Light-Chain Deposition Diseases of the CNS: Review of Pathogenesis, Imaging Features, and Radiographic Mimics.

Light-chain deposition disease (LCDD) is a rare CNS disorder characterized by the extracellular accumulation of monoclonal immunoglobulin light chains in various organs. LCDD typically arises secondary to an underlying plasma cell dyscrasia, such as monoclonal gammopathy of undetermined significance or multiple myeloma. However, rare cases can occur in the absence of a demonstrable plasma cell disorder. The kidneys, liver, lungs, and heart are the most affected organs. Intracerebral LCDD, particularly without an underlying plasma cell neoplasm, represents an exceedingly uncommon entity with li

Right Ventricular Thrombus Masquerading as a Tumor.

Cardiac tumors are an uncommon phenomenon. Although they can be cardiac in origin, most represent a distant neoplastic growth metastasizing to the heart. Cardiac tumors can be benign or malignant. They may be symptomatic or, more commonly, found incidentally. Clinical presentation is typically related to that of dispersed neoplasm. We report a case of a 36-year-old young man with an unusually large and smooth-surfaced right ventricular mass. The patient presented to the emergency department with exertional dyspnea for two weeks. Past medical history was significant for deep venous thrombosis w

Mucin-producing bile duct tumor treated successfully with endoscopic ultrasound-guided hepaticogastrostomy.

We report the case of an 82 year-old Japanese man with a history of multiple heart surgeries who presented with nausea and loss of appetite. Laboratory tests showed elevated liver and biliary enzymes. Imaging showed a possible space-occupying lesion within a dilated bile duct. Endoscopic retrograde cholangiography showed an intact ampulla of Vater with a visible mucus-filled orifice presenting a fish-mouth appearance and fluoroscopy showed a possible tumor-causing mild stenosis of the bile duct. Cholangioscopy revealed papillary protrusions in the common bile duct. Pathological evaluation was

Moonlighting nuclear pore proteins: tissue-specific nucleoporin function in health and disease.

The nuclear pore complex is the main transportation hub for exchange between the cytoplasm and the nucleus. It is built from nucleoporins that form distinct subcomplexes to establish this huge protein complex in the nuclear envelope. Malfunctioning of nucleoporins is well known in human malignancies, such as gene fusions of NUP214 and NUP98 in hematological neoplasms and overexpression of NUP88 in a variety of human cancers. In the past decade, the incremental utilization of next-generation sequencing has unraveled mutations in nucleoporin genes in the context of an increasing number of heredi

Overactive bladder as a dysfunction of the autonomic nervous system - A narrative review.

Overactive bladder (OAB) is a common condition with increasing prevalence. However, its aetiology is not fully understood. The main symptoms are sudden urge to urinate, with or without incontinence; increased daytime frequency; and nocturia. OAB-like symptoms can be caused by a number of diseases, including urinary tract infections, neurologic and metabolic diseases, and neoplasms. The final diagnosis is made after excluding all the abovementioned factors. Autonomic nervous system (ANS) dysfunction has also been identified as a possible cause of so-called 'idiopathic OAB'. In order to check th

A Review of the Potential Benefits of Increasing Vitamin D Status in Mongolian Adults through Food Fortification and Vitamin D Supplementation.

Serum 25-hydroxyvitamin D (25(OH)D) concentrations are low in Mongolia, averaging 22 ng/mL in summer and only 8 ng/mL in winter. Mongolians have high incidence and/or prevalence of several diseases linked to low 25(OH)D concentrations, including ischemic heart disease, malignant neoplasms, cirrhosis of the liver, ischemic stroke, lower respiratory tract infections, preterm birth complications, and diabetes mellitus. Fortifying regularly consumed foods such as flour, milk, and edible oils with vitamin D3 could raise 25(OH)D concentrations by about 10 ng/mL. However, to achieve 25(OH)D concentra

Radiomics Features Differentiate Between Normal and Tumoral High-Fdg Uptake.

Identification of FDGavid- neoplasms may be obscured by high-uptake normal tissues, thus limiting inferences about the natural history of disease. We introduce a FDG-PET radiomics tissue classifier for differentiating FDGavid- normal tissues from tumor. Thirty-three scans from 15 patients with Hodgkin lymphoma and 68 scans from 23 patients with Ewing sarcoma treated on two prospective clinical trials were retrospectively analyzed. Disease volumes were manually segmented on FDG-PET and CT scans. Brain, heart, kidneys and bladder and tumor volumes were automatically segmented on PET images. Stan

Sonographic diagnosis of fetal cardiac rhabdomyomas and cerebral tubers: a case report of prenatal Tuberous Sclerosis.

UNLABELLED: The rhabdomyoma constitutes more than 60% of all cardiac tumors that are diagnosed in the prenatal and postnatal age. In more than 50% of cases, it is the first clinical manifestation of tuberous sclerosis (TS), autosomal dominant genetic condition and multisystem involvement. METHODS: we report a case of cardiac rabdomyomatosis in twin pregnancy bicorial biamniotic, with suspicion for tuberous sclerosis, diagnosed at our hospital. For the diagnosis of cardiac rhabdomyomas we used the two-dimensional ultrasound, and 3D echocardiography. For the diagnosis of intracranial subependym

What do chronic obstructive pulmonary disease patients die from? A multiple cause coding analysis.

Information on obstructive lung disease (OLD) deaths is generally derived from the underlying cause of death on the death certificate. This neglects information on other conditions mentioned and may underestimate the burden of disease. Descriptive analyses of all conditions mentioned on the death certificate for all decedents where OLD (chronic obstructive pulmonary disease or asthma) was mentioned as a contributing cause of death were conducted for England and Wales for 1993-1999. OLD was mentioned in 312,664 or 8.0% of all deaths. OLD comprised the underlying cause of death in 59.8% of death

[Probiotics and their therapeutic properties].

Probiotics are specific products of microorganisms, and by being biologically active positively act on stabilizing the bacteriological flora of the gastrointestinal tract. They are live or lyophilized bacterial cultures, especially those derived from lactic fermentation (Lactobacillacea and Streptococcacea). The first to use the term probiotics were Lilly and Stillwell in 1965 when referring to substances produced by protozoa, which in turn stimulated the growth of other organisms. Probiotics are devoid of side affects and do not cause accumulation of toxic substances in the body. They are adm

[Gastrointestinal cancer-associated antigen (GICA) and intravascular hemolysis. Report of 2 cases and review of the literature].

GICA or CA 19-9, monosialyl Lewis (a) antigen, is a ganglioside, component of cellular wall. GICA is known as a neoplastic marker with high correlation, specificity and sensibility for the gastrointestinal neoplasms, mainly of the pancreas. Two cases of intravascular haemolysis are reported (a case in woman with Moschowitz's syndrome and a case in man with mitral prosthetic valve and mild congestive heart failure) with elevated serum levels of GICA. First in the literature, the Authors hypothesize a strict correlation between the aspecific elevation of GICA serum levels and diffuse membrane ce

Thoracic surgical oncology.

Thoracic surgical oncology involves surgical treatment of lesions of the thoracic wall, pulmonary parenchyma, or mediastinum (also including heart, esophagus, or trachea). The most common neoplasms of the thoracic wall are osteosarcoma and chondrosarcoma. Histopathologic type, the use of chemotherapy for osteosarcoma, and completeness of surgical margins are prognostic for survival. Relative to solitary pulmonary masses, carcinomas are most common, with histopathologic type, tumor size, tumor grade, and lymph node status prognostic for survival. Of the aforementioned variables, lymph node stat

[Treatment of Hodgkin's disease with bulky mediastinal involvement. Study of 28 cases].

BACKGROUND: Bulky mediastinal involvement in Hodgkin disease (HD) is usually considered an adverse prognostic factor. Patients treated with radiotherapy alone (RT) or chemotherapy alone (CT) have a high relapse rate. The combined chemotherapy plus radiotherapy is the therapy of choice. The objective of the present study was to report our results and complications in the treatment of 28 patients with HD with bulky mediastinal involvement. MATERIALS AND METHODS: A retrospective analysis was made of a series of 28 patients with bulky mediastinal disease treated in our hospital from 1973-1993, wi

Alpha lipoprotein (HDL) cholesterol in the serum and the risk of coronary heart disease and death.

The Minnesota Prospective Study on coronary heart disease (CHD) in 284 business and professional men began in 1947. In 1953 and 1955 measurements were made of serum alpha-lipoprotein (HDL) cholesterol on the men. The mean was 45 . 2 mg/dl, the same as reported for HDL cholesterol in men of that age at Framingham. As with HDL at Framingham and elsewhere, alpha cholesterol was not correlated with age, blood-pressure, total cholesterol, or smoking habit, but was negatively correlated (r=-0.37) with relative body-weight. In the next 25 years 133 men died, 55 from CHD. Mean alpha cholesterol of tho

Right Atrial and SVC Cardiac Sarcoidosis: Not All Atrial Masses Are Tumors.

BACKGROUND: Right atrial (RA) masses create diagnostic challenges; extension into surrounding venous structures is rare. CASE SUMMARY: A 56-year-old woman presented with variable heart block and was found to have an RA mass extending into the superior vena cava (SVC). 18F-fluorodeoxyglucose positron emission tomography demonstrated metabolic activity within the myocardium, mediastinum, and pancreas concerning for sarcoidosis versus neoplasm. Endomyocardial biopsy was inconclusive. Transvenous pacemaker placement was not feasible given partial SVC obstruction by the mass. Ultimately, the patie

T-cell prolymphocytic leukemia with extensive cardiovascular infiltrate leading to multiple myocardial infarctions and cardiac death.

Lymphocytic neoplasm involving the heart is not common and usually presents with pericardial effusion or focal myocardial infiltration. Myocardial infarctions due to leukemic infiltration of the coronary arteries are rarely reported. We present the case of a 52-year-old Guatemalan man with a one-year history of untreated T-cell prolymphocytic leukemia. He was admitted to our hospital for chemotherapy and evaluation of a pulmonary cavitary lesion by wedge resection. During sedation, the patient experienced acute respiratory failure and hypovolemic shock, from which he could not be resuscitated.

Extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue with amyloid deposition: a clinicopathologic case series.

Extranodal marginal zone lymphoma of mucosa-associated lymphoid tissue (MALT) lymphoma is a mature B-cell neoplasm that typically follows an indolent clinical course. Amyloid deposition associated with MALT lymphoma is uncommon. We describe the clinical and pathologic features of 20 cases of MALT lymphoma and associated amyloid deposition across diverse primary sites. Frozen section immunofluorescence performed on 4 cases suggests that these deposits are a localized form of AL amyloid. Clinical follow-up was available for 15 patients. Amyloid deposits distant from the initial site occurred in
