This effect is diminished in individuals with high schizotypal traits (HST) who are at risk for schizophrenia. However, the neural correlates of this effect remain unclear. The present study aimed to examine this issue through resting-state functional magnetic resonance imaging (rs-fMRI). METHODS: Thirty-nine individuals with HST

CannChange: a protocol for a feasibility study using fMRI-based neurofeedback to change the neurobiology of craving in cannabis use disorder.

INTRODUCTION: Cannabis use disorder (CUD) affects ∼33 million people globally and can be underscored by intense cravings to use cannabis, which can trigger compulsive use and relapse. Functional MRI (fMRI) evidence demonstrates hyperactivity of addiction brain pathways during cannabis cue-reactivity, consistent with prominent neuroscientific theories of addiction, particularly within the anterior cingulate cortex (ACC). The ACC also emerges as a key region of real-time fMRI-based neurofeedback (fMRI-neurofeedback) studies demonstrating voluntary changes during cravings in persons who use subst

Brain Connectivity of the Cingulate Cortex in Alcohol Use Disorder: Exploring Its Association With Mindfulness.

Alcohol use disorder (AUD) represents a significant challenge in mental health. Severe AUD is characterized by uncontrolled alcohol consumption and is associated with dysregulation in brain circuits responsible for reward, motivation, decision-making, affect, and stress response. Mindfulness is known to positively influence those dysregulations and may enhance abstinence-related self-efficacy (confidence in resisting alcohol consumption), which is one of the best predictors for abstinence following inpatient treatment. Large-scale networks underlie mindfulness, including the default mode and s

Observational activation of anterior cingulate cortical neurons coordinates hippocampal replay in social learning.

Social learning enables a subject to make decisions by observing the actions of another. How neural circuits acquire relevant information during observation to guide subsequent behavior is unknown. Utilizing an observational spatial working memory task, we show that neurons in the rat anterior cingulate cortex (ACC) associated with spatial trajectories during self-running in a maze are reactivated when observing another rat running the same maze. The observation-induced ACC activities are reduced in error trials and are correlated with activities of hippocampal place cells representing the sam

Anterior cingulate cortex dysfunction underlies anxiety-like behaviors in MT2-deficient mice.

Anxiety disorders are prevalent mental health conditions with significant impacts on quality of life. While the melatonin type 2 receptor (MT2) has been implicated in anxiety, its specific role and neural mechanisms remain unclear. This study investigates the deficiency of MT2 induces robust anxiety-like phenotypes in mice, mediated through anterior cingulate cortex (ACC) dysfunction. Global knockout (MT2-KO) and conditional ACC-targeted deletion (MT2-cKO) models consistently demonstrated increased anxiety responses in standardized behavioral batteries, establishing ACC MT2 as a key neuromodul

A safety and feasibility randomized placebo controlled trial exploring electroencephalographic effective connectivity neurofeedback treatment for fibromyalgia.

Fibromyalgia is a chronic pain condition contributing to significant disability worldwide. Neuroimaging studies identify abnormal effective connectivity between cortical areas responsible for descending pain modulation (pregenual anterior cingulate cortex, pgACC) and sensory components of pain experience (primary somatosensory cortex, S1). Neurofeedback, a brain-computer interface technique, can normalise dysfunctional brain activity, thereby improving pain and function. This study evaluates the safety, feasibility, and acceptability of a novel electroencephalography-based neurofeedback traini

General mechanisms of task engagement in the primate frontal cortex.

Staying engaged is necessary to maintain goal-directed behaviors. Despite this, engagement exhibits continuous, intrinsic fluctuations. Even in experimental settings, animals, unlike most humans, repeatedly and spontaneously move between periods of complete task engagement and disengagement. We, therefore, looked at behavior in male macaques (macaca mulatta) in four tasks while recording fMRI signals. We identified consistent autocorrelation in task disengagement. This made it possible to build models capturing task-independent engagement. We identified task general patterns of neural activity

Reduced functional connectivity of the right dorsolateral prefrontal cortex at rest in obsessive-compulsive disorder.

BACKGROUND: Neuroimaging studies have revealed the role of the right dorsolateral prefrontal cortex (DLPFC) in the neurobiological mechanism of obsessive-compulsive disorder (OCD). However, only a few studies have examined the functional connectivity (FC) pattern of the right DLPFC at rest in OCD. OBJECTIVE: The aim of this research is to examine the FC patterns of the right DLPFC at rest in OCD. METHODS: Twenty-eight medication-free patients with OCD and 20 healthy controls underwent resting-state functional magnetic resonance imaging. Seed-based FC and support vector machine (SVM) were use

The memory trace of an intrusive trauma-analog episode.

Intrusive memories are a core symptom of posttraumatic stress disorder. Compared with memories of everyday events, they are characterized by several seemingly contradictory features: intrusive memories contain distinct sensory and emotional details of the traumatic event and can be triggered by various perceptually similar cues, but they are poorly integrated into conceptual memory. Here, we conduct exploratory whole-brain analyses to investigate the neural representations of trauma-analog experiences and how they are reactivated during memory intrusions. We show that trauma-analog movies indu

Cognitive and Salience Network Connectivity Changes following a Single Season of Repetitive Head Impact Exposure in High School Football.

BACKGROUND AND PURPOSE: During a season of high school football, adolescents with actively developing brains experience a considerable number of head impacts. Our aim was to determine whether repetitive head impacts in the absence of a clinically diagnosed concussion during a season of high school football produce changes in cognitive performance or functional connectivity of the salience network and its central hub, the dorsal anterior cingulate cortex. MATERIALS AND METHODS: Football players were instrumented with the Head Impact Telemetry System during all practices and games, and the helm

High-frequency terahertz stimulation alleviates neuropathic pain by inhibiting the pyramidal neuron activity in the anterior cingulate cortex of mice.

Neuropathic pain (NP) is caused by a lesion or disease of the somatosensory system and is characterized by abnormal hypersensitivity to stimuli and nociceptive responses to non-noxious stimuli, affecting approximately 7-10% of the general population. However, current first-line drugs like non-steroidal anti-inflammatory agents and opioids have limitations, including dose-limiting side effects, dependence, and tolerability issues. Therefore, developing new interventions for the management of NP is urgent. In this study, we discovered that the high-frequency terahertz stimulation (HFTS) at appro

A regulatory role of the medial septum in the chloroquine-induced acute itch through local GABAergic system and GABAergic pathway to the anterior cingulate cortex.

Itch, a common somatic sensation, serves as a crucial protective system. Recent studies have unraveled the neural mechanisms of itch at peripheral, spinal cord as well as cerebral levels. However, a comprehensive understanding of the central mechanism governing itch transmission and regulation remains elusive. Here, we report the role of the medial septum (MS), an integral component of the basal forebrain, in modulating the acute itch processing. The increases in c-Fos+ neurons and calcium signals within the MS during acute itch processing were observed. Pharmacogenetic activation manipulation

Abnormal functional hubs in migraine patients: a resting-state MRI analysis about voxel-wise degree centrality.

The purpose was to explore the spatial centrality of the whole brain functional network related to migraine and to investigate the potential functional hubs associated with migraine. 32 migraine patients and 55 healthy controls were recruited and they received resting-state functional magnetic resonance imaging voluntarily. Voxel-wise Degree Centrality (DC) was measured across the whole brain, and group differences in DC were compared. False Discovery Rate and permutation test (5000 times) were used for multiple comparisons. Finally, significant differences in functional connectivity (FC) betw

Deconstruction the feedforward inhibition changes in the layer III of anterior cingulate cortex after peripheral nerve injury.

The anterior cingulate cortex (ACC) is one of the critical brain areas for processing noxious information. Previous studies showed that peripheral nerve injury induced broad changes in the ACC, contributing to pain hypersensitivity. The neurons in layer 3 (L3) of the ACC receive the inputs from the mediodorsal thalamus (MD) and form the feedforward inhibition (FFI) microcircuits. The effects of peripheral nerve injury on the MD-driven FFI in L3 of ACC are unknown. In our study, we record the enhanced excitatory synaptic transmissions from the MD to L3 of the ACC in mice with common peroneal ne

Phenotyping Superagers Using Resting-State fMRI.

BACKGROUND AND PURPOSE: Superagers are defined as older adults with episodic memory performance similar or superior to that in middle-aged adults. This study aimed to investigate the key differences in discriminative networks and their main nodes between superagers and cognitively average elderly controls. In addition, we sought to explore differences in sensitivity in detecting these functional activities across the networks at 3T and 7T MR imaging fields. MATERIALS AND METHODS: Fifty-five subjects 80 years of age or older were screened using a detailed neuropsychological protocol, and 31 pa

Lateralization of the cerebral network of inhibition in children before and after cognitive training.

Inhibitory control (IC) plays a critical role in cognitive and socio-emotional development. IC relies on a lateralized cortico-subcortical brain network including the inferior frontal cortex, anterior parts of insula, anterior cingulate cortex, caudate nucleus and putamen. Brain asymmetries play a critical role for IC efficiency. In parallel to age-related changes, IC can be improved following training. The aim of this study was to (1) assess the lateralization of IC network in children (N = 60, 9-10 y.o.) and (2) examine possible changes in neural asymmetry of this network from anatomical (st

Presymptomatic and early pathological features of MAPT-associated frontotemporal lobar degeneration.

Early pathological features of frontotemporal lobar degeneration (FTLD) due to MAPT pathogenic variants (FTLD-MAPT) are understudied, since early-stage tissue is rarely available. Here, we report unique pathological data from three presymptomatic/early-stage MAPT variant carriers (FTLD Clinical Dementia Rating [FTLD-CDR] = 0-1). We examined neuronal degeneration semi-quantitatively and digitally quantified tau burden in 18 grey matter (9 cortical, 9 subcortical) and 13 white matter (9 cortical, 4 subcortical) regions. We compared presymptomatic/early-stage pathology to an intermediate/end-stag

Hooked on a thought: Associations between rumination and neural responses to social rejection in adolescent girls.

Rumination is a significant risk factor for psychopathology in adolescent girls and is associated with heightened and prolonged physiological arousal following social rejection. However, no study has examined how rumination relates to neural responses to social rejection in adolescent girls; thus, the current study aimed to address this gap. Adolescent girls (N = 116; ages 16.95-19.09) self-reported on their rumination tendency and completed a social evaluation fMRI task where they received fictitious feedback (acceptance, rejection) from peers they liked or disliked. Rejection-related neural

The Neural Bases of Action-Outcome Learning in Humans.

From an associative perspective the acquisition of new goal-directed actions requires the encoding of specific action-outcome (AO) associations and, therefore, sensitivity to the validity of an action as a predictor of a specific outcome relative to other events. Although competitive architectures have been proposed within associative learning theory to achieve this kind of identity-based selection, whether and how these architectures are implemented by the brain is still a matter of conjecture. To investigate this issue, we trained human participants to encode various AO associations while un
