Aortic stenosis has emerged as a global pandemic with the ageing population and has become the third leading cause of structural heart disease after hypertension and coronary artery disease in industrialized countries. Older age and presence of serious comorbid conditions make surgery prohibitively risky in a large group of patients. The long-term results of balloon aortic valvuloplasty have been unsatisfactory, limiting the use of this procedure to only palliation or as a bridge to surgery. Percutaneous aortic prosthetic valve implantation has emerged as a new treatment modality for patients

Surgical valve replacement is the treatment of choice in patients with severe valvular heart disease. However, published data clearly show that long-term survival is seriously compromised in those with a low inotropic reserve. In addition, a growing number of patients (estimated to be around 15% of those with aortic stenosis) present in such a poor and deteriorating clinical condition that they cannot be accepted for surgery because it involves a very high risk of death. Percutaneous balloon valvuloplasty is a less aggressive alternative which offers a form of palliative treatment that can inc

The most common cause of tricuspid valve dysfunction is functional tricuspid regurgitation (TR) secondary to mitral valve disease. Annuloplasty is feasible in most patients with functional TR, and valve repair can also be performed in most patients with tricuspid valve dysfunction of other etiologies. Valve replacement is considered to be indicated only for those patients whose tricuspid valves have severe organic change or have been damaged by infective endocarditis. Although good long-term results of tricuspid valve replacement using bioprostheses have been reported, a bileaflet mechanical p

BACKGROUND: Little is known about the incidence of prosthesis-patient mismatch (PPM) and its impact after transcatheter aortic valve replacement with self-expanding valves in an Asian population. We aimed to assess postprocedural effective orifice area with standardized methods and the impact of PPM on midterm outcomes after CoreValve or Evolut R (Medtronic) implantation in an Asian population.
METHODS: Among 201 consecutive patients undergoing CoreValve or Evolut R implantation, PPM was assessed at 30 days and defined based on core laboratory measured indexed effective orifice area as severe

BACKGROUND: Mitral annular calcification (MAC) may represent a significant challenge for heart surgeons with an extremely high perioperative risk during mitral valve (MV) surgery. The risk is further increased when patients fail to be eligible for any percutaneous treatment, particularly because circumferential calcifications involving the anterior leaflet suggest a critical obstruction of the left ventricular outflow tract (LVOT).
OBJECTIVES: The objective of this study was to evaluate residual mitral regurgitation (MR) after surgical mitral valve replacement using a Sapien 3 Ultra (Edwards

Although sizing for mitral annuloplasty (MAP) is a critical part of mitral valve repair, no consistent method is used, and precise size adjustment is done somewhat by intuition. We developed a new original ring size tester and have been applying it to our mitral valve repair procedure. The concept is that the tester shapes the annulus into exactly the same size and form as the target annuloplasty while confirming a good leaflet coaptation with a saline test to simulate annuloplasty. This original tester provides the best ring size for MAP that is neither too large nor too small. In this paper,

BACKGROUND: Transcatheter aortic valve replacement (TAVR) in bicuspid aortic valves (BAVs), especially with severe calcification, presents challenges including prosthesis underexpansion and paravalvular leak.
CASE SUMMARY: We present a critically unwell 70-year-old man with severe aortic stenosis in a heavily calcified Sievers-0 BAV, complicated by heart failure and chronic kidney disease. Intravascular lithotripsy (IVL) was used for calcium modification, followed by balloon valvuloplasty and deployment of a 26-mm Evolut PRO+ valve. The patient had excellent clinical recovery, with no paraval

Valve embolization is a catastrophic complication of mitral valve-in-valve and valve-in-ring procedures and occurs due to inadequate ventricular positioning, undersizing, or insufficient anchoring. Emergent cardiac surgery to retrieve the embolized valve is usually required for overt embolization. In the situation described herein, a decision was made to attempt to deploy a second valve at a proper depth to anchor the embolized valve and prevent further migration. This case highlights the technique of deploying a second valve to anchor the embolized valve if valve migration has occurred with m

Valvular heart disease is one of the common heart diseases in clinical practice, characterized by valve stenosis and/or incomplete closure. At present, drug therapy, surgery, and emerging percutaneous intervention therapy are the main treating methods for heart valve disease. Although the research and development of percutaneous intervention therapy devices is relatively mature, there are still problems such as postoperative mechanical hemolysis. The occurrence of mechanical hemolysis is associated with factors such as excessive shear stress experienced by red blood cells, direct interaction b

Heart valve disease is a significant cause of mortality worldwide. However, to date, a nonthrombogenic, noncalcific prosthetic, which maintains normal valve mechanical properties and hemodynamic flow, and exhibits sufficient fatigue properties has not been designed. Current prosthetic designs have not been optimized and are unsuitable treatment for congenital heart defects. Research is therefore moving towards the development of a tissue engineered heart valve equivalent. Two approaches may be used in the creation of a tissue engineered heart valve, the traditional approach, which involves see

BACKGROUND: Leaflet modification (LM) techniques, including electrosurgical and mechanical BASILICA (bioprosthetic or native aortic scallop intentional laceration to prevent iatrogenic coronary artery obstruction) and intraleaflet deployment, are important in the lifetime management of aortic valve disease. However, benchtop assessment is limited.
OBJECTIVES: The aim of this study was to provide a benchtop assessment of LM in native valves and bioprosthetic surgical aortic valves (SAVs).
METHODS: Features of calcific native aortic valves (n = 100) and naive SAVs (7 types) were assessed. Calc

Mitral regurgitation (MR) is the most common heart valve disease, and severe MR is associated with a poor prognosis if left untreated. Although surgical repair or replacement constitutes the standard therapy when indicated, many high-risk patients are considered ineligible for surgery. Transcatheter mitral valve replacement (TMVR) offers a less invasive alternative to conventional surgery and may also overcome some of the limitations of percutaneous repair techniques. Currently, multiple TMVR devices are undergoing clinical evaluation, showing promising results. However, challenges mainly rela

BACKGROUND/AIM: To evaluate three mitral bioprostheses (of comparable measured internal diameters) under controlled, stable, hemodynamic and surgical conditions by bench, echocardiographic, computerized tomography and autopsy comparisons pre- and postvalve implantation.
METHODS: Fifteen similar-sized Yorkshire pigs underwent preprocedural computerized tomography anatomic screening. Of these, 12 had consistent anatomic features and underwent implantation of a mitral bioprosthesis via thoracotomy on cardiopulmonary bypass (CPB). Four valves from each of three manufacturers were implanted in ran

The causes of early bioprosthesis failure include infective endocarditis, pannus formation, and structural valve deterioration. We reported a patient who suffered from early mitral bioprosthesis failure due to leaflets restricted by the subvalvular apparatus and early pannus formation. In patients with symptoms relapse and mitral regurgitation recurrence early after mitral valve replacement, early pannus formation needs to be anticipated, and surgical intervention should be performed if symptoms persist after medical treatment.
