Clinical cardiac electrophysiologic study of celiprolol.

The objective of the study was to evaluate the acute effect of intravenous celiprolol on the electrophysiologic properties of the cardiac conduction system in man and to assess potential problems in terms of its causing heart block or sinus bradycardia. Eight patients with controlled coronary artery disease and hypertension but without conduction system disease were studied. All cardiac drugs e.g., digoxin, sympathomimetics and other beta blockers were discontinued prior to entering the study. Surface ECG leads I, AVF, and Vi were applied to each patient, and the ECG was continuously displayed

Optimal Cardiac Resynchronization Therapy with Conduction System Pacing Guided by Electro-Anatomical Mapping: A Case Report.

INTRODUCTION: Biventricular pacing has been the gold standard for cardiac resynchronization therapy in patients with left bundle branch block and severely reduced left ventricular ejection fraction for decades. However, in the past few years, this role has been challenged by the promising results of conduction system pacing in these patients, which has proven non-inferior and, at times, superior to biventricular pacing regarding left ventricular function outcomes. One of the most important limitations of both procedures is the long fluoroscopy times. CASE DESCRIPTION: We present the case of a

Spontaneous efferent activity in branches of the vagus nerve controlling heart rate and ventilation in the dogfish.

Efferent activity was recorded from cranial nerves in the decerebrate dogfish (Scyliorhinus canicula) before and after injection of paralysing drugs. The recordings were made from the mandibular (Vth) and glossopharyngeal (IXth) nerves and the branchial (respiratory) and cardiac branches of the vagus (Xth) nerve. All the respiratory branches (Vth, IXth and Xth) and both cardiac branches fired rhythmic bursts of activity, synchronous with ventilation, which continued (at a higher rate) following paralysis, indicating that they originated in the CNS rather than arising reflexly from stimulation

Successful termination of ventricular tachycardia by intrinsic antitachycardia pacing delivered from a left bundle branch area pacing lead: a case report.

BACKGROUND: Intrinsic antitachycardia pacing (iATP) is an advanced algorithm designed to optimize pacing sequences for ventricular tachycardia (VT) termination. Although left bundle branch area pacing (LBBAP) has emerged as a physiological pacing strategy and is increasingly incorporated into cardiac resynchronization therapy, its role as a delivery site for iATP remains unclear. CASE SUMMARY: We report a 57-year-old man with advanced heart failure who underwent implantation of LBB-optimized cardiac resynchronization therapy with defibrillator for secondary prevention. During follow-up, multi

Sudden death of an infant with cardiac, nervous system and genetic involvement--a case report.

UNLABELLED: We present a case of sudden death of a 1-month-old male infant with heart, brainstem and genetic polymorphism involvement. Previously considered quite healthy, the child died suddenly and unexpectedly during sleep. The autopsy protocol included an in-depth anatomopathological examination of both the autonomic nervous system and the cardiac conduction system, and molecular analysis of the serotonin transporter gene promoter region, in which a specific genetic condition seems to be associated with sudden infant death. Histological examination revealed the presence of congenital cardi

[Cardiac involvement in rheumatoid arthritis].

Rheumatoid arthritis (RA) is a systemic disease of unknown etiology characterized by a chronic inflammatory process mainly leading to destruction of synovial membrane of small and major diarthrodial joints. The prevalence of RA within the general adult population is about 1% and female subjects in fertile age result mostly involved. It's an invalidating disease, associated with changes in life quality and a reduced life expectancy. Moreover, we can observe an increased mortality rate in this population early after the onset of the disease. The mortality excess can be partially due to infective

Histopathology of the conduction system in sudden cardiac death.

In order to clarify the importance of the conduction system involvement in cases of sudden cardiac death, 35 autopsied hearts, obtained from the patients who died within one hour after the onset of a critical attack, were examined histopathologically and compared with 27 both age- and disease-matched and 30 only age-matched control hearts from individuals who had not died suddenly. The conduction system was serially sectioned using Lev's method and observed under a light microscope. Purkinje cell lesions, which made a structural maze compatible to the electrophysiological re-entry mechanism, a

Conduction system pacing in everyday clinical practice: EHRA physician survey.

With the increasing interest in conduction system pacing (CSP) over the last few years and the inclusion of this treatment modality in the current guidelines, our aim was to provide a snapshot of current practice across Europe. An online questionnaire was sent to physicians participating in the European Heart Rhythm Association research network as well as to national societies and over social media. Data on previous experience with CSP, current indications, preferred tools, unmet needs, and perceptions for the future are reported and discussed.

Right bundle branch pacing: Criteria, characteristics, and outcomes.

BACKGROUND: Targets for right-sided conduction system pacing (CSP) include His bundle and right bundle branch. Electrocardiographic patterns, diagnostic criteria, and outcomes of right bundle branch pacing (RBBP) are not known. OBJECTIVE: Our aims were to delineate electrocardiographic and electrophysiological characteristics of RBBP and to compare outcomes between RBBP and His bundle pacing (HBP). METHODS: Patients with confirmed right CSP were divided according to the conduction system potential to QRS complex interval at the pacing lead implantation site. Six hypothesized RBBP criteria as

Transient fetal atrioventricular block: A series of four cases and approach to management.

INTRODUCTION: Fetal atrioventricular block (AVB) is a failure of conduction from atria to ventricles. Immune- and nonimmune-mediated forms occur, especially in association with congenital heart disease. Second-degree (2°) AVB may be reversible with dexamethasone and intravenous immunoglobulin in immune-mediated disease. However, once third-degree AVB develops, it is deemed irreversible with need for a pacemaker and risk for cardiomyopathy. Rarely, 2° AVB is a transient, benign phenomenon in the immature conduction system. Few case series of transient AVB have been reported, but a management ap

Electrical interaction between cardiomyocyte sheets separated by non-cardiomyocyte sheets in heterogeneous tissues.

Electrical coupling between cardiomyocytes is important in synchronous beating and normal heart functions. Cardiomyocytes are also electrically coupled to non-cardiomyocytes. The electrical interactions between cardiomyocytes and non-cardiomyocytes, or those between separated cardiomyocytes, are important for normal heart function because abnormalities of the coupling and variation of the cell population induce pathological heart functions and arrhythmias. In this study the three-dimensional time course of the electrical interaction between two rat neonatal cardiomyocyte sheets separated by no

Electrophysiologic consequences of KATP gain of function in the heart: Conduction abnormalities in Cantu syndrome.

BACKGROUND: Gain-of-function (GOF) mutations in the KATP channel subunits Kir6.1 and SUR2 cause Cantu syndrome (CS), a disease characterized by multiple cardiovascular abnormalities. OBJECTIVE: The purpose of this study was to better determine the electrophysiologic consequences of such GOF mutations in the heart. METHODS: We generated transgenic mice (Kir6.1-GOF) expressing ATP-insensitive Kir6.1[G343D] subunits under α-myosin heavy chain (α-MHC) promoter control, to target gene expression specifically in cardiomyocytes, and performed patch-clamp experiments on isolated ventricular myocytes

A degenerative lesion of the approach to the atrioventricular node producing second-degree and third-degree atrioventricular block.

We report a case of a degenerative approach lesion in an 83-year-old male with diabetes mellitus, hypertension, and ischemic heart disease. His ECGs changed from first-degree atrioventricular (AV) block 14 years ago, to third-degree AV (A-H) block. A pacemaker was implanted for bradycardia. He died 4 years later from heart and renal failure. Serial sections through the conduction system revealed total depletion and fatty replacement of the atrial muscle at the approaches to the AV node.

Disopyramide-induced heart block.

A 57-year-old woman with right bundle branch block +LPH and ventricular premature contractions developed complete heart block (CHB) following administration of disopyramide phosphate (Norpace). The ECG at the onset of CHB suggested block to have occurred in the trifascicular conduction system. On rechallenging the patient with the drug following the implantation of a permanent pacemaker, intermittent complete heart block developed. This case suggests that disopyramide should be used with caution in patients with bifascicular block patterns on ECG.

A novel intrinsic analgesic mechanism: the enhancement of the conduction failure along polymodal nociceptive C-fibers.

Although conduction failure has been observed in nociceptive C-fibers, little is known regarding its significance or therapeutic potential. In a previous study, we demonstrated that C-fiber conduction failure, which is regarded as an intrinsic self-inhibition mechanism, was reduced in circumstances of painful diabetic neuropathy. In this study, we extend this finding in the complete Freund's adjuvant model of inflammatory pain and validate that the degree of conduction failure decreased and led to a greater amount of pain signals conveyed to the central nervous system. In complete Freund's adj

Permanent His bundle pacing to replace biventricular pacing for cardiac resynchronization therapy.

INTRODUCTION: Cardiac resynchronization therapy (CRT) or biventricular pacing (BIVP) has become a common procedure for the treatment of ventricular dyssynchrony in patients with heart failure, particularly in those with bundle branch block patterns (QRS durations >150ms) on the electrocardiogram (ECG). However, a large group of non-responders are made up of patients with dyssynchrony and QRS duration below 130ms. Recent studies have introduced permanent His bundle pacing as another method for achieving normalization of the QRS duration even in a majority of patients with right or left bundle b

Self-organizing human heart assembloids with autologous and developmentally relevant cardiac neural crest-derived tissues.

UNLABELLED: Neural crest cells (NCCs) are a multipotent embryonic cell population of ectodermal origin that extensively migrate during early development and contribute to the formation of multiple tissues. Cardiac NCCs play a critical role in heart development by orchestrating outflow tract septation, valve formation, aortic arch artery patterning, parasympathetic innervation, and maturation of the cardiac conduction system. Abnormal migration, proliferation, or differentiation of cardiac NCCs can lead to severe congenital cardiovascular malformations. However, the complexity and timing of ear

Preoperative evaluation of patients with electrocardiographic conduction defects.

Prophylactic pacemakers should be recommended for all preoperative patients with second- and third-degree heart block, regardless of whether the block is in the AV node or the trifascicular conduction system. Symptomatic patients with potential for trifascicular block should also have a transvenous pacemaker. Those without symptoms should be monitored during anesthesia and the postoperative recovery period.
