[Insect sting allergy--clinical aspects, diagnosis and therapy].

Systemic allergic reactions following insect stings, most often by Hymenoptera (honey bee, wasp), are usually of the immediate type and thus mediated by venom specific IgE-antibodies. The most frequent clinical symptoms are: Urticaria, angioedema, asthma and anaphylactic shock. While up to 4% of individuals in epidemiological surveys give a history of systemic allergic reactions following Hymenoptera sings, fatal cases are extremely rare. Diagnosis is based on history, skin test and estimation of venom specific serum IgE-antibodies (RAST). For treatment of mild systemic reactions antihistamine…

Allergy to bumblebee venom.

Allergy to bumblebee venom is a rare form of Hymenoptera venom allergy. Because bumblebees are increasingly used for the pollination of greenhouse plants, the prevalence of this Hymenoptera allergy has increased during the past decade. The clinical presentation, diagnosis and therapy of bumblebee venom allergy are similar to other Hymenoptera venom allergies. There is a significant immunological cross-reactivity between bumblebee and honeybee venom. It has been claimed that immunotherapy with honeybee venom can protect patients with bumblebee venom allergy. This concept, however, has been call…

Arthropod poisons and the cardiovascular system.

Animal poisons induce myocardial damage after envenomation. The cardiac manifestations after black widow spider bite are rarely observed and their prognostic significance are not known. Hymenoptera induce anaphylactic manifestations commonly observed in adults with previous coronary artery disease although myocardial involvement is observed in a few without previous heart disease. The mechanism of cardiac involvement after scorpion envenomation has been elucidated, however after widow spider bite or hymenoptera stings the mechanism of myocardial damage of those without previous heart disease i…

Floor of the nose mucosa lysis and labial abscess caused by a bee sting.

Hymenoptera order includes bees, which have a stinging apparatus at the tail capable of delivering venom to the affected tissues. Myocardial infarction, acute renal failure, Necrotizing fasciitis, fatal infection and hemifacial asymmetry, are some of the unusual reactions reported following hymenoptera stings. This paper reports a case of bee sting in the right floor of the nose that mimicked an odontogenic infection affecting the upper lip, canine space and nasal cavity such as in cases of infection secondary to pulpal or periodontal pathology of the anterior teeth. After a thorough clinical …

Imported fire ant hypersensitivity. Studies of human reactions to fire ant venom.

It is now apparent that venom and venom components of the Hymenoptera superfamilies of Apida (honeybee) and Vespida (wasps, yellow jackets, and hornets) are becoming increasingly important in the diagnosis and treatment of hypersensitivity reactions. Stings from fire ants (superfamily Formicidae, family Myrmicinae) have also been recognized as causes of systemic reactions in man. Fire ant venom is unique in its composition, consisting mainly of alkaloids in aqueous suspension with only trace amounts of protein. This study compares the skin reactivity of fire ant venom and synthesized alkaloid …

The sudden onset of mastocytosis in the course of venom-induced anaphylactic reaction in a patient with myelodysplastic syndrome.

Mastocytosis is a disease resulting from a proliferation of clonal, abnormal mast cells in tissues and organs, defined as Philadelphia-negative myeloproliferative neoplasm. We present a male patient with clinically, morphologically and immunohistochemically confirmed mastocytosis with preceding myelodysplastic syndrome, occurred after wasp bite in the course of anaphylactic reaction. The propensity to hymenoptera venom-induced anaphylaxis and the presence of an increased population of atypical mast cells in bone marrow found after anaphylactic shock may suggest the possible relationship betwee…

Bio-activities of Clerodendrum inerme extracts, essential oil, main terpenes and Ag nanoparticles against the stable fly, Stomoxys calcitrans (Diptera: Muscidae) and their non-target effects against its parasitoid, Spalangia cameroni (Hymenoptera: Pteromalidae) and earthworms.

The stable fly, Stomoxys calcitrans, is a cosmopolitan noxious dipteran hematophagous insect of veterinary and medical importance. There is a renewed interest to develop eco-friendly green pesticides to control stable flies while avoiding the health and environmental hazards of synthetic pesticides. In that framework, Clerodendrum inerme essential oil (EO) was produced by hydrodistillation, and analysed by using (GC-FID) and GC-MS). Isoeugenol (33.1 %) and dibutyl phthalate (29.3 %) were identified as the main components. Silver nanoparticles (Ci-based AgNPs were produced using the methanol (M

Anaphylaxis as occupational risk.

PURPOSE OF REVIEW: Anaphylaxis is a severe form of allergic reaction that may cause death. Main triggers of anaphylaxis have been reported to also cause occupational anaphylaxis (OcAn). The purpose of this article was to summarize the current scientific evidence on OcAn and review the specific literature published in 2012 and 2013. RECENT FINDINGS: Allergens derived from Hymenoptera and natural rubber latex are the most frequently reported triggers of OcAn, but other high and low-molecular weight agents have been described. Among the latter, foods, insects, snakes, chemicals, and medications …

Patients with or without a clonal mast cell disease undergoing venom immunotherapy differ in their CD8 T memory cell response.

Hymenoptera venom immunotherapy (VIT) is usually effective in inducing long-term tolerance to the allergen. In patients with a clonal mast cell disease (CMD) and Hymenoptera venom-induced anaphylaxis, however, life-long VIT is recommended, as tolerance to the venom is often lost after VIT termination. We compared blood samples from patients with CMD and non-CMD with severe Hymenoptera venom-induced anaphylaxis before and more than 3 years after VIT to identify differences that might explain the lack of long-term tolerance in patients with CMD. Serum tryptase, IgE, IgG4, KIT D816V variation, as…

Clinical Utility of Rush Venom Immunotherapy: Current Status.

Hymenoptera venom allergy (HVA) is the leading cause of anaphylactic reactions in adults and the second most common cause in children. Venom immunotherapy (VIT) is used to elicit an immune tolerance against hymenoptera venom in allergic patients and is based on the administration of purified venom extracts regularly for defined periods. The protocols of administration include 2 phases: an up-dosing phase that incrementally reaches the final dose resulting in a protective effect, and a maintenance phase in order to obtain the sustained effect. The goal of this review is to detail the efficacy a…

Aortic prosthetic graft thrombosis after bee sting-induced anaphylaxis.

Hymenoptera stings may cause severe cardiovascular events through direct prothrombotic venom effects and anaphylaxis-related hemodynamic collapse. We report the case of a 62-year-old male beekeeper with an infrarenal aortoaortic prosthetic graft who developed grade 3 anaphylaxis after a bee sting. Within 2 hours, he experienced bilateral lower limb paresthesia and severe pain. Computed tomography angiography revealed complete graft thrombosis extending into the left iliac axis. Emergency axillobifemoral bypass restored perfusion. Postoperative complications included compartment syndrome, acute…

An isolated bee sting involving multiple cranial nerves.

Hymenoptera stings are self-limiting events or due to allergic reactions. Sometimes envenomation with Hymenoptera can cause rare complications such as acute encephalopathy, peripheral neuritis, acute renal failure, nephrotic syndrome, silent myocardial infarction, rhabdomyolysis, conjunctivitis, corneal infiltration, lens subluxation, and optic neuropathy. The mechanism of peripheral nervous system damage is not clearly known. In our studied case after bee sting on face between the eyebrows with little erythema and 1 × 1 cm in size, bilateral blindness developed and gradually improved. Lateral…

Naturally occurring parasitoids of Drosophila suzukii (Diptera: Drosophilidae) and other drosophilids in California fruit regions.

Drosophila suzukii (Matsumura) (Diptera: Drosophilidae) has become a damaging economic pest of small fruits in the invaded range in the Americas, Europe, and North Africa. This study surveyed naturally occurring parasitoids of D. suzukii and other frugivorous Drosophilidae in California's coastal and interior fruit production regions. Surveys were conducted from 2012 to 2018 through collections of infested fruits and the use of sentinel fruit or host traps. Two pupal parasitoids, Pachycrepoideus vindemiae (Rondani) (Hymenoptera: Pteromalidae) and Trichopria drosophilae (Perkins) (Hymenoptera: …

[Insect venom allergy].

Systemic allergic reactions to hymenoptera stings are most often mediated by venom-specific IgE antibodies. The most frequent symptoms are urticaria, angioedema, asthma and anaphylactic shock. Rarely, cerebral or myocardial ischemia with permanent damage may result from severe anaphylactic shock. Each year several individuals die in Switzerland from allergic reactions following hymenoptera stings. The most valuable diagnostic tests are skin tests and estimation of venom-specific serum-IgE antibodies. The most important drugs for emergency treatment are adrenaline and antihistamines. During ana…

[The human basophil degranulation test. Attempt at determining its reliability].

A single-blind assay was made between a laboratory in Paris and our own, in order to assess objectively the Human Basophil Degranulation test (TDBH). The study was made on 28 subjects. They presented with anaphylactic or systemic reactions to hymenoptera venom. The samples from these patients were sent to Paris without any other diagnostic information. Agreement of results between ANGERS and PARIS was very poor, at around 38.5%. Agreement between the TDBH at ANGERS and PARIS with other biological parameters and skin tests was also weak, at 41.7 to 55% in ANGERS and 45.8 to 58.3% in PARIS. The …

Predicting cross reactivity between the fire ant venom allergen Sol i 3 and the antigen 5 from vespid venoms based on molecular modeling computing.

The fire ant Solenopsis invicta is an aggressive invasive species whose venom frequently triggers hypersensitivity reactions, including severe anaphylaxis. In endemic regions, its stings represent a significant cause of Hymenoptera-related allergy. Four venom allergens have been identified - phospholipase A1 (Sol i 1), antigen 2 (Sol i 2), antigen 3 (Sol i 3), and antigen 4 (Sol i 4) - with Sol i 3 recognized as the predominant sensitizer. However, the molecular determinants that drive Sol i 3 allergenicity and its potential cross-reactivity with other Hymenoptera venoms remain insufficiently …
