Implementation of computerized prescriber order entry in four academic medical centers.

PURPOSE: Lessons learned through the transition to computerized prescriber order entry (CPOE) at four academic medical centers are reviewed. SUMMARY: CPOE is an important strategy in efforts to improve medication and patient safety and achieve compliance with federal health care information technology objectives. Pharmacy-led CPOE implementation teams at Brigham and Women's Hospital, Georgia Health Sciences Health System, UC Health University Hospital, and University of Utah Hospitals and Clinics were challenged to overcome different types of resource, staffing, and hardware-software constrai

Respiratory follow-up to improve outcomes for Aboriginal children: twelve key steps.

BACKGROUND: Among Aboriginal children, the burden of acute respiratory tract infections (ALRIs) with consequent bronchiectasis post-hospitalisation is high. Clinical practice guidelines recommend medical follow-up one-month following discharge, which provides an opportunity to screen and manage persistent symptoms and may prevent bronchiectasis. Medical follow-up is not routinely undertaken in most centres. We aimed to identify barriers and facilitators and map steps required for medical follow-up of Aboriginal children hospitalised with ALRIs. METHODS: Our qualitative study used a knowledge

Recommendations for reporting morbid events after heart valve surgery.

Major reasons for the considerable heterogeneity among published results of heart valve surgery are inconsistency in follow up techniques, reporting systems and classification of adverse events. The present recommendations are intended to harmonize the presentation of clinical material in order to improve comparison of data from different sources for the analysis of pooled data. The quality of an observational study is largely, if not entirely, due to the follow up technique, which may be graded according to six categories: Self-reporting of adverse events/well-being by the patients may be cla

Understanding Pediatric Clinical Deterioration Through Rapid Response System Trigger Clusters.

OBJECTIVES: Medical emergency teams (METs) are activated in response to signs and symptoms, or triggers, of clinical deterioration in acute care settings. However, the patterns in which triggers manifest and impact outcomes are poorly understood. We identified and described the patterns in which multiple triggers cluster to activate pediatric METs and examined the associations between these clusters and outcomes. METHODS: Pediatric MET events from January 2015 to December 2019 in the Get With The Guidelines®-Resuscitation national registry focused on METs (N = 4289) were grouped into MET trig

Anthrax attacks and practice patterns: a learning opportunity for health care systems.

Sudden and unexpected events directly influencing clinical practice patterns are uncommon. After the first report of bioterrorism-related anthrax, the authors studied retrospectively 13 months of anthrax-related antibiotic prescription rates for Veterans Affairs outpatients in one urban area where no cases of anthrax were reported. During the 26 days after the first anthrax report, the rate of acute respiratory illnesses treated with fluoroquinolones was 62.8 per 10,000 outpatient visits, an increase of 41 percent over the rate of 44.4 observed approximately one year earlier (p = 0.058). Acute

Expanding research on clinical agent data to a virtual space of location, time, and context: exploration of data structures and limitations using a pragmatic digital twin.

INTRODUCTION: Fragmented healthcare data and the inflexible structure of current hospital information systems (HIS) hinder the routine use of data for research and process optimization. Conventional HISs often fail to represent physical resources, spatial relationships, and temporal dependencies among hospital entities. In this feasibility study, we demonstrate the practical application of OMNI-SYS, a pragmatic object-centric hospital information system (oHIS) framework that represents objects within a 3D space of Time, Location, and Context. Using pre-operative patient tracking as an example,…

[Survey on Information Sharing and Approaches to Cooperation between Hospitals and Community Pharmacies in the Care of Outpatients Receiving Chemotherapy].

Outpatients undergoing chemotherapy receive oral anticancer drugs, supportive care medicine, and drugs for complications from health insurance pharmacies(ie, drugstores). Therefore, drugstore personnel and patients were surveyed using a questionnaire to ascertain the current conditions of information sharing between drugstores and hospitals. Only 31% of the patients surveyed responded that they received cancer chemotherapy via the drugstores, while a few of them understood the need for information sharing with the drugstore. We also found that the drugstores required a considerable amount of p

Impact of Digitalization on Performance of Dutch Hospitals: Longitudinal Quantitative Study.

BACKGROUND: Hospitals continue to invest heavily to increase their level of digitalization. While advanced digital maturity is assumed to improve hospital performance, empirical evidence remains mixed. This tension is mirrored by the productivity paradox of IT, whereby investments in digital technologies do not consistently translate into observable performance gains. OBJECTIVE: This study aims to examine the relationship between the Healthcare Information and Management Systems Society (HIMSS) Electronic Medical Record Adoption Model (EMRAM) score and hospitals' financial, operational, and w

The challenged conscience: Moral predicaments of hospital frontline administrators in China's medical disputes.

Although medical disputes have become a pervasive challenge in global healthcare systems, the role of hospital organizations in this process has received limited scholarly attention. In the Chinese context, existing research on medical disputes often treats hospitals as the background or monolithic actors, neglecting internal organizational dynamics. This article addresses this gap by examining hospital frontline administrators (HFAs), a pivotal group in resolving medical disputes. Drawing on the theory of street-level bureaucracy and ethnographic fieldwork, the study demonstrates how structur
