Percutaneous (Periportal) Hip Arthroscopy.

Capsulotomy in hip arthroscopy is essential for optimal visualization and instrument maneuverability. Commonly used capsulotomy techniques, such as interportal and T-type capsulotomy, result in damage to the iliofemoral ligament, a major stabilizer of the hip joint. The periportal technique, a relatively novel capsulotomy technique, involves the creation of 2 to 3 relatively small portals, thereby preserving the capsuloligamentous complex while allowing for the passage of an arthroscope and instruments into the joint space. Emerging evidence suggests that the periportal approach leads to impro…

Arthroscopic Hip Capsular Reconstruction.

Hip capsular reconstruction is an emerging surgical option for treating irreparable capsular deficiency in the setting of hip microinstability. While prevention through meticulous capsular preservation and repair remains paramount, reconstruction offers a viable solution in revision cases or after aggressive capsulectomy. Multiple graft options, including iliotibial band, dermal allograft, and Achilles tendon, have demonstrated biomechanical restoration of stability, though optimal choice and long-term efficacy remain under investigation. Careful patient selection, adherence to surgical princi…

Hip Fractures: A Review.

IMPORTANCE: More than 14.2 million people worldwide and 280 000 in the US experience a hip fracture each year. The median 1-year mortality rate after a hip fracture is 22% and approximately 42% to 71% of patients regain their prefracture level of basic activities of daily living within 6 months. OBSERVATIONS: Hip fractures are classified as intracapsular or extracapsular and most commonly occur after a fall. Intracapsular hip fractures include femoral neck (34%) and femoral head (rare). Extracapsular fractures consist of intertrochanteric (48%) and subtrochanteric fractures (5.8%). In the US,…

Management of Hip Fractures.

Hip fractures in women are a serious health concern, demanding prompt treatment and pain control methods. This study examines fracture frequency, diagnostic techniques, and classification, focusing on femoral neck and intertrochanteric fractures. It discusses risk factors including age, gender, bone strength, and medication and their impact on fracture risk. Treatments range from noninvasive care to surgeries such as internal fixation and joint replacement. Perioperative measures such as anesthesia, antibiotics, and clot prevention to improve outcomes and lessen complications are also discusse…

Complications of Hip Hemiarthroplasty.

Hip hemiarthroplasty is a commonly performed orthopedic surgery, used to treat proximal femur fractures in the elderly population. Although hip hemiarthroplasty is frequently successful in addressing these injuries, complications can occur. Commonly seen complications include dislocation, periprosthetic fracture, acetabular erosion, and leg-length inequality. Less frequently seen complications include neurovascular injury and capsular interposition. This article presents a comprehensive review of the complications associated with the management of hip hemiarthroplasty. [Orthopedics. 2023;46(4)…

Hip Resurfacing in Teenagers.

BACKGROUND: The goals of hip resurfacing are to relieve pain, restore function, and preserve future reconstructive options. Hip resurfacing is an attractive and sometimes the only option when the femoral canal is blocked, making total hip arthroplasty (THA) difficult. Hip resurfacing also can be an attractive option for a teenager on the rare occasion when a hip implant is needed. METHODS: A cementless ceramic-coated femoral resurfacing implant matched with a highly cross-linked polyethylene acetabular bearing was used in 105 patients (117 hips) aged 12 to 19 years. The mean follow-up was 14 …

Hip pain in childhood.

Hip pain in a child can have infectious, inflammatory, traumatic, neoplastic, or developmental causes, which can make the diagnosis challenging. Meticulous history taking and a detailed clinical examination guide the radiological investigation. In this article, we address some of the main causes of hip pain in childhood and their findings on diagnostic imaging.

Pediatric Hip and Pelvis.

The most common pediatric orthopedic conditions of the hip and pelvis involve abnormal architecture of the joint leading to pain and dysfunction. Developmental dysplasia of the hip and femoroacetabular impingement are 2 common and distinct forms of structural pathology in the pediatric hip. The authors also discuss 2 of the more common, and often questioned, pediatric hip disorders-slipped capital femoral epiphysis and Legg-Calvé-Perthes disease. Future investigations are aimed at identifying risk factors to provide pediatric orthopedists tools to risk stratify their patients and understand wh…

Acupuncture for hip osteoarthritis.

BACKGROUND: Hip osteoarthritis (OA) is a major cause of pain and functional limitation. Few hip OA treatments have been evaluated for safety and effectiveness. Acupuncture is a traditional Chinese medical therapy which aims to treat disease by inserting very thin needles at specific points on the body. OBJECTIVES: To assess the benefits and harms of acupuncture in patients with hip OA. SEARCH METHODS: We searched Cochrane CENTRAL, MEDLINE, and Embase all through March 2018. SELECTION CRITERIA: We included randomized controlled trials (RCTs) that compared acupuncture with sham acupuncture, a…
