Management of displaced supracondylar fractures of the humerus using lateral versus crossed Kirschner wires: a prospective randomized controlled clinical trial.

AIMS: We sought to evaluate whether loss of reduction in lateral Kirschner (K)-wiring is noninferior to crossed K-wiring in closed reduction and percutaneous pinning of Type III supracondylar humerus fractures. METHODS: A prospective, randomized, non-inferiority trial was conducted. Emergency department patients aged three to seven years with Type III supracondylar humerus fractures were invited to participate with informed consent. Block randomization allocated patients into one of two pin configuration groups (lateral or crossed). Surgical technique and postoperative management were standar

Effect of reconstruction plate removal on dental implants in fibula flap mandibles: A biomechanical and clinical study.

Vascularized autologous bone transplantation combined with implant restoration is a preferred method for functional mandibular reconstruction. However, there is currently no consensus on whether internal fixation devices, such as titanium plates and screws, must be removed during the reconstruction process. This study aimed to assess the biomechanical and clinical necessity of removing these fixation devices. Eight patients who underwent mandibular reconstruction with fibula flaps and subsequent dental implantation were included. The study utilized finite element analysis to simulate and compa

Effects of different platform preparation and retrieval techniques on broken file removal and fracture strength.

The aim was to evaluate the effects of different strategies on broken instrument removal and root fracture strength. After canal preparation of anterior single-rooted teeth, the samples were divided into one control group (n = 10) and six experimental groups (n = 15). A #25/.06 rotary file was broken at a length of 4 mm in the middle-third canal region in the experimental groups, and no instrument fractures were created in the control group. Access to the instrument was achieved with Gates-Glidden (GG) burs or ultrasonic tips. Dentin around the instrument was removed to create partial or full

Surgical Treatment of Pediatric Open Forearm Fractures: A Comparison of Outcomes With or Without Internal Fixation.

BACKGROUND: Pediatric patients with open forearm fractures who undergo operative irrigation and debridement (I&D) are often treated with implant fixation due to concern for delayed healing, nonunion, or loss of alignment. For an open forearm fracture treated with I&D, it is unknown whether reduction and casting alone without implant fixation affects outcomes in young patients. We aim to compare the results of pediatric open forearm fractures treated with I&D and cast immobilization with no implant fixation (NIF) to those treated with implant fixation (IF). METHODS: We retrospectively reviewed

Full Tibialis Anterior Muscle Turnover Flap for Distal Tibial Coverage in a High-Risk Patient With Ankle Arthrodesis: A Case Report.

Distal tibial wounds with exposed bone or hardware pose a reconstructive challenge, particularly in medically high-risk patients unsuitable for free flap reconstruction. The tibialis anterior muscle is rarely used for reconstruction because sacrifice of the muscle may result in loss of ankle dorsiflexion. However, in selected patients with ankle arthrodesis, complete tibialis anterior sacrifice may be more acceptable because active ankle dorsiflexion is no longer essential. This case report describes a 66-year-old woman with significant cardiopulmonary comorbidity who developed anterior distal

From the Destruction of Two Lumbar Segments to Thoracic-Lumbar-Pelvic Fusion: A Case Caused by Congenital Insensitivity to Pain with Anhidrosis and Literature Review.

BACKGROUND: Congenital insensitivity to pain with anhidrosis (CIPA) with Charcot arthropathy is a rare combination in orthopaedic clinical practice. The experience dealing with such patients is limited. Here with this case of approximately 10 years follow-up, we wish to shed light on the choices of strategies of surgeries and alerting clinicians with post-surgery complications. The possible underlying reasons for the recurrent Charcot arthropathies as well as strategies for peri-operative management for such surgical cases are also discussed. CASE PRESENTATION: The patient underwent a surgery

Medial lag screw migration in an intramedullary nail combination.

Intramedullary fixation is increasingly used for intertrochanteric fractures in the elderly population. The management of hardware migration and non-union remains a challenge for orthopaedic surgeons, often necessitating complex revision procedures. We report a case of medial lag screw migration with propagation of a reverse oblique neck of femur fracture 8 weeks postoperatively in a patient in their mid-70s with pre-existing paraplegia. A revision procedure was performed. Existing hardware was removed without complication. The lag screw tract was packed with layers of a haemostatic agent to p

Closed reduction and percutaneous stabilization of pediatric T-condylar fractures of the humerus.

T-condylar fractures of the humerus are a rare fracture pattern in children. The usual recommendation is to treat them with open reduction and internal fixation, in accordance to the usual practice in adult fracture patterns. This involves extensive surgical approach to the elbow to allow anatomic reduction and placement of hardware for rigid fixation. The authors present a technique of closed reduction of the intra-articular component of the T-condylar fracture that is stabilized with partially threaded pins that afford interfragmentary compression, followed by the use of two elastic titanium
