Intravenous immune globulin: dosage and administration.

Intravenous immune globulin (IV-IgG) is approved for use in substitution therapy of immunodeficiency syndromes and treatment of idiopathic thrombocytopenic purpura (ITP). The initial dose for substitution therapy is 0.2 g/kg body weight repeated monthly. If necessary, the dose may be increased to 0.3 g/kg and the frequency to every 2-3 weeks. The approved dosage for ITP is 0.4 g/kg daily for 2 to 5 consecutive days. Repeat doses of 0.4 g/kg have been used as maintenance therapy. The initial infusions of IgG to immunodeficient patients who have not been treated during the previous 8 weeks or ne

Serum anti-lipid A antibodies in multiple myeloma and Waldenström's macroglobulinaemia.

Anti-lipid A antibodies were determined in sera from 38 patients with IgG multiple myeloma, 33 patients with IgA multiple myeloma and 38 patients with Waldenström's macroglobulinaemia using an enzyme-linked immunosorbent assay (ELISA). Compared to 34 healthy adults, significantly (p less than 0.025, p less than 0.005, p less than 0.0025) lower serum anti-lipid A antibody levels were measured for the respective patient groups. Low anti-lipid A antibody levels correlated with a higher infection rate with gram-negative bacteria in patients with monoclonal B-cell malignancies. The highest infectio

Subacute sclerosing panencephalitis and hypoparathyroidism.

Subacute sclerosing panencephalitis (SSPE) occurred in 3 unrelated children affected with different genetic forms of hypoparathyroidism (HPT). Clinical or laboratory evidence of an immunological deficit before SSPE onset was ascertained in cases 1 and 3. At the onset of SSPE, all cases showed signs of increased central nervous system (CNS) excitability due to poorly regulated blood calcium levels, and cases 1 and 2 had papilledema. Epidemiological evaluation suggests that the association of SSPE and HPT is not accidental. In HPT, both a subtle immunological deficit and CNS involvement may be p

Fatal progressive vaccinia in two immunodeficient infants.

Fatal progressive vaccinia developed in two infants, a girl and a boy, vaccinated at the age of 2 months. Immunodeficiencies comprised both humoral and cell-mediated immunity. In the girl low levels of immunoglobulins and a defect function of lymphocytes was demonstrated. The boy had hypogammaglobulinemia and lack of T-lymphocytes. There was a marked hypoplasia of thymus and lymphoid organs in both infants. Both infants had the haplotype A3, B8 and in the boy a crossing over had taken place within the HLA region. No effect was achieved with antivaccinia immunoglobulin and concomitant antiviral

Adenocarcinoma of the stomach with common variable immunodeficiency syndrome.

Two patients had adenocarcinoma of the stomach in association with common variable immunodeficiency syndrome. There has been an increased prevalence of malignancy in this late-onset immunodeficient state. Similar to five previously reported cases, our patients had gastric carcinoma as a late complication. Further documentation of this association stresses the need for long-term follow-up in this premalignant condition.

Atopy as a minimal immunodeficiency?

Despite impressive recent advances in the understanding of the chemical and cellular bases of the reaginic response, the pathogenesis of atopic diseases still remains a matter of speculation. The frequent finding of atopic diseases in some primary immunodeficiencies such as selective IgA deficiency and the Wiskott-Aldrich syndrome offers a unique opportunity for studying the immune mechanisms underlying the genesis of atopy. Recent studies in subjects with selective IgA deficiency have challenged the well known hypothesis that atopy is the result of defective "immune exclusion" by the secretor

A State of the Art Review on the Recalcitrant Candidiasis as a Clue of Inborn Errors of Immunity in Newborns.

BACKGROUND: Defects in the development or function of the immune system are characteristic of inborn errors of immunity (IEI), a diverse group of inherited disorders. IEI can manifest in various ways during the neonatal stage, posing a challenge in their identification and treatment. The family who have a background of IEI, including a previous instance of an infant's death caused by infection, sepsis from bacteria in children requiring intravenous (IV) antibiotics, and failure to thrive, as well as newborns with fungus infection such as chronic recalcitrant candidiasis. This review sought to

Insights into Clinical Challenges and Management of Primary and Secondary Antibody Deficiency in Pregnancy.

Pregnancy in patients with primary or secondary antibody deficiencies presents unique clinical challenges shaped by impaired humoral immunity, increased susceptibility to infection, and decreased maternal-fetal placental IgG transfer. Immunologic shifts across gestation, limited B-cell reconstitution after anti-CD20 therapy, and alterations in IgG metabolism further complicate maternal and neonatal outcomes. While earlier diagnosis, advances in immunoglobulin replacement therapy (IgRT), antimicrobial prophylaxis, and management of infectious/noninfectious complications have enabled many affect

Reported Demographics of Primary Immunodeficiency Diseases in the United States.

BACKGROUND: Primary immunodeficiency diseases (PIDDs) are rare genetic disorders impairing immunity. Studies evaluating diagnostic rates of PIDDs in historically marginalized US populations are limited. OBJECTIVE: To conduct a scoping review that identifies the extent of race and ethnicity reporting in US-based observational studies of people with PIDDs, and the demographic composition of study populations compared with the broader US population. METHODS: We conducted pragmatic searches of MEDLINE in April 2024 and ultimately included studies dating back 10 years. Results were screened and e
