UNLABELLED: The aim of the study was to investigate the accuracy of iodomethyl norcholesterol, a new adrenal isotopic scanning agent, in the strategy of aldosteronism localization. Among 1499 patients examined in the clinic in 1987, 49 presented with primary aldosteronism. Nine were explored by adrenal scintigraphy (SCI). Mean age was 50 +/- 10 ans, blood pressure was 188 +/- 26/110 +/- 17 mmHg. Initial serum kalemia was 3.2 +/- 0.4 mMol/l, urinary potassium 67 +/- 39 mMol/d; standing plasma active renin was 9.9 +/- 5.0 pg/ml (20 less than N less than 50), supine plasma aldosterone was 316 +/-

The rare association of parathyroid adenoma with primary hyperaldosteronism, occurring in each of 2 patients, was recently reported from this unit. The present communication records the subsequent development, in one of these patients, of Cushing's syndrome with an anterior pituitary adenoma. Screening for other endocrine abnormalities was negative. A literature search failed to produce any other cases of the association of these three conditions in one patient.

Psychiatrist manifestations-especially acute psychotic episode-of primary hyperaldosteronism is not very common in the scientific literature. The main symptoms are high blood pressure and hypokalemia, the diagnosis seems to be based on the determination of aldosterone and plasma renin activity, also the treatment includes majorly adrenalectomy. The take-away lesson from this case is to think of organic cause in front of acute psychotic episode.

A less-than-normal decline in nocturnal blood pressure (BP) has been associated with excessive hypertensive complications. This is concerning because secondary hypertension is often associated with this so-called nondipper BP profile. Unilateral adrenalectomy was performed in 32 patients with primary hyperaldosteronism due to tumour of the adrenal gland and with arterial hypertension (AH) of various degree of severity. A rare case of primary hyperaldosteronism due to corticosuprarenal carcinoma is presented. The case was treated by suprarenalectomy and is currently under antiblastic treatment with mitotane (o-p'DDD). The clinical and endocrinological elements for differential diagnosis from the much more common suprarenal adenomas are examined in particular detail.

Twenty-nine patients with primary hyperaldosteronism were evaluated with computed tomography (CT), adrenal venous sampling, and adrenal venography. Twenty-three patients had aldosteronomas and six had bilateral adrenocortical hyperplasia. Sixteen (70%) of the adenomas were accurately located by CT. All nodules of 1.5 cm or larger diameter and 50% of nodules 1.0-1.4 cm in diameter were demonstrated. Nodules of less than 1.0 cm in diameter generally were not detected. In 29 untreated resting patients with cirrhosis of the liver and ascites plasma aldosterone levels were determined. The determinations were performed with a radioimmunoassay using highly specific antibodies without chromatography.
