BACKGROUND: There is little information about systemic hemodynamic changes in the evolution of heart failure (HF) from hypertension.
METHODS: We compared 24-hour mean hemodynamic variables derived from ambulatory pulse wave analysis (Mobil-O-Graph) in at-risk patients with hypertension (Stage A/B HF) and patients with Stage C HF secondarily stratified by ejection fraction (EF: low <40% or high ≥40%).
RESULTS: Differences (mean ± SEM) between Stage A/B (n = 109) and Stage C HF (n = 52) were, respectively: stroke volume index (SVI) 36.8 vs. 31.9 mL/m2 (P < .001), heart rate (HR) 68.1 vs. 76.4 …

Cheyne-Stokes respiration (CSR), a rhythmic rise and fall in ventilation often experienced by patients with heart failure during sleep, is typically accompanied by an oscillation in heart rate (HR) at the same frequency. The mechanisms responsible for this oscillation are still debated. In this study, we used the experimental model of the transient hypoxia test (i.e., a laboratory test that mimics the transient nature of the cyclic desaturations that occur during hyperpnoeic phases of CSR) to assess accurately the temporal relationship between the HR response to transient hypoxia and the tidal…

IMPORTANCE: Resting heart rate (RHR) is a widely available measure of cardiovascular fitness that has been associated with several cardiovascular outcomes. RHR has previously been associated with the risk of atrial fibrillation (AF) among individuals of European ancestry, but little is known about this association in Black adults.
OBJECTIVE: To evaluate the association between RHR and incident AF in a large community-based sample of Black adults, independently of established risk factors.
DESIGN, SETTING, AND PARTICIPANTS: This cohort study uses data from the Jackson Heart Study, a prospecti…

There is growing research into contributing processes and biological correlates of moral inclinations. Recently, a negative association between utilitarianism and resting heart rate variability (HRV) as an indicator of cardiac vagal tone/parasympathetic activity was reported. We aimed to replicate and extend these findings by additionally investigating the sympathetic parameter electro-dermal activity (EDA), but found no associations in the total sample (N = 157). However, when taking sex and the use of combined oral contraceptives (COC) into account, we found a positive association between HR…

Attenuated heart rate recovery (HRR) following peak exercise has been shown to be a predictor of mortality in populations of adults with Fontan palliation, coronary artery disease, heart failure, and heart transplantation. However, few have studied HRR in children and adolescents with congenital heart disease (CHD). This case-control study compared HRR patterns from exercise stress testing in children and adolescents with and without repaired acyanotic CHD (raCHD). Retrospective analysis included patients aged 10-18 years who had exercise testing between 2007 and 2017. The raCHD cohort include…

AIMS: Heart transplantation (HTx) results in complete autonomic denervation of the donor heart, causing resting tachycardia and abnormal heart rate (HR) responses to exercise. We determined the time course of suggestive cardiac reinnervation post HTx and investigated its clinical significance.
METHODS AND RESULTS: Heart rate kinetics during standard cardiopulmonary exercise testing at 2.5-5 years after HTx was assessed in 58 patients. According to their HR increase 30 s after exercise onset, HTx recipients were classified as denervated (slow responders: <5 beats per minute [b.p.m.]) or potent…

BACKGROUND: Heart rate variability (HRV) is a noninvasive method for assessing autonomic function. Age, sex, and chronic conditions influence HRV.
OBJECTIVES: Our aim was to evaluate HRV measures exploring differences by age, sex, and race in a sample from a rural area.
METHODS: Analytical sample (n = 1,287) included participants from the 2010 to 2016 evaluation period of the Baependi Heart Study, a family-based cohort in Brazil. Participants underwent 24-hour Holter-ECG (Holter) monitoring. To derive population reference values, we restricted our analysis to a 'Работает' subset (i.e. absence…

BACKGROUND: A blunted heart rate reserve (HRR) during dipyridamole stress echocardiography (DSE) is a prognostically unfavorable sign of cardiac autonomic dysfunction. Short-term adjustments of heart rate (HR) are thought to rise from changes in neural input to the heart. DSE is applied in potential heart donors to rule out underlying coronary artery disease and left ventricular dysfunction. The aim of this study is to assess HRR during DSE in brain death.
METHODS: We enrolled two groups: group 1 (N.=49, 22 men, 54.6±8.8 years) with patients in brain death enrolled in the nationwide marginal …

Monitoring fetal heart rate in an important aspect in evaluating fetal well being. Maternal-fetal interaction has shown evolution during fetal maturation. In this work, we studied maternal-fetal heart rate synchronization in early and late gestation fetuses. We also evaluated variations in the synchronization due to congenital heart defect (CHD). Maternal-fetal heart rate synchronization for 22 early gestation (Age < 32 weeks), $late gestation (Age >32 weeks) and 7 CHD fetuses (5 of them with gestational age < 32 weeks). The synchronization ratio between the mother and the fetus was more local…

UNLABELLED: Cardiovascular diseases are a significant cause of morbidity and mortality in the general population. In this sense, the autonomic imbalance is the cornerstone of the pathophysiology underlying the development of these diseases. The aim of this study was to determine the efficacy of exercise training on heart rate variability (HRV) in adult patients with chronic heart failure.
METHODOLOGY: A systematic literature review was conducted in electronic databases. The considered studies were randomised clinical trials, quasi-experimental studies with non-randomised control group, quasi-…

Phenytoin is a known human teratogen with unknown etiology. Several mechanisms have been proposed including disturbances in folate metabolism, induction of embryonic hypoxia following phenytoin-induced bradycardia, free radical formation following re-oxygenation and phenytoin-induced maternal hyperglycemia. Using high frequency ultrasound, we demonstrated that phenytoin induced a dramatic decrease in the heart rate of embryos. This coincided with a moderate transient decrease in maternal heart rate and blood glucose levels. Embryonic heart rate had not fully recovered 24 h later in some embryo…

The occurrence of arrhythmia is often related to basic heart rate. Prognostic significance is associated with such a relationship; furthermore, heart rate modulation may result as an ancillary effect of therapy, or be considered as a therapeutic tool. This review discusses the cellular mechanisms underlying arrhythmia occurrence during tachycardia or bradycardia, considering rate changes per se or as a mirror of autonomic modulation. Besides the influence of steady-state heart rate, dynamic aspects of changes in rate and autonomic balance are considered. The discussion leads to the conclusion …

The effect of maternal exercise (the two-step test of Master) on the fetus was studied in 28 pregnant women. These subjects consisted of 5 cardiac cases, 5 cases with IUGR, 5 cases with preeclampsia and 13 normal pregnant women. Overt deceleration of the FHR was found after exercise only in 3 cardiac cases whose FHR tracings had been normal before. Slight changes such as a small rise or a small fall in FHR after exercise were seen in 4 of 7 cases who had shown decelerations in FHR before, and in a normal case. Fetal tachycardia appeared in 2 normal cases immediately before exercise; in one cas

Energy expenditure is an important parameter in the assessment of gait and orthotic treatment of heavily handicapped patients, since any orthosis which increases the effort required is likely to be discarded with consequently disturbing implications. Heart rate has been shown to be a reliable monitor of energy expenditure and is convenient because ECG can be telemetered by a miniature radio transmitter. Using a transmitter causes minimal interference with the patient, but care must be employed to obtain ECG signals free of artefact from subjects who are exercising. Means of reducing artefact a…
