Phenobarbital-induced alterations in the metabolism of [3H]vitamin D3 by the perfused rachitic rat liver in vitro.

Anticonvulsant therapy of seizure disorders in man is associated with the development of complications involving bone and mineral metabolism including hypocalcemia, elevated serum immunoreactive parathyroid hormone levels, and increased amounts of unmineralized bone or osteoid. The latter has been attributed to a reduction in serum-25-hydroxycholecalciferol levels resulting from increased hepatic metabolism of vitamin D. Using an in vitro recycling hepatic perfusion system, we have demonstrated that 5 d of phenobarbital treatment increases the hepatic production of [(3)H]25-hydroxyvitamin D(3)

Vitamin D, bone metabolism and fracture risk.

Insufficient serum levels of 25OH vitamin D (25OHD) is a risk factor for osteoporosis. A new paradigm has emerged with the locally synthesized 1,25(OH)2D within osteoblasts and osteoclasts as the essential pathway for the effects of 25OHD in regulating bone remodeling via direct or indirect activation of the specific receptor VDR. Vitamin D has positive effects on fracture risk but these results have been consistently observed whenever daily doses were above 800 UI/d administered to compliant patients together with adequate calcium supplementation and with an achieved biological target of seru

FGF-23 and vitamin D: don't shoot the messenger?

The discovery of fibroblast growth factor-23 (FGF-23) as a key regulator of phosphate and vitamin D metabolism has forced a rethink about the mineral and bone disorder of chronic kidney disease (CKD). FGF-23 powerfully predicts adverse cardiovascular outcomes in patients with CKD and an important question is whether treatment regimens should now be tailored to address FGF-23 levels in addition to those of calcium, phosphate, parathyroid hormone and vitamin D. Nevertheless, despite the known action of active vitamin D therapies to increase FGF-23, this should probably still form an important pa

A case of severe osteomalacia secondary to phosphate diabetes in a renal transplant recipient.

Transient hypophosphatemia is frequently observed during the first months after renal transplantation and is usually asymptomatic. Phosphate diabetes is defined as inadequate tubular phosphorus reabsorption leading to persistent renal phosphorus wasting, which is an important but overlooked cause of osteodystrophy in the post-renal transplantation population. We report the case of a 58-year-old male who presented with severe multiple osteoarticular pains within 3 months after successful first kidney transplantation. Bone disease was attributed initially to mild hyperparathyroidism secondary to

Prevalence of clinical remission in patients with sporadic idiopathic hypoparathyroidism.

BACKGROUND: Remission of disease activity is a characteristic feature of autoimmune endocrine disorders such as Graves' disease, Addison's disease and occasionally in patients with premature ovarian failure. Autoimmunity is also implicated in sporadic idiopathic hypoparathyroidism (SIH) with clinical remission of disease reported in three cases. OBJECTIVE: To assess the rate of remission in patients with sporadic idiopathic hypoparathyroidism and review the cases reported so far. SUBJECTS AND METHODS: Subjects included 53 patients (M:F, 24:29) with SIH who had been symptomatic for at least 1

[GNAS1 gene abnormality in pseudohypoparathyroidism I a].

Pseudophypoparathyroidism (PHP) is characterized by hypocalcemia, hyperphosphatemia and elevated levels of parathyroid hormone (PTH) due to resistance to PTH. PHP type I a is caused by heterozygous inactivating mutation of the GNAS1 gene, which encodes signal transducer, Gsalpha. PHP type I a is associated with Albright's osteodystrophy (AHO). Those patients who have AHO phenotype without hormone resistance are affected by pseudopseudohypoparathyroidism.

Effect of intermittent cyclical treatment with etidronate disodium (HEBP) and calcium plus alphacalcidol in postmenopausal osteoporosis.

We evaluated intermittent cyclical treatment with etidronate disodium (HEBP) and calcium plus alphacalcidol in postmenopausal osteoporosis, with special reference to bone mineral density (BMD) and prevention of spinal fracture. The patients were 40 women, over 50 years of age, with lumbo-dorsal pain and low BMD (less than 0.70 g/cm(2)), measured by dual-energy X-ray absorptiometry (DXA). The patients were randomly assigned to two groups. The first group (HEBP) received 200 mg of HEBP per day for 2 weeks, followed by 2 g calcium lactate and 0.5 microg alphacalcidol per day for the next 10 weeks

[Long-term follow-up study of bone mineral density in a patient with adult idiopathic Fanconi syndrome].

A 64-year-old woman complaining of severe lumbar pain was admitted to our hospital because of the finding of pre-existing mitral valve regurgitation on examination. Laboratory data revealed the proximal type of renal tubular acidosis, renal glucosuria, phosphaturia, generalized aminoaciduria and low-molecular-weight proteinuria. She did not have any cause of these tubular dysfunctions, and was diagnosed as adult idiopathic Fanconi syndrome. Dual energy X-ray absorptiometry (DEXA) revealed a reduction of bone mineral density in the lumbar spine to about 65% of the age-and gender-matched control

Hypocalcaemia induced by glucocorticoids in a child with hypoparathyroidism treated with 1-alpha-hydroxyvitamin D3.

Glucocorticoids tend to lower the intestinal absorption of calcium, leading to a negative calcium balance. A 7-year-old girl with hypoparathyroidism was maintained as normocalcaemic on 1-alpha-hydroxyvitamin D3 (1 alpha D3) and calcium gluconate lactate. During an episode of aplastic anaemia she was treated with prednisolone, with a subsequent dramatic fall of serum calcium despite 1 alpha D3 treatment and serum 1,25-dihydroxyvitamin D3 (1,25(OH)2D3) in the high-normal range. Glucocorticoids seem to have interfered directly with cellular events responsible for intestinal absorption of calcium.

Serum vitamin D metabolites in younger and elderly postmenopausal women.

Previous investigations have suggested that a lower-than-normal serum 1,25(OH)2D is found in elderly women with postmenopausal osteoporosis. We examined the fundamental aspects of this theory by investigating serum vitamin D metabolites in four representative samples of Caucasian women. These included 44 early postmenopausal women divided into two subgroups: fast bone losers, that is, bone loss greater than 3%/year (n = 20), and "physiological" bone loss (n = 24); and 28 70-year-old women divided into two subgroups: with and without osteoporotic fractures. Serum 1,25(OH)2D concentrations were

Effects in postmenopausal women of natural and synthetic estrogens on calcitonin and calcium-regulating hormone secretion. Relevance to postmenopausal osteoporosis.

The major function of calcitonin in man appears to be the prevention of excessive or unwanted bone resorption. There is a marked sex difference in circulating levels, with a relative deficiency in women. As calcitonin secretion in young women is known to be increased by estrogens, long periods of estrogen lack - such as the postmenopause - are possibly associated with a more pronounced calcitonin deficiency. This exaggerated deficiency could be an important factor in the pathogenesis of postmenopausal bone loss, especially as the latter is believed to be due to excessive bone resorption. We ha

Effects of diet calcium and 1,25-dihydroxyvitamin D3 on colon calcium active transport.

Unidirectional fluxes of calcium were studied in the absence of electrochemical gradients across rat descending colon segments in vitro. Dietary calcium restriction and 1,25-dihydroxyvitamin D3 [1,25-(OH)2D3]enhanced absorption by increasing the mucosal-to-serosal fluxes, whereas secretory (serosal-to-mucosal) fluxes were unchanged. Low-calcium diet also stimulated calcium uptake by everted gut sac segments of ascending as well as descending colon, whereas transverse colon was unresponsive. These results show that the colon is a target organ for 1,25-(OH)2D3 and demonstrate the participation o

1,25-dihydroxycholesterol treatment of long-term haemodialysed patients with renal osteodystrophy.

Renal osteodystrophy still poses major problems in the management of chronic renal failure, though it had been extensively studied over the last years. The present therapeutic possibilities - massive-dose vitamin D, aluminium hydroxide and supplementary Ca-intake, administration of active vitamin D metabolites - are discussed in the light of personal observations.

[Bone and calcium metabolism in adult hypothyroidism (author's transl)].

A study of blood and urinary calcium and phosphorus, 25 hydroxyvitamin D plasma level (25 OH D3), bone radiology and hystology is performed in 12 hypothyroïd adult patients. This study confirms the notion of myxoedematous ostéopetrosis. Radiologic and histologic findings shows an osteocondensation, more important on the skull, and augmentation of cortical thickness of the bones is noted. Quantitative histologic findings, shows some signs of diminution of bone cellular activity. Low plasma level of 25 OH D3 in these patients is discussed in the view of an event of a disturb in the metabolism of

Prevalence, pathogenesis and treatment of post-menopausal osteoporosis.

About 11% of post-menopausal women with wrist fractures have spinal osteoporosis with compressed vertebrae, and about 25% of postmenopausal osteoporosis patients have had a wrist fracture. The estimated prevalence of post-menopausal spinal osteoporosis is 4% of the female population at age 60 and about 8% at age 80. Osteoporotic patients have lower plasma oestrone and androstenedione levels, lower calcium absorption and higher urinary hydroxyproline than matched controls. Of six treatments tested in three different ways, the least successful were vitamin D2 and 1 alpha-OHD3 and the most succes

Regulation of the vitamin D endocrine system located in the kidney.

During the past decade, it has been clearly established that the kidney is an endocrine organ which converts 25-hydroxyvitamin D to the active hormone 1,25-dihydroxyvitamin D. The kidney and intestine are the sites of an alternative hydroxylation which places an 24 hydroxyl in the R position on either 25-hydroxyvitamin D or 1,25-dihydroxyvitamin D. The production of 1,25-dihydroxyvitamin D is stimulated by hypocalcemia through the parathyroid gland, hypophosphatemia, the sex hormones and the peptide hormones insulin and prolactin. This vitamin D based endocrine system is of central importance

Oral 25-hydroxyvitain D3 in treatment of osteomalacia associated with ileal resection and cholestyramine therapy.

Severe histological osteomalacia developed in a woman with Crohn's disease 2 years after ileal resection and the start of cholestyramine therapy. Treatment with oral 25-hydroxyvitamin D3, 50 microgram daily, produced marked biochemical, radiological, and histological improvement after 6 months. It is suggested that cholestyramine, by reducing vitamin D absorption, precipitated the rapid development of osteomalacia in this patient. This case report emphasizes the importance of routine vitamin D supplementation in all patients on long term cholestyramine therapy, and indicates that even in patie

Effect of 1,25-dihydroxycholecalciferol on ethanol mediated suppression of calcium absorption.

Rats raised on laboratory diet and given 20% ethanol in water as a sole source of fluid for 12 days were given 0.2 mug of 1,25-dihydroxycholecalciferol 24 h before measuring duodenal calcium transport by an in vitro everted gut sac technique. This vitamin D metabolite did not reverse the ethanol-induced inhibitiom of transport capacity. Ultrastructural studies of intestinal epithelial cells from animals ingesting ethanol demonstrated changes in mitochondria and other subcellular organelles as well as accumulation of electron-dense, calcium-containing granules in microvilli and a paucity of gra
