Mutations in Bruton's tyrosine kinase impair IgA responses.

X-linked agammaglobulinemia (XLA) is a primary immunodeficiency caused by mutations in Bruton's tyrosine kinase (BTK), and is characterized by markedly decreased numbers of blood B cells and an absence of all immunoglobulin isotypes. We performed whole exome sequencing in a male pediatric patient with dysgammaglobulinemia with IgA deficiency. Genetic analysis revealed a BTK missense mutation (Thr316Ala). To investigate whether a BTK mutation underlay this antibody deficiency with marked decrease of IgA in this patient, we performed functional analyses of B cells and phagocytes, and molecular a…

IgA facilitates the persistence of the mucosal pathogen Helicobacter pylori.

IgA antibodies have an important role in clearing mucosal pathogens. In this study, we have examined the contribution of IgA to the immune c … H. pylori is controlled more efficiently in IgA-/- mice than their WT counterparts. The effects of IgA deficiency of the offspring can be compensated by maternal IgA delivered by WT foster mothers. We attribute the improved immune control observed in IgA-/- mice to IgA-mediated protection from complement killing, as H. pylori colonization is restored to wild ty …

Screening by anti-endomysium antibody for celiac disease in diabetic children and adolescents in Austria.

BACKGROUND: Unrecognized celiac disease (CD) may be found in a substantial proportion of patients with type I diabetes mellitus. METHODS: A cohort of 403 Austrian children and adolescents with type I diabetes mellitus (210 males and 193 females; age range, 1-22 years) was screened for celiac disease using the IgA anti-endomysium antibody test (EMA) and the immunoglobulin (Ig)G anti-gliadin (AGA-IgG) and IgA anti-gliadin (AGA-IgA) antibody test. RESULTS: Twelve patients' sera (2.98%) yielded positive EMA results and two patients' sera (0.49%) with IgA deficiency had high AGA-IgG values. All b…

IgA1 is the major immunoglobulin component of immune complexes in the acquired immune deficiency syndrome.

Compared to a panel of healthy controls, sera from 13 of 23 (57%) patients with the acquired immune deficiency syndrome (AIDS) were shown to have elevated levels of circulating immune complexes (CIC) containing IgA. Levels of IgG-containing CIC were increased in seven patients (30%); no patients had elevated levels of IgM-containing CIC. Additional experiments showed that in all instances in which IgG CIC were demonstrable, IgA was also present; however, IgA CIC could be found that did not contain IgG. The IgA in the CIC was restricted to the IgA1 subclass. These data suggest selective abnorma

Decreased IgA antibody production in the stomach of gastric adenocarcinoma patients.

Gastric adenocarcinoma is closely associated with Helicobacter pylori infection. It is also much more frequent in patients with common variable immunodeficiency or selective IgA-deficiency than in the general population. To investigate a possible link between local antibody production and gastric tumors, we studied gastric B cell infiltration and local IgA production in patients with H. pylori induced gastric adenocarcinomas. These studies showed that total and H. pylori-specific IgA antibody levels were substantially lower in gastric tissue from the cancer patients compared to those from asym

Chron's disease, rare association with selective IgA immunodeficiency, and development of life-threatening bacterial infections.

Life-threatening necrotizing fasciitis and relapsing Lemierre syndrome associated with Fusobacterium necrophorum septicaemia occurred in young adults with a moderate Chron's disease and a missed profound IgA deficiency. This unexpected association of a chronic bowel inflammatory syndrome with prominent IgA abnormalities and severe bacterial infection deserves careful attention by physicians faced with young patients with Chron's disease.

Mucosal secretory IgA and secretory piece in adult coeliac disease.

Immunofluorescence studies with specific antisera to secretory IgA (11S IgA) and secretory piece were carried out on the jejunal mucosa of nine patients with adult coeliac disease (treated and untreated). The results were compared with those obtained in four normal patients and in four patients with local and systemic IgA (7S) deficiency. 11S IgA and secretory piece were localized to the upper third of the epithelial cells of both surface and glandular epithelium in all groups of patients. However, in the untreated coeliac patients fluorescence was also demonstrated in the basement membrane an

[Antibody deficiency states in children].

In more than 2500 immunoelectrophoreses and quantitative immunoglobulin determinations 19 cases of transitory hypogammaglobulinaemia, two cases of sex-linked congenital agammaglobulinaemia, six cases of selective IgA deficiency and nine cases of dysgammaglobulinaemia were diagnosed. In congenital agammaglobulinaemia life-long substitution with gammaglobulin must be instituted. Transitory hypogammaglobulinaemia which occurs mostly at the beginning of the second trimenon usually regresses spontaneously at the age of 15 to 18 months. The increased susceptibility to infections can be overcome by g…

The effect of magnesium deficiency in mice on serum immunoglobulin concentrations and antibody plaque-forming cells.

Magnesium-deficient mice immunized with SRBC showed a significant decrease (P less than 0.001) in the number of PFC in their spleens compared with mice on a control diet. Serum immunoglobulin concentrations (IgG1, IgG2, IgA and IgM) were determined after 3, 6, 9 and 12 days on the respective diets. The serum IgG2 and IgM concentrations of magnesium- deficient mice were decreased (P less than 0.005) by 6 days and remained at these concentrations until 12 days. The serum IgG1 and IgA concentrations of magnesium deficient animals also decreased (P less than 0.01) by 6 days but returned toward con

A Nonsense SMAD3 Mutation in a Girl with Familial Thoracic Aortic Aneurysm and Dissection without Joint Abnormality.

INTRODUCTION: Thoracic aortic aneurysms and dissections (TAAD) are rare in children and often associated with underlying genetic disorders accompanied with other systemic manifestations, including connective or osteo-articular tissue diseases. CASE PRESENTATION: We report the case of a 10-year-old girl with a novel nonsense SMAD3 mutation, p.Glu102X, who presented with familial TAAD without any signs of osteoarthritis. Histological analysis of aorta fragments from the patient with TAAD obtained during surgery revealed elastin degradation and inflammatory infiltration of T cells with dense CD3

Systemic Ozone Therapy by Rectal Insufflation for Immunoglobulin A Deficiency.

INTRODUCTION: IgA deficiency is a primary immunodeficiency predominantly due to an antibody defect, for which there is no replacement therapy. Treatment consists of prevention and treatment of infections and other associated conditions. Given the immunomodulatory and regulatory properties of the redox balance of ozone therapy in infectious and inflammatory conditions, evaluation of its effect on IgA deficiency is of interest. OBJECTIVE: Assess the benefits and possible adverse effects of ozone treatment in patients with IgA deficiency. METHODS: A monocentric randomized controlled phase 2 cli…

Common variable immunodeficiency disorders: What generalists should know.

Primary immune deficiency disorders (PIDDs) are common and underdiagnosed. Predominant antibody deficiencies (PADs) are the most common type of immune deficiency and comprise 55% of the immune deficiencies diagnosed.[1] Although immunoglobulin A (IgA) deficiency remains the most common type of PID, common variable immunodeficiency disorders remain the most common symptomatic PID for which medical therapy is sought.

Abnormalities in the distribution of serum immunoglobulin concentrations in juvenile rheumatoid arthritis.

Concentrations of serum IgG. IgA, and IgM were determined in 200 patients with juvenile rheumatoid arthritis. The relative frequency distribution of IgG and IgM approached that of a log-normal curve; however, there was marked skewing of the distribution of the serum concentrations of IgA. The prevalence of selective IgA deficiency was 4%. In order to permit further intragroup comparisons, the serum immunoglobulin concentrations were standardized by comparison to a sex-age matched control group. By this process it was found that there was concordance of the serum levels of IgG with IgA, and IgG…
