A randomized placebo-controlled trial of D-cycloserine to enhance exposure therapy for posttraumatic stress disorder.

BACKGROUND: Posttraumatic stress disorder (PTSD) is a complex and debilitating anxiety disorder, and, although prolonged exposure therapy has been proven effective, many patients remain symptomatic after treatment. In other anxiety disorders, the supplementary use of D-cycloserine (DCS), a partial agonist at the glutamatergic N-methyl-D-aspartate receptor, showed promise in enhancing treatment effects. We examined whether augmentation of prolonged exposure therapy for PTSD with DCS enhances treatment efficacy. METHODS: In a randomized, double-blind, placebo-controlled trial we administered 50

Cognitive behavioral treatment for childhood anxiety disorders: long-term effects on anxiety and secondary disorders in young adulthood.

BACKGROUND: The present study's aim was to examine the long-term effects (8 to 13 years post-treatment; M = 9.83 years; SD = 1.71) of the most widely used treatment approaches of exposure-based cognitive behavioral treatment for phobic and anxiety disorders in children and adolescents (i.e., group treatment and two variants of individual treatment). An additional aim was to compare the relative long-term efficacy of the treatment approaches. METHOD: At long-term follow-up, participants (N = 67) were between 16 and 26 years of age (M = 19.43 years, SD = 3.02). Primary outcome was the targeted

Role of baseline emotion dysregulation and perceived daily stress in adherence to and completion of narrative exposure therapy among forced migrants.

BACKGROUND: Forced migrants are at risk of developing post-traumatic stress disorder (PTSD). Narrative exposure therapy (NET) is an empirically supported treatment for forced migrants with PTSD. However, multiple barriers can make it difficult for them to consistently engage in and complete mental health treatment. Identifying factors contributing to the feasibility of NET could enhance its treatment outcomes. OBJECTIVES: The first aim was to examine whether baseline levels of perceived daily stress and emotion dysregulation were associated with NET completion. The second aim was to test whet

Direct comparison of reconsolidation of traumatic memories and prolonged exposure therapy: A randomized controlled trial.

New therapies are needed for posttraumatic stress disorder (PTSD), particularly in military service members (SMs). Reconsolidation of traumatic memories (RTM) is a novel treatment with promising results in small clinical trials. We randomized 94 active duty or retired SMs (Mage = 45.80, 31.0% women) with PTSD to receive up to ten 90-min sessions of RTM (n = 48) or prolonged exposure (PE; n = 46) to ascertain whether RTM achieved a greater and/or faster response. The Clinician-Administered PTSD Scale for DSM-5 (CAPS-5) was used to establish eligibility and assess response at 2 weeks and 2, 6, a

Examining written exposure therapy for the treatment of posttraumatic stress disorder in Azerbaijan: A pilot study.

Written exposure therapy (WET) is a brief, evidence-based treatment for posttraumatic stress disorder (PTSD) that has demonstrated effectiveness in a variety of settings, mostly within the United States. The pilot study described here examined the feasibility, acceptability, and effectiveness of WET in Azerbaijan, where access to evidence-based trauma-focused treatments is limited. Patients diagnosed with PTSD (N = 62) received five weekly sessions of WET delivered by trained clinicians. Self-report measures were used to assess PTSD and depressive symptoms, as well as maladaptive beliefs, at p

An Educational Program to Enhance the Identification of Obsessive-Compulsive Disorder and the Utilization of Exposure and Response Prevention Among Outpatient Mental Health Providers.

BACKGROUND: Obsessive-compulsive disorder (OCD) is a prevalent and impairing psychiatric disorder that is frequently misdiagnosed. Further, despite a breadth of evidence in support of Exposure and Response Prevention (ERP) in the treatment of OCD, it is often underutilized or used incorrectly. METHODS: This quality improvement project was conducted utilizing the Plan Do Study Act Cycle at an outpatient group mental health facility with a sample of 19 mental health providers. A pretest post-test design was utilized to assess whether a 1-h educational program would result in an improvement in k

Impact of Switching to an Inhibitory Learning Approach to Exposure Therapy on Outcomes of an Intensive Outpatient Program.

OBJECTIVE: Exposure therapy is a commonly used, efficacious treatment for reducing symptoms of obsessive-compulsive disorder and anxiety disorders. Though exposure therapy has primarily been conducted using principles derived from emotional processing theory, many researchers are now suggesting that it would be more successful if it was conducted in accordance with learning research (i.e., the inhibitory learning approach). There is a strong conceptual rationale for conducting exposure therapy in accordance with the inhibitory learning approach but a paucity of direct empirical comparisons of

Association between therapists' negative beliefs about exposure therapy and its use in an Australian sample: a brief report.

Despite a wealth of research demonstrating the efficacy of exposure therapy for the anxiety and related disorders, research shows that few therapists use this technique in clinical practice. Negative beliefs about exposure therapy have been shown to be the most significant barrier to use or optimal use. A richer and more nuanced understanding of the relationship between negative beliefs and exposure use, and its implications for training and practice, is warranted. This research aimed to identify the specific negative beliefs about exposure held by Australian psychologists, and the association

Optimizing in vivo exposure using occasional reinforced extinction with aversive imagery in spider fearful individuals.

Although exposure-based therapy is widely recognized as effective for treating various anxiety disorders, a significant proportion of patients fail to benefit or experience a return of fear following successful treatment. One promising strategy involves occasional presentation of fear-evoking stimuli during extinction (occasional reinforced extinction, ORE). This study investigates a novel approach to translate ORE into clinical practice by incorporating occasional vivid imagery of individuals' worst-case fear scenarios during in-vivo exposure. Forty-seven spider-fearful individuals were rando

To SHARE or not to SHARE: Exploring incarcerated women's decisions about enrolling in exposure-based group therapy.

Exposure therapies effectively treat traumatic stress sequelae, including that which follows sexual violence victimization (SVV). Carceral facilities house women with significantly higher rates of SVV than community samples, yet they rarely implement this form of treatment. In this study, women with histories of SVV (n = 63) completed semistructured qualitative interviews about their decision to enroll or not enroll in an exposure-based group therapy called Survivors Healing from Abuse: Recovery through Exposure while incarcerated. All study participants were previously incarcerated in a priso

[Exposure Therapy with the Help of a Wearable Defibrillator Device used in the Treatment of Severe Obsessive-Compulsive Disorder and Congenital LQTS - An Example for Interdisciplinary Treatment].

We report on a case of a young female suffering from both obsessive-compulsive disorder (OCD) and a severe underlying cardiac disease. Due to the somatic comorbidity, treatment according to guidelines with exposure and reaction prevention was not initially conducted, due to potentially fatal risks to the patient. However, through collaboration with a cardiology clinic, we were able to find an innovative solution which allowed for the continuation of the exposure therapy. This case report demonstrates a successful interdisciplinary collaboration and is intended to sensitize the reader to the ne

Lagged effects of substance use on PTSD severity in a randomized controlled trial with modified prolonged exposure and relapse prevention.

OBJECTIVE: To advance understanding of the effectiveness of evidence-based treatments for comorbid posttraumatic stress disorder (PTSD) and substance use disorder (SUD), research must provide a more nuanced picture of how substance use affects change in PTSD symptoms over the course of treatments and whether prolonged exposure techniques can be efficacious during active substance use. A data set that included patients with PTSD/subthreshold-PTSD and SUD treated with an exposure-based intervention provided an opportunity to conduct a secondary analysis to test how patients' substance use impact

Internet-Delivered Exposure Therapy for Fibromyalgia: A Randomized Controlled Trial.

BACKGROUND: Fibromyalgia (FM) is a common and disabling chronic pain disorder, for which existing pharmacological and psychological treatments have yet yielded insufficient effects. Previous literature has shown that exposure therapy may be an effective treatment for chronic pain. This study constitutes the first randomized controlled trial evaluating exposure therapy for FM. METHODS: A total of 140 participants with diagnosed FM were randomized to a 10-week Internet-delivered exposure treatment (iExp; n=70) or a waitlist control condition (WLC; n=70). Primary outcome measure were FM symptoms

The association between estradiol levels, hormonal contraceptive use, and responsiveness to one-session-treatment for spider phobia in women.

Preclinical studies have demonstrated that conditioned fear extinction is impaired in females with low endogenous levels of the sex hormone estradiol, due to menstrual fluctuations or hormonal contraceptive use. As fear extinction is a laboratory model of exposure therapy for anxiety and trauma disorders, here we assessed the hypothesis that treatment outcomes may be diminished when exposure therapy occurs during periods of low estradiol. 90 women with spider phobia (60 cycling and 30 using hormonal contraceptives) underwent a one-session exposure treatment for spider phobia, following which,

Plasticity of Functional MAOA Gene Methylation in Acrophobia.

Epigenetic mechanisms have been proposed to mediate fear extinction in animal models. Here, MAOA methylation was analyzed via direct sequencing of sodium bisulfite-treated DNA extracted from blood cells before and after a 2-week exposure therapy in a sample of n = 28 female patients with acrophobia as well as in n = 28 matched healthy female controls. Clinical response was measured using the Acrophobia Questionnaire and the Attitude Towards Heights Questionnaire. The functional relevance of altered MAOA methylation was investigated by luciferase-based reporter gene assays. MAOA methylation was

The Moderating Effect of State Anger on Treatment Outcome in Female Adolescents With PTSD.

Trauma experienced in childhood and adolescence negatively affects the development of adaptive regulation of emotions and is associated with greater symptoms of anger. Prior research has suggested that high levels of anger may impede the outcome of treatment in adults with posttraumatic stress disorder (PTSD). The current study investigated whether high levels of anger resulted in poorer treatment outcomes in adolescent girls with PTSD. Participants included 61 female adolescent survivors of sexual abuse or assault who were randomized to either prolonged exposure for adolescents (PE-A) or clie

Predictors of the application of exposure in vivo in the treatment of agoraphobia in an outpatient clinic: An exploratory approach.

OBJECTIVE: Although exposure in vivo is considered to be the most effective therapy component in the treatment of agoraphobia (AG), there is a remarkable lack of its application in psychotherapeutic routine care. We examined the severity of anxiety, psychological distress/comorbidity, therapeutic process/alliance, and sociodemographic status as potential predictors of in vivo exposure. METHOD: We applied correlational analyses and logistic regression analyses in a sample of N = 92 patients (main diagnosis AG) in an outpatient setting. RESULTS: Logistic regression analyses did not yield any s
