Increase in serum alpha-fetoprotein without recurrent disease after hepatectomy for hepatocellular carcinoma.

The case of a 58-year-old man with clinically-stable and compensated HBsAg-positive liver cirrhosis is reported. In April 1991, the patient underwent partial hepatectomy to treat a solitary 3.5 cm hepatocellular carcinoma (HCC), (Edmonson scale I), in the 5th liver segment. His serum alpha-fetoprotein (AFP) level was 24 ng/ml. After hepatectomy, the AFP level dropped to 8 ng/ml, but between the 4th and 12th month it rose gradually from 72 ng/ml to 4,520 ng/ml. Hepatic recurrence of HCC was excluded, but a 6 cm solitary metastasis (Edmonson scale III-IV) was detected on the right adrenal. Adren…

[Fatal hydatid pulmonary embolism during hepatectomy for hydatid cyst. Apropos of a case].

Liver Hydatid cysts located close to the inferior vena cava and the sus-hepatic veins are known for their risk of rupture inside these veins. The authors report the case of a patient who died from massive pulmonary embolism during the surgical treatment (hepatectomy) of liver hydatid cysts. Diagnosis and treatment of these fistula are reviewed.

Influence of epidermal growth factor on liver regeneration after partial hepatectomy in mice.

The role of epidermal growth factor (EGF) in liver regeneration was studied in mice after partial hepatectomy. Two weeks before partial hepatectomy, mice were sham-operated (control) or sialoadenectomized (removal of submandibular glands) to reduce plasma EGF levels. Sialoadenectomized mice showed low plasma EGF levels (29.7 +/- 6.6 pmol/l; mean +/- S.E.M.) compared with controls (66.0 +/- 8.3 pmol/l). After partial hepatectomy, sialoadenectomized mice were treated with or without a daily s.c. injection of 5 micrograms EGF and the rate of DNA synthesis in the regenerating liver was monitored b…

Robotic parenchyma-sparing resection with a multiplanar-driven approach: the mini-rollercoaster hepatectomy.

For tumors close to the hepatic veins (HV), anatomical plane dissection is usually preferred in minimally invasive surgery to ensure oncological radicality and a safer and less technically demanding procedure. Drawing from open surgery experience with intraoperative ultrasound (IOUS) guidance and preoperative 3D casts, the concept of transversal hepatectomy performed along a multiplanar plane crossing the HV, termed "rollercoaster hepatectomy", has been proposed. Its feasibility as a robotic parenchyma-sparing strategy remains largely unexplored. We report a case of liver metastases treated wi…

Increased Liver Stiffness Despite Biochemical Recovery in Living Donors 1-Year Post-Hepatectomy.

BACKGROUND: Living donor hepatectomy is widely regarded as safe based on short-term outcomes and biochemical recovery; however, the long-term structural adaptation of the regenerated liver remains poorly understood. We evaluated longitudinal changes in liver stiffness and steatosis using transient elastography in living liver donors. METHODS: We retrospectively analysed prospectively collected data from 152 donors who underwent pure laparoscopic right or extended right hepatectomy and completed FibroScan assessments before donation and at 12 months postoperatively. RESULTS: At 1 year, liver …

Post-hepatectomy-related DIC Predicts Poor Survival After Resection for Colorectal Liver Metastases.

BACKGROUND/AIM: The prognostic significance of post-hepatectomy-related disseminated intravascular coagulation (DIC) in patients undergoing hepatic resection for colorectal liver metastases (CRLM) remains unclear. PATIENTS AND METHODS: This single-center retrospective study included 89 patients who underwent hepatic resection for CRLM between December 2003 and March 2024. Post-hepatectomy-related DIC was defined as a revised Japanese Association for Acute Medicine DIC score of 3 or higher on postoperative day 3. Prognostic factors for overall and disease-free survival were analyzed using Cox …

[Two Cases of Gastric Cancer Accompanied by Simultaneous Liver Metastases Treated with Hepatectomy].

Case 1: Urgent examination of a 72‒year‒old man with hematemesis revealed a gastroesophageal tumor. Biopsy revealed gastric cancer, and computed tomography(CT)revealed enlarged lymph nodes and multiple liver metastases. Laparoscopic gastrectomy led to the diagnosis of a pT3N2M1(HEP), pStage Ⅳ liver tumor. A CT scan performed 2 months postoperatively revealed enlargement of the liver tumor, suggesting hepatocellular carcinoma and prompting right liver lobectomy. Histopathological examination of the operative specimen revealed metastatic hepatocellular carcinoma. Adjuvant chemotherapy(S‒1)was ad

Multicystic biliary hamartoma with long-term gradual enlargement treated by laparoscopic partial hepatectomy.

Multicystic biliary hamartoma (MCBH) is an extremely rare liver tumor characterized by a well-circumscribed, multicystic honeycomb appearance on imaging. Herein, we report a case of long-term gradually enlarging MCBH that underwent laparoscopic hepatectomy. A 69-year-old man presented with mildly elevated carcinoembryonic antigen levels and underwent computed tomography (CT) 11 and 5 years ago, at which time the physicians did not note any particular abnormal findings. The current contrast CT for screening demonstrated a cystic lesion in segment 2 of the liver. A retrospective review of CT ima…

Perioperative sildenafil citrate administration in hepatectomy: a study protocol of randomized controlled trial.

BACKGROUND: Intraoperative blood loss (IBL) is the major concern in hepatectomy, which was closely relating to the central venous pressure (CVP). The controlled low central venous pressure (CLCVP) technique is moderately required for minimizing hemorrhage during hepatectomy nowadays. Nevertheless, an excessive reduction in CVP might give rise to the jeopardy of inadequate perfusion of vital organs. Sildenafil, being a first-line pharmaceutical for treating pulmonary hypertension, its mechanism of action lies in diminishing pulmonary vascular resistance (PVR) by dilating pulmonary arteries, and…

Refractory Chylous Ascites Leading to Chylothorax Following Extended Right Hepatectomy for Intrahepatic Cholangiocarcinoma.

Chylous ascites, a rare but severe complication of abdominal surgery, often results from lymphatic vessel damage during procedures, such as extended resection and lymphadenectomy. Although conservative management through dietary modifications and medications is the primary approach, refractory cases may lead to severe complications including nutritional deficiencies and even death. Herein, we report a case of refractory chylous ascites that progressed to chylothorax after extended right hepatectomy with lymph node dissection for intrahepatic cholangiocarcinoma. A 73-year-old woman developed ch…
