BACKGROUND: The aims of the study were to determine the radiological leak rate in those patients who had undergone a resection for left-sided colorectal cancer and to see if the presence of a leak can be related with the postoperative clinical period. We also aimed to identify any common factors between patients with leak.
METHODS: A retrospective analysis of prospectively collected data of all patients who underwent a left-sided colorectal cancer resection with formation of a defunctioning ileostomy was undertaken. Between 2005 and 2010, 418 such patients were identified.
RESULTS: A water-s…

BACKGROUND: De novo Crohn’s disease (CD) of the neo-small intestine in ulcerative colitis (UC) patients after total proctocolectomy (TPC) is a new disease entity, which may persist even after a secondary diverting permanent ileostomy for pouch failure. We sought to compare outcomes of primary ileostomy (PI, i.e., stoma created after colectomy without trying of ileal pouch) and secondary ileostomy (SI, i.e., stoma created after pouch failure) and to evaluate factors associated with the development of CD of the neo-small intestine proximal to ileostomy.
METHODS: A total of 123 eligible patients…

To evaluate the effectiveness of fecal transplantation (FT) in preventing the development of diversion colitis (DC) and reducing its complications in patients who underwent protective loop ileostomy (PLI) following low anterior resection for rectal cancer. The study was prospectively conducted at Tertiary Hospital. Seventeen patients who underwent PLI were assigned to the FT group (FT+), and 19 patients served as the control group (FT-). In the FT+ group, ileal content was delivered to the efferent loop via a catheter placed during surgery. Both groups were evaluated postoperatively through bi…

PURPOSE: To evaluate the predictive value of the Prognostic Nutritional Index (PNI) and multiple factors for early stoma-related complications in patients undergoing prophylactic ileostomy.
METHODS: A retrospective analysis was conducted in a cohort of 229 patients who underwent prophylactic ileostomy. The optimal cut-off value of the PNI was determined using receiver operating characteristic (ROC) curve analysis. Based on this threshold, patients were stratified into a low nutritional risk group (PNI >45.55, n = 122) and a high nutritional risk group (PNI ≤45.55, n = 107). Univariate and mul…

BACKGROUND: Prolapsing technique is a type of natural orifice specimen extraction surgery that can overcome the difficulty of precise transection of the distal rectum and subsequent anastomosis in a narrow pelvic space. Currently, protective ileostomy is widely utilized in low anterior resection for low rectal cancer, which may reduce the severe consequences caused by anastomotic leakage. The study aimed to combine the prolapsing technique with a one-stitch method of ileostomy and evaluate the surgical outcomes.
METHODS: A retrospective analysis was conducted on patients with low rectal cance…

This case report describes a novel procedure for opening the lumen of a completely obstructed anastomosis when open surgery is not an option. Two patients underwent ileocecal or colorectal resection and one-stage anastomosis reconstruction without diverging ileostomy. The patients developed post-surgical abdominal distension and nausea. Emergency imaging indicated complete anastomotic obstruction and distal intestinal anastomosis emptiness. Colonoscopy revealed an anastomosis that was completely discontinued by a membranous structure. Considering that open surgery was not a viable treatment op…

AIM: Postoperative ileus is the most commonly observed morbidity following ileostomy closure. Studies have demonstrated that the defunctionalized bowel of a loop ileostomy undergoes a series of functional and structural changes, such as atrophy of the intestinal villi and muscular layers, which may contribute to ileus. A single-centre study in Spain demonstrated that preoperative bowel stimulation via the distal limb of the loop ileostomy decreased postoperative ileus, length of stay and time to gastrointestinal function.
METHOD: A multicentre randomized controlled trial involving patients fr…

BACKGROUND: When restorative proctocolectomy (RPC) is performed, a temporary diverting loop ileostomy is often fashioned and usually closed 2-3 months later. Pouchography is used to assess pouch integrity, although its benefits have been questioned and no definitive data support its routine use. Our aim was to assess the utility of pouchography before ileostomy closure in patients with a negative clinical examination.
METHODS: We retrospectively reviewed our database of patients who underwent ileostomy takedown between 1987 and 2010. Two hundred and thirty-two patients were identified who und…

A delayed colo-anal anastomosis has been proposed as a way to avoid diverting stoma after low anterior resection. Surgical and functional results were reviewed in 17 patients operated between 1999 and 2007 using this technique. Complications included one colonic necrosis, two pelvic abscesses and one colovaginal fistula. Results of continence and quality of life scores were satisfactory. Rates of parietal and septic complications are low after delayed colo-anal anastomosis and functional results are good. The use of this technique is particularly effective to avoid diverting ileostomy and for …

BACKGROUND: Strategies to optimise bowel function before stoma reversal surgery (SRS) are needed to improve postoperative outcomes. Efferent bowel stimulation (EBS) prior to stoma closure has been proposed as a potential strategy to reduce postoperative ileus (POI) incidence and improve the patient's postoperative course.
METHODS: A multicentre randomised controlled trial was designed to evaluate the effectiveness of pre-SRS EBS in reducing POI incidence. Patients scheduled for elective SRS were randomised 1:1 to either an intervention group, receiving daily EBS with saline and a nutritional

PURPOSE: No consensus exists regarding the use of preoperative bowel preparation for patients undergoing a low anterior resection (LAR). Several comparative studies show similar outcomes when a single time enema (STE) is compared with mechanical bowel preparation (MBP). It is hypothesized that STE is comparable with MBP due to a decrease in intestinal motility distal of a newly constructed diverting ileostomy (DI).
METHODS: In this prospective single-centre cohort study, patients undergoing a LAR with primary anastomosis and DI construction were given a STE 2 h pre-operatively. Radio-opaque m…

BACKGROUND/AIM: There is ample evidence in the recent literature that gum chewing after elective colonic anastomosis decreases postoperative ileus (POI). But there are very few studies on small bowel anastomosis done in relaparotomy cases. This study aimed to evaluate the effect of gum chewing on the duration of POI following small bowel anastomosis performed for the closure of intestinal stoma, made as temporary diversion in the selected cases of typhoid perforation peritonitis.
PATIENTS AND METHODS: Hundred patients undergoing elective small bowel anastomosis for the closure of stoma were r

BACKGROUND: Fulminant Clostridioides difficile colitis (CDC) carries high morbidity and mortality, often necessitating emergent surgical intervention. While total abdominal colectomy (TAC) remains the standard of care, diverting loop ileostomy with antegrade colonic lavage (DLI) has emerged as a colon-preserving alternative. This study aimed to compare outcomes between DLI and TAC and evaluate the safety of a DLI first approach, including cases that ultimately failed DLI.
METHODS: We performed a retrospective analysis of adult patients with CDC undergoing TAC or DLI between 2012 and 2021 usin…

INTRODUCTION: Uncomplicated surgical approaches that minimize anastomotic complications while improving revisional metabolic/bariatric surgical (MBS) outcomes are needed.
METHODS: This prospective single-center study assessed the feasibility, safety, and efficacy of the novel linear magnetic anastomosis system (LMAS [3 cm]) in performing a side-to-side duodeno-ileostomy (MagDI) bipartition to revise clinically suboptimal primary sleeve gastrectomy (SG). Patients with severe obesity with/without type 2 diabetes (T2D) with suboptimal weight loss, regain, and/or T2D recurrence post SG underwent …
