Situational analysis of health systems for ear and hearing care in the World Health Organization (WHO) Eastern Mediterranean Region: A systematic review and evidence synthesis to inform national policies and strategies.

In the Eastern Mediterranean Region (EMR), 78.1 million people experience hearing loss of any degree, with 22.1 million having disabling hearing loss, projected to reach 51.7 million by 2050. Unless global action is taken, the worldwide burden could reach over 700 million people with disabling hearing loss by 2050. This systematic review presents the first comprehensive health systems analysis of ear and hearing care (EHC) in the region. Following PRISMA guidelines, we analysed 146 articles through the WHO health systems framework to identify systemic barriers to effective EHC integration. Our

Systemic readiness and policy gaps for metabolic syndrome care in botswana prisons: A national WHO building blocks evaluation.

BACKGROUND: Metabolic syndrome is a growing global health challenge, affecting one in four adults worldwide, with prevalence in sub-Saharan Africa estimated at 20-30%. In Botswana, urban prevalence is 27%, reflecting a high burden of NCDs. Prisoners are particularly vulnerable due to overcrowding, poor nutrition, sedentary confinement, and limited access to health services. OBJECTIVES: To evaluate the systemic capacity, effectiveness, and readiness of Botswana's prison health care delivery mechanisms in managing metabolic syndrome, focusing on diagnostic, treatment, and continuity-of-care pro

Reframing spinal cord injury rehabilitation in fragile contexts: a multi-level systems framework for Lebanon.

Background: Spinal cord injury (SCI) requires long-term, multidisciplinary rehabilitation to optimize functioning and participation. In fragile health systems, however, rehabilitation services are often fragmented, under-resourced, and inconsistently accessible. This Current Debate paper examines SCI rehabilitation in Lebanon as an example of how health system fragility constrains rehabilitation delivery and disability inclusion. Methods: Using an International Classification of Functioning, Disability and Health (ICF)-informed approach, we developed a multi-level conceptual framework through s

Managing health systems under armed conflict: a scoping review of international evidence and a document review of Iranian experiences.

Wars and armed conflicts are among the most critical crises that can severely impact a country's vital infrastructures, particularly the health system. During wartime, health systems face major challenges, including attacks on healthcare facilities and personnel, which disrupt access to services and undermine public health. This study aims to conduct a scoping review to assess reported international strategies for wartime health system management, focusing on resource allocation, workforce distribution, financing, and access to essential supplies, followed by a document review of Iranian exper

Balancing the scales? Evaluating the impact of results-based financing on maternal health outcomes and related inequality of opportunity in Zimbabwe.

This study evaluates the impact of results-based financing (RBF) on maternal health outcomes and the inequality of opportunity (IOP) in these outcomes in Zimbabwe. We employ a difference-in-differences approach that leverages the staggered implementation of the programme across 60 districts, exploiting temporal variation in the introduction of RBF and individual-level variation in birth timing. Our analysis uses nationally representative, pooled cross-sectional data from the 2005/2006, 2010/2011, and 2015 Zimbabwe demographic and health surveys. Employing the extended two-way fixed effects (ET

Determinants of adherence towards tuberculosis guidelines among Indonesian private practitioners: a qualitative study.

INTRODUCTION: In high tuberculosis (TB) burden countries, the private sector manages a large proportion of initial visits by presumptive patients with TB. In Indonesia, the second largest contributor of TB cases globally, private practitioners (PPs) often do not adhere to national TB guidelines. A district public-private mix programme to mitigate this issue was started in 2019, yet engagement remains low. This study examines what affects the decisions of Indonesian PPs regarding TB management. METHODS: We performed semistructured in-depth interviews and focus group discussions (FGDs) with 24

Realigning Global Health Realities Towards Children's Surgery: Progress and Possibilities.

Despite the high population of children, increasing surgical disease burden and shortage of pediatric surgeons, as well as limited infrastructure, children's surgical care in low- and middle-income countries (LMICs) has been neglected for decades. This has contributed to unacceptably high morbidity and mortality, long term disabilities and economic loss to families. The work of the global initiative for children's surgery (GICS) has raised the profile and visibility of children's surgery in the global health space. This has been achieved a philosophy of inclusiveness, LMIC participation, focus

Extreme weather events and HIV: development of a conceptual framework through qualitative interviews with people with HIV impacted by the California wildfires and their clinicians.

BACKGROUND: People with HIV (PWH) are disproportionately vulnerable to the impacts of wildfires, given the need for frequent access to healthcare systems, higher burden of comorbidities, higher food insecurity, mental and behavioral health challenges, and challenges of living with HIV in a rural area. In this study, we aim to better understand the pathways through which wildfires impact health outcomes among PWH. METHODS: From October 2021 through February 2022, we conducted individual semi-structured qualitative interviews with PWH impacted by the Northern California wildfires and clinicians

Inpatient care of small and sick newborns: a multi-country analysis of health system bottlenecks and potential solutions.

BACKGROUND: Preterm birth is the leading cause of child death worldwide. Small and sick newborns require timely, high-quality inpatient care to survive. This includes provision of warmth, feeding support, safe oxygen therapy and effective phototherapy with prevention and treatment of infections. Inpatient care for newborns requires dedicated ward space, staffed by health workers with specialist training and skills. Many of the estimated 2.8 million newborns that die every year do not have access to such specialised care. METHODS: The bottleneck analysis tool was applied in 12 countries in Afr

Posterior crossbite corrections in the early mixed dentition with quad helix or rapid maxillary expander: a cost-effectiveness analysis of a randomized controlled trial.

BACKGROUND: Unilateral posterior crossbite is a common malocclusion, and early treatment is recommended to enable normal growth. There are several possibilities regarding choice of appliances used for correcting this malocclusion; however, when treatment is financed by public funds the decision needs to be based not only on the effects but also on the effect in relation to the costs. OBJECTIVES: The aim was to perform a cost-effectiveness analysis comparing quad helix (QH) and rapid maxillary expanders (RME; hyrax-type) in children in the early mixed dentition. Assessing the feasibility of solar-powered digital health infrastructure in rural and peri-urban Kenya: a mixed-methods study.

BACKGROUND: Health systems in low- and middle-income countries (LMICs) face persistent infrastructure constraints, particularly unreliable electricity and limited digital connectivity, that hinder the effective implementation of digital health interventions. These challenges are especially pronounced in rural and peri-urban settings, where facilities rely on unstable power sources and fragmented information systems. Although digital health technologies can strengthen health systems and improve care delivery, their effectiveness depends on reliable infrastructure. Integrating renewable energy w

Lifestyle front offices in dutch hospitals: a cross-sectional survey on similarities, differences and future needs.

BACKGROUND: Since 2019, Dutch hospitals have been developing approaches aiming to bridge the gap between hospital care and community-based lifestyle support. This has led to the introduction of the so called 'Lifestyle Front Office" (LFO) in 2022. The LFO enables healthcare professionals to refer patients for lifestyle and (psycho)social support adjacent to medical treatment in the context of specialist medical care. After a consultation at the LFO, patients are referred to the appropriate community-based lifestyle interventions. LFOs have been quickly adopted in Dutch hospitals, leading to co
