Replicating motivational and cognitive-motivational typologies of doping attitudes among recreational fitness athletes: implications for harm reduction and risk communication.

BACKGROUND: The non-medical use of image- and performance-enhancing drugs (IPEDs) in recreational fitness represents an important public health and harm-reduction challenge. While prevalence estimates provide useful benchmarks, they often obscure the motivational and cognitive-motivational heterogeneity underlying substance-use decisions and prevention needs in gym populations. OBJECTIVE: This study re-examined a previously published typology and evaluated its reproducibility of doping attitudes and prevention needs among German recreational fitness athletes using selected constructs from the

Features of Alcohol Harm Reduction Campaigns That Resonate With Older Adults: Insights From an Australian Qualitative Study Using Prompt Videos.

INTRODUCTION: Many countries around the world including Australia, New Zealand, England and the United States of America face increasing alcohol use and alcohol-related harms amongst the ageing population. Despite alcohol being linked to diseases such as cancer and dementia, many conventional alcohol harm reduction campaigns don't resonate with older audiences. This qualitative study aimed to investigate what features of alcohol harm reduction campaigns appeal to a middle-aged and older audience. METHODS: In-depth interviews were conducted with 45 Australian participants aged 54-74 years (mea…

The rising wave of cathinone use in people attending harm reduction facilities: a French repeated cross-sectional study (2019-2023).

BACKGROUND: In Europe, cocaine use has recently been on the rise among people who use drugs (PWUD). In parallel, cathinones, another type of psychostimulant that were once associated within the unique context of chemsex, are now spreading to broader patterns of use. Here, we examined trends in reported drug use, including cathinones, among PWUD attending the main harm reduction center in Lyon, France, between 2019 and 2023. METHODS: We conducted a year-by-year comparison of standardized assessments completed at entry and updated annually for all PWUD attending the harm reduction center. These …

Appraisal of joint tobacco and alcohol tax interventions for harm reduction in the UK: the SYNTAX integrated evidence synthesis project.

BACKGROUND: Alcohol and tobacco are risk factors for non-communicable disease and deaths in England. People using both substances have increased health risks. While increasing tax on each product has been shown to reduce consumption and harm, little research has examined effects of changing both taxes together. OBJECTIVES: Work package 1: describe potential tobacco and alcohol tax policies; work package 2: analyse purchasing patterns across population subgroups; work package 3: estimate industry tax-pass-through; consumer response to price changes for 12 products; work package 4: simulate imp

Perceptions of access to harm reduction services during the COVID-19 pandemic among people who inject drugs in metropolitan Chicago.

BACKGROUND: The COVID-19 pandemic amplified the risk environment for people who inject drugs (PWID), making continued access to harm reduction services imperative. Research has shown that some harm reduction service providers were able to continue to provide services throughout the pandemic. Most of these studies, however, focused on staff perspectives, not those of PWID. Our study examines changes in perceptions of access to harm reduction services (e.g., participant reported difficulty in accessing syringes and naloxone) among PWID participating in a longitudinal study conducted through the …

"We Keep Each Other Safe": The Development and Implementation of a Harm Reduction Health Communication Campaign to Prevent Fentanyl Overdoses.

BackgroundFentanyl overdose deaths rose precipitously in the United States from 2015 to 2022, dramatically outpacing all other drugs. Within Texas, Travis County (Austin, Texas) has the highest fentanyl overdose death rate across the state, surpassing larger metro areas such as Houston and Dallas. Local research indicates that many users are aware of fentanyl in the drug supply, but few utilize harm reduction behaviors, indicating the need locally for improved messaging and education among people who use drugs (PWUD). As such, the local health department approached our team to develop a cultur…

Exploring and describing alcohol harm reduction interventions: a scoping review of literature from the past decade in the western world.

CONTEXT: Regular alcohol use is a predominant risk factor for disease, injury, and social harm. While robust evidence is advocating for implementing interventions to reduce the harms of illegal substance use, less literature is dedicated to identifying and understanding interventions aiming at reducing the various harms associated with alcohol. OBJECTIVES: This review describes how alcohol harm reduction (AHR) interventions are currently conducted and analyzes the facilitators and barriers identified by the studies on their efficacy. METHOD: This scoping review with evidence appraisal includ…

Unmet needs and harm reduction preferences of syringe services program participants: differences by co-use of illicit opioids and methamphetamine.

BACKGROUND: The current fourth wave of the United States opioid overdose epidemic is characterized by the co-use of opioids and stimulants, including illicit opioids and methamphetamine. The co-use of these two drugs, known as "goofballing," is associated with higher risk for several adverse outcomes, including more frequent injections, greater health risks, and higher morbidity. Considering these differences, this unique subpopulation of people who inject drugs (PWID) may also have unique unmet needs and harm reduction preferences. METHODS: We collected self-reported data from participants (…

A Mixed Methods Analysis of Southern HIV Service Organizations' Implementation of Harm Reduction to Address the HIV and Opioid Syndemic.

Southern HIV Service Organizations (SHSOs) are promising sites for the adoption and implementation of harm reduction as a means for addressing the HIV and opioid syndemic. However, little research to date has examined exactly how harm reduction is operationalized within and among SHSOs. Using program evaluation data (i.e., organizational assessment data and semi-structured qualitative group interview data with SHSO staff), this study aimed to characterize organizational implementation of harm reduction among SHSOs that sought harm reduction capacity-building assistance (i.e., training, coachin…

Resilience Through Harm Reduction? Protective Behavioral Strategies as a Factor of Resilience Among Polish Young Adults Using Various Psychoactive Substances.

Resiliency theory might provide a conceptual framework for understanding why adverse substance-related consequences vary considerably among young adults using psychoactive substances. Protective behavioral strategies (PBS) are behaviors that reduce substance use and related consequences. Despite such suggestions, to date no research has explicitly treated PBS as resilience factors. The purpose of this study was to examine compensatory and protective models of resilience among young adults using various substances. Data were obtained from an online questionnaire distributed through online socia

A strategy for the detection of benzodiazepine drugs using low-resolution paper-spray mass spectrometry for harm reduction drug checking.

The ability to detect newly emerging substances is of great importance in reducing harms for people who use drugs. New psychoactive substances including novel benzodiazepines in the illicit drug supply have been linked to high rates of overdose deaths while complicating drug checking as an overdose prevention strategy. Paper-spray mass spectrometry (PS-MS) has emerged as a novel strategy to rapidly detect trace components in street drug samples. While targeted, low-resolution PS-MS methods have proven effective, newly emerging substances are often missed. To address this, a method was applied

Coordination, cooperation, and creativity within harm reduction networks in Iran: COVID-19 prevention and control among people who use drugs.

An unprecedented public health crisis confronts the world. Iran is among the hardest-hit countries, where effects of the COVID-19 pandemic are stretched across society and felt by the most marginalised people. Among people who use drugs, a comprehensive response to the crisis calls for broad collaboration, coordination, and creativity involving multiple government and non-government organisations. This commentary provides early insights into an unfolding experience, demonstrating the operational and policy impact of an initiative, bringing together a diverse array of harm reduction stakeholder…

Access to harm reduction programs among persons who inject drugs: findings from a respondent-driven sampling survey in Tehran, Iran.

BACKGROUND: Over the past two decades, drug injection-related risk behaviors have been the major drivers of the HIV epidemic in Iran. This study assesses the access of people who injected drugs (PWID) to harm reduction services (needle-exchange programs [NEP] and methadone maintenance treatment [MMT]) in Tehran, Iran in 2007, almost five years after the large-scale implementation of these programs. METHODS: 572 consenting PWID (>18 years old, ever injected in the past month, lived in Tehran or its suburbs) were recruited (24 seeds) into a sero-behavioral survey using respondent-driven samplin…

Perceived harm reduction impact of a drug checking service in Vienna: A qualitative study among current and prospective service users.

BACKGROUND AND AIMS: The evolving landscape of illicit drug markets necessitates effective approaches to harm reduction and addiction prevention for people who use drugs (PWUD). In response, integrated drug checking services (DCS), which combine chemical analyses and psychosocial interventions, have been shown to contribute to safer drug use behaviour, particularly in festival settings. However, little is known about how people who predominantly use drugs occasionally engage with and perceive fixed-site DCS and how specific service characteristics contribute to its overall impact. In this stud…
