Up to 20% of all postoperative patients with repaired congenital heart disease later develop symptoms of heart failure, but slow disease progression makes patients difficult to study. The incidence is higher in the presence of lesions with a solitary or systemic right ventricle (RV). Several mechanisms cause the development of heart failure in this population, and the treatment options are not always aligned to conventional therapy for heart failure in a biventricular heart with cardiomyopathy. The pathogenesis and options for management of this complex problem in specific high-risk lesions ar…

A case of a two month infant with complex congenital heart disease (aortic coarctation with ventricular septal defect) associated to a cellular brain migration failure is presented. The management strategy consisted on the correction of congenital heart disease by means of a two-stage surgery without a further preoperative evaluation of the neurological status. The patient developed several perioperative complications such as two episodes of cardiac arrest, reconnection to cardiopulmonary bypass, cardiac tamponade, chilothorax and septic shock. A neurological protocol consisting in electroence…

Recently, cavitation on the surface of mechanical heart valves has been studied as a cause of fractures occurring in implanted mechanical heart valves. The cause of cavitation in mechanical heart valves was investigated using the 25 mm Medtronic Hall valve and the 23 mm Omnicarbon valve. Closing of these valves in the mitral position was simulated in an electrohydraulic totally artificial heart. Tests were conducted under physiologic pressures at heart rates from 60 to 100 beats per minute with cardiac outputs from 4.8 to 7.7 L/min. The disk closing motion was measured by a laser displacement …

Medically refractory heart failure may be present in children with cardiomyopathy (CMP) or complex congenital heart disease (CHD). In adults, the surgical management of this condition is either heart transplantation or the Batista operation. From March 1995 to January 2000, a total of 6 children, aged from 1 to 16 years, with medically refractory heart failure associated with CMP or complex CHD underwent cardiac transplantation and one of them also had the Batista operation as a bridge to transplantation. One of the 6 patients died of intractable sepsis 17 days after the operation, but the oth…

BACKGROUND: The association of cardiac and liver disorders has not been extensively outlined in the literature.
METHODS: A survey of the MEDLINE database was performed to assess the current status of research regarding the association between cardiac and liver disorders.
RESULTS: Combined cardiac and hepatic disorders occur in 3 different settings: heart diseases affecting the liver, liver diseases affecting the heart, and cardiac and hepatic disorders with joint etiology. The spectrum of heart diseases affecting the liver includes mild alterations of liver function tests in heart failure, c…

From this detailed analysis of the main dietary trials conducted over the last 30 years in the secondary prevention of coronary heart disease, it can be said that the older trials were conducted on low risk patients and used high fat diets (about 40% of energy as lipids), comprising low saturated fat and cholesterol intake but very high (15 to 20% of energy) polyunsaturated fat intake, particularly from the omega-6 fatty acid family. These experimental diets were designed to reduce blood cholesterol and failed to improve prognosis. By contrast, recent trials were not primarily designed to redu…

Between May 1983 and May 1987, 161 children of all age groups underwent surgery for congenital cardiac malformations without prior cardiac catheterization and angiocardiography. Their diagnoses were established by clinical investigation, electrocardiogram, chest X-ray and cross-sectional echocardiography only in all cases. The most common surgical procedures were ligation of a patent ductus arteriosus (n = 68), correction of aortic coarctation (n = 28), balloon atrial septostomy (n = 27), and closure of an atrial septal defect (n = 23). To prove the efficiency and accuracy of this method all p

Acute and therapeutically uncontrollable cardiac insufficiency in a case of chronic heart disease with acute risk of rejection and with a record of previous transplantation or during a heart operation should be considered as an indication for implantation of an artificial heart or a ventricle-supporting system, if a donor heart is not available. Two results are expected from such an approach, restoration of adequate circulation and improvement of organ functionally providing a chance for later transplantation. Attention should be given, according to the authors' own experience, to effective su…

A cross-sectional survey of a total semirural biracial population of children was conducted to determine the prevalence and types of cardiac … tionately greater number of black children and boys were affected. Thirteen children had congenital heart disease, five had mitral valve prolapse, and only two had rheumatic heart disease. The prevalence of mitral valve prolapse was probably underestimated in this study.

Report on a fibrosarcoma of the right ventricle in a 70-year-old woman. The tumour was diagnosed by autopsy. Clinically were of importance the chronic insufficiency of the right heart, in the ECG the low voltage, a bifascicular block and a subepicardiac lesion in the area of the anterior wall. Roentgenogram of the thorax and the laboratory findings corresponded to a massive cardiac decompensation. The cytology from the effusion of the pericardium which often occurred gives a possibility of making a diagnosis intra vitam, if it is thought of a cardiac tumour.

A 32-week fetus was demonstrated phonocardiographically and echocardiographically to have a regular atrial rate of 150 per minute and a regular ventricular rate of 39 per minute, indicating complete heart block. The diagnosis was suspected when two groups of heart sounds at two distinct rates were heard on auscultation, and was confirmed by the postnatal ECG. The maternal history was significant for the presence of systemic lupus erythematosus. The basis for the echocardiographic diatnosis of complete heart block, the presence of atrial heart sounds in complete heart block, and the relationshi…

Thromboembolism is a complication which represents a major limiting factor in animals with long-term postoperative survival periods following total artificial heart replacement. Thrombi are formed on the artificial valves, the biomaterials and in stagnation areas within the artificial ventricles. In design planning the stagnation areas should be avoided as well as major turbulences. The filling volume should be maximally used for the stroke volume and the epicenter of the membrane should be underneath the valve areas. The ellipsoid heart eliminates stagnation areas and the stroke volume is 82%…

Most patients with heart failure (HF) have multiple comorbidities, which impact their quality of life, aggravate HF, and increase mortality. Cardiovascular comorbidities include systemic and pulmonary hypertension, ischemic and valvular heart diseases, and atrial fibrillation. Non-cardiovascular comorbidities include diabetes mellitus (DM), chronic kidney and pulmonary diseases, iron deficiency and anemia, and sleep apnea. In patients with HF with hypertension and left ventricular hypertrophy, renin-angiotensin system inhibitors combined with calcium channel blockers and/or diuretics is an eff…

BACKGROUND: Ex vivo heart perfusion (EVHP) has emerged as a transformative preservation strategy in heart transplantation with increasing adoption over the past decade. We sought to characterize the midterm outcomes of heart transplantation using EVHP.
METHODS: We queried the United Network for Organ Sharing database to identify adult heart transplants performed between October 2018 and December 2025. Patients were stratified by use of EVHP or static cold storage (SCS). The primary outcome was 3-year posttransplant survival, assessed using the Kaplan-Meier method. Secondary outcomes were seve…
