Safe sex behavior in drug-addicted males as a function of object relations.

An inventory of sexual behavior was administered in a hospital detoxification program to 127 drug-addicted males, who also completed a modified form of the Defense Mechanisms Inventory (DMI) which yields a measure of object relations. Condom use was shown to be related to an object relations mode that typifies the individual who can structure reality and needs without requiring an integral aspect of "the other" in his approach. Standardization and factor analytic data are also presented for the DMI and the object relations measure for this population. The resulting pattern and the small effect

Transforming the Patient Grievance Review Process by Addressing Both Patient and Physician Well-Being.

Patient grievances are an inevitable part of health care delivery, yet existing review processes do not focus on the unforeseen harms that grievances can have on the health care team. At a large academic children's hospital, a human-centered approach was used to redesign the grievance review process. Through listening sessions with frontline physicians and the formation of a multidisciplinary work group, the authors developed and implemented a novel approach to responding to patient concerns related to physician care, while also supporting the physicians. The redesigned process focused on comm

Difference in healthcare resource utilization and health-related quality of life between patients with major depressive disorder (MDD) and treatment-resistant depression (TRD) in a public hospital in Malaysia - a retrospective case-note review and cross-sectional survey.

BACKGROUND: Major depressive disorder (MDD) is a growing global health concern, with its economic impact projected to reach USD 6.0 trillion by 2030. Treatment-resistant depression (TRD), characterized by non-responsiveness to initial treatments, adds a significant cost burden. In Malaysia, the National Health and Morbidity Survey 2023 reported a rising prevalence of depression in Malaysia, affecting approximately one million individuals. RESEARCH DESIGN AND METHODS: A combined retrospective chart review and cross-sectional survey was conducted at the Psychiatry and Mental Health Clinic of Ho

The impact of patient experience dimensions on hospitals reputation and electronic word-of-mouth.

Our objective was to identify the dimensions of the patient experience that directly influence a hospital's reputation and indirectly impact electronic word-of-mouth communication carried out by patients. We collected data from 484 hospital users and analyzed the data using PLS-SEM. Our results show that paying attention to patient preferences and physical comfort, providing information and education, and treating patients' families and friends well tend to have a significant impact on the hospital's reputation. In turn, a hospital's reputation may influence patients to perform electronic word

Violent Behavior among hospitalized medical and surgical patients.

To characterize violent behavior in hospitalized medical and surgical patients, we reviewed documented violent incidents at the San Francisco General Hospital during a two-year period. Twenty-nine incidents of verbal and physical violence occurred. One patient was gravely ill and three were delirious. All the incidents were associated with increased levels of tension and loss of impulse control. In most cases, contention with the staff regarding pain medication or ward regulations was a precipitating event. Of the 28 patients with mental disorders, 19 were substance abusers, six had organic br

Survey of abuse and violence by patients and relatives towards intensive care staff.

Fungal sinusitis is broadly classified into invasive and non invasive types. Invasive type presents with fungal hyphae within the mucosa, submucosa, bone, or blood vessels of the paranasal sinuses and includes Acute Invasive Fungal Sinusitis, Acute Fulminant, Chronic Invasive Fungal Sinusitis, Chronic Granulomatous Fungal Sinusitis. Invasive forms of fungal rhinosinusitis are rare and confined to groups of patients who are immunocompromised (Key in Fungal rhinosinusitis). Noninvasive type shows absence of fungal hyphae within the mucosa paranasal sinuses. It includes Allergic Fungal rhinosinus

Internal Medicine Residents' Experience Performing Routine Assessment of What Matters Most to Patients Upon Hospital Admission.

PROBLEM: Failure to elicit patients' values, goals, and priorities can result in missed opportunities to provide patient-centered care. Little is known about resident physicians' direct experience of eliciting patients' values, goals, and priorities and integrating them into routine hospital care. INTERVENTION: In 2017, we asked resident physicians on general internal medicine wards rotations to elicit and document a "Personal History" from patients upon hospital admission, in addition to a traditional social history. We defined a Personal History as documenting "what matters most to the pati

Implementing the living will and other advance directives: guidelines for health care providers.

Living wills and other advance directives for the refusal of medical treatment by patients no longer able to make their own medical decisions have gained increasing acceptance in the last decade. In addition to constitutional and common law rights to refuse treatment, living will statutes have been enacted in many states to permit advance directives under specified circumstances. This article explores both statutory and non-statutory advance directives and offers guidelines for hospitals and medical staffs when decisions regarding life-sustaining treatment must be made for adults with terminal

Patient selection or rational patient choice? Insights from a hospital competition model with heterogeneous agents.

Hospitals differ along several dimensions, including ownership structure, strategic objectives, and resource capacity, while patients vary in illness severity, comorbidities, and other factors affecting their healthcare needs. Available empirical evidence of patient selection by hospitals, however, is rather limited. In this paper, we develop a theoretical model of quality competition under regulated prices that incorporates provider heterogeneity-such as distinctions between public and private hospitals-and patient heterogeneity in terms of illness severity. In our framework, hospitals are no

Difficult Patient Behaviors in the Hospital Setting: A Narrative Review.

In the inpatient medical setting, difficult patient behaviors can complicate care delivery. When difficult behaviors are present, patients may experience worsened outcomes, and medical providers may experience distress. Most recommendations for such behaviors in the literature focus on outpatient care or inpatient psychiatric settings; recommendations for inpatient medical settings are limited. While difficult behaviors are commonly associated with personality disorders, patients without AQ6personality disorders may also exhibit these behaviors when acutely stressed, such as when they are expe

Improving healthcare worker hand hygiene adherence before patient contact: a before-and-after five-unit multimodal intervention in Tuscany.

BACKGROUND: Despite the importance of hand hygiene in reducing infection, healthcare worker compliance with hand hygiene recommendations remains low. In a previous study, we found a generally low level of compliance at baseline, with substantial differences between doctors and nurses and between hospital units. We describe here the results of our multimodal intervention intended to improve levels of healthcare worker hand hygiene. METHODS: A 6-month, before-and-after, multimodal interventional study in five hospital units in Florence, Italy. We used direct observation to assess hand hygiene r

Interdisciplinary team for the treatment of eating disorders.

Eating disorders are pathologies with a complex, multifactorial etiopathogenesis. Their treatment requires an interdisciplinary team. This paper discusses some characteristics and the most common difficulties encountered by such a team (multiple transfers, concealed alliances, role inversion, dissociation). Since their task that is carried out with patients in a critical situation and conflicting families, the team members are exposed to stressing conditions (illustrated by their dreams), and faced with possibilities of withdrawal and/or burn-out.

Effective education for patient focused staff. To be successful, you must fill the wide "gaps" between today's capabilities and tomorrow's needs.

In summary, your staff's Patient Focused education must be realigned to their unique clinical, non-clinical and multidisciplinary needs if a restructuring program is be successful. Without this realignment, traditional education services will be inadequate to meet these needs and your program may eventually fail. Perhaps Naisbitt and Auburdene said it best: "By identifying the forces pushing the future, rather than those that have contained the past, you possess the power to engage with your reality." Filling the wide "gaps" between today's educational capabilities and tomorrow's educational n

Ward atmosphere as a factor in irregular discharge from an alcohol rehabilitation unit.

A 20-bed alcohol rehabilitation unit with a high patient retention rate had the experience of a sudden increase in irregular discharges. Administration of the Ward Atmosphere Scale developed by Rudolph Moos showed a drop in various therapeutic aspects of the ward environment that paralleled the increase in irregular discharges. These results were discussed with staff, and soon after the number of irregular discharges diminished. The Ward Atmosphere Scale was readministered and showed a move toward a more therapeutic atmosphere. The reasons for these fluctuations are discussed.

Factors Related to 30-day Readmission following Hospitalization for Any Medical Reason among Patients with Mental Disorders: Facteurs liés à la réhospitalisation à 30 jours suivant une hospitalisation pour une raison médicale chez des patients souffrant de troubles mentaux.

OBJECTIVE: This study evaluated the contributions of clinical, sociodemographic, and service use variables to the risk of early readmission, defined as readmission within 30 days of discharge following hospitalization for any medical reason (mental or physical illnesses), among patients with mental disorders in Quebec (Canada). METHODS: In this longitudinal study, 2,954 hospitalized patients who had visited 1 of 6 Quebec emergency departments (ED) in 2014 to 2015 (index year) were identified through clinical administrative databanks. The first hospitalization was considered that may have occu
