A rare case of Streptomyces fradiae hand actinomycetoma.

Actinomycetoma is a rare, chronic, and progressive bacterial and fungal infection that affects subcutaneous tissues, fascia, and bones. This a case of a 27-year-old Sudanese male, shepherd, with a mycetoma lesion in the right hand caused by Streptomyces fradiae. He presented with swelling and deformity of the hand, and subsequent imaging showed soft tissue swelling, deep infiltration of the hand and sinus formation. A skin biopsy of the lesion confirmed the diagnosis of actinomycetoma, and the patient was treated with ceftriaxone IV for 14 days followed by oral amoxicillin clavulanic acid for

Endosteal new bone formation in the long bones of adjuvant treated rats.

Histopathological changes were examined mainly in the diaphyseal parts of long bones, especially femur in adjuvant-treated male Lewis-SPF rats, with reference to clinical symptoms of chronic osteoarthritis. The diaphyseal bone marrow of long bones in these rats sequentially showed three different processes of chronic pathological changes, which, however, partly overlapped each other. Initially, multiple epitheloid cell granulomas or granulomatous lesions containing fibrin deposits began to appear in the marrow space about 22 days after adjuvant injection, when the joint score of arthritis reac

Unusual metastatic patterns of urologic malignancies: a case series and literature review.

BACKGROUND: Urologic cancers typically metastasize to common organs, while spread to sites such as the heart, hand, cranial bones, penis, or tonsil is exceedingly rare. The clinical relevance and prognostic impact of these unusual metastases remain unclear. This case series highlights six exceptional cases, providing unique insights to support awareness and management. CASE DESCRIPTION: We retrospectively analyzed six cases of patients diagnosed with urologic cancers who developed metastases at uncommon sites. Each case was evaluated based on clinical presentation, imaging findings, histopath

Low velocity hand-gun injuries of the maxillofacial region.

In contrast to the number of reports in the literature describing high velocity injuries of the maxillo-facial region, there are relatively few which deal with civilian type gunshot injuries. The purpose of this study was to undertake a retrospective analysis of 40 cases of low-velocity hand-gun injuries of the maxillo-facial region. The majority of cases were due to assault followed by accidental shootings and failed suicide attempts. Injuries ranged from mild soft tissue damage without fractures to severe, comminuted fractures of the facial bones. In some cases, soft tissue damage was severe

Intra-Articular Osteoid Osteoma of the Trapezoid Bone: A Rare Presentation Mimicking Wrist Synovitis.

Osteoid osteomas predominantly occur in the cortices of long bones, with the femur and tibia being the most commonly affected sites. However, they can occasionally present in atypical locations, such as the carpus, which can lead to diagnostic confusion with other conditions. This case report details an intraarticular osteoid osteoma in the trapezoid bone. Initial evaluations, including standard radiographs, joint ultrasound, and wrist MRI performed twice, initially pointed toward a diagnosis of wrist synovitis. This case underscores the diagnostic challenges posed by atypical presentations of

[Complications of the finger osteosynthesis using Kirschner's wires in arterial inflow disorder].

At an osteosynthesis of Kirshner wires to one of widespread complications is their spontaneous migration that leads to formation of bone deformation or to necessity of application of less effective kinds of an immobilization. In posttraumatic change of arterial inflow to hand and fingers leads to an oxygen level reduction in tissues, their chronic ischemia and to increase of a lactate and pyruvate contents in peripheral blood that can influence on Kirshner wires. Spontaneous migration of Kirshner wires from bones of fingers and decrease of their fixing in the bones is characteristic for patien

Carpometacarpal Joint Fracture Dislocation of Second to Fifth Finger.

BACKGROUND: Carpometacarpal joint fracture dislocation of the second to fifth finger is a rare hand injury associated with high energy trauma. Due to severe swelling and overlapping of bones on the radiograph of wrist-hand, dislocations are missed. We reported a series of six patients with rare carpometacarpal joint fracture dislocation treated with open reduction. METHODS: We retrospectively studied six cases of carpometacarpal joint fracture dislocation. All patients were treated with open reduction and internal fixation with Kirschner wire. Functional assessment was done with Quick Disabil

[Surgical treatment of hand deformities in multiple enchondromatosis: a case report].

Multiple enchondromatosis (Ollier's disease) is a rare disease characterized by widespread enchondromas. In general, the short tubular bones of the hand are involved, with progressive lesions resulting in cosmetic problems and functional deformities. Diaphysectomy and reconstruction with structural autografts or allografts are usually recommended in the treatment of extensive enchondromas involving the fingers. Curettage and grafting and ray amputation are other surgical procedures that can be applied depending on the severity of involvement. A 25-year-old woman with enchondromatosis presented

X-chromosomally inherited split-hand/split-foot anomaly in a Pakistani kindred.

A Pakistani kindred comprising seven generations and 36 members with the split-hand/split-foot anomaly is described. The full expression of the trait, monodactylous or split hand and split foot, mainly of the lobster-claw type, was present in 33 males and 3 females. Other females showed a distinctly milder expression of the trait, usually in the form of partial syndactyly, metacarpal and phalangeal hypoplasia, and malformation. The distribution of the affected members in the pedigree is compatible with X-chromosomal inheritance. Hemizygous males and presumably homozygous females exhibit the ty

Novel Mini-Invasive Surgical Technique for Treating Fifth Metacarpal Neck Fractures: A Case Report.

BACKGROUND Fracture of the fifth metacarpal of the hand is due to trauma to the clenched fist. The non-displaced fracture can be treated by splinting and immobilization, but fracture dislocation requires individualized management to ensure the return of function. The Jahss maneuver for reduction of volar displaced metacarpal neck fractures involves flexion of the metacarpophalangeal and proximal interphalangeal joint at 90°, with the proximal phalanx used to reduce the metacarpal head. This report is of a 25-year-old male Italian pianist with a displaced fifth metacarpal neck fracture successf

The Krukenberg procedure: a method of treatment for unilateral anomalies of the upper limb in Chinese children.

Eight Chinese patients with congenital absence of the hand were treated with the Krukenberg procedure to provide a pinching function of the forearm bones. The rehabilitation program was simple and the functional results were satisfactory. We have selected three cases to illustrate the Krukenberg procedure and the indications for this operation, a procedure that is particularly suitable for the Chinese who are culturally adapted to chopstick functions.

Avascular Necrosis of an Adolescent Distal Radius: A Literature Review.

A 12-year-old boy was tackled mid-air, resulting in a fall on the outstretched hand without fracture. The patient was treated conservatively but developed sharp pain and stiffness 6 months later. Imaging revealed distal radius avascular necrosis with physeal involvement. Owing to the injury chronicity and location, we treated the patient conservatively with hand therapy. After 1 year of therapy, the patient returned to normal activities without pain and with a resolution of findings on imaging. Avascular necrosis is more frequent in carpal bones (eg, Kienböck disease of the lunate and Preiser

Septic Mycobacterium Avium Intracellulare Extensor Tenosynovitis of the Wrist.

Mycobacterium avium intracellulare (MAI) infections of the hand, wrist, and upper extremity are rare, but potentially devastating atypical mycobacterial infections that can affect tendon, bone, and other soft tissues of the musculoskeletal system. We present an immunocompromised patient presenting with acute swelling and pain in the dorsum of the hand and wrist that underwent a wrist extensor tenosynovectomy with intraoperative cultures revealing infection with MAI. The patient developed severe progression of the infection with osteomyelitis of the distal forearm and carpal bones, multiple sub

Sauropods.

This article begins as many others on sauropods before it: "Sauropod dinosaurs were the largest animals to ever walk the Earth, by far". The largest sauropods were easily four times heavier than the largest land mammals (and the largest other dinosaur species, for that matter). The iconic body plan of sauropods is dominated by their very long neck, in some species exceeding 14 meters in length, provided with a relatively small head (Figure 1). The neck was mostly held horizontally or at a low angle. The massive but relatively short trunk was supported by four columnar legs, much like in an ele
