Changes in publicly insured general practitioner consultations before and after workplace injury: interrupted time series by consultation duration and care plans in Australia.

BACKGROUND: General practitioners (GPs) are central to the treatment and management of injured workers. Australian workers' compensation funds GP consultations for work-related injuries. Almost all other GP consultations are subsidised by public healthcare insurance (Medicare Benefit Scheme). This study aimed to examine changes in publicly-insured GP consultations before and after injury for injured workers with long-duration claims, focusing on consultation duration types and GP-provided mental health and chronic diseases management plans. METHODS: This retrospective cohort study using inter

Assessment of Perceived Barriers to Herpes Zoster Vaccination among Geriatric Primary Care Providers.

The herpes zoster vaccine is recommended for use in adults 60 years of age and older to reduce the incidence and morbidity associated with infection. Its limited uptake has been attributed to logistical barriers, but uncertain efficacy and safety in subsets of this patient population could also be contributing. The purpose of this study was to evaluate the current vaccination practices, barriers to vaccination, knowledge of vaccination reimbursement and strategies to evaluate for insurance coverage among an urban, safety net, teaching hospital, geriatric primary care provider group through a s

Management of Patients with Asthma, COPD, and OSA in Outpatient Unit: ACOSOU-Global Perspectives and Challenges.

Obstructive sleep apnea (OSA) is increasingly recognized as a major comorbidity in chronic respiratory diseases, particularly asthma and chronic obstructive pulmonary disease (COPD). The coexistence of OSA with asthma or COPD significantly complicates the clinical course, leading to poorer disease control, more frequent exacerbations, reduced lung function, impaired sleep quality, and increased cardiovascular and overall mortality. In asthma, OSA exacerbates airway inflammation, enhances bronchial hyperresponsiveness, and decreases responsiveness to standard therapies. In COPD, the "overlap sy

High dietary acid load increases the risk of disability in women aged 75 years and older: A community-based cohort study.

BACKGROUND: Metabolic acidosis caused by acidogenic diets increases muscle catabolism. High acidogenic diets can increase muscle loss in older adults; however, their association with functional outcomes remains unclear. OBJECTIVES: To investigate whether high acidogenic diets increase the incidence of disability. DESIGN: Longitudinal study. SETTING: Community-based. PARTICIPANTS: We included 1,704 community-dwelling Japanese individuals aged ≥ 75 years without disabilities at baseline (52.2 % females). MEASUREMENTS: Baseline dietary acid load was assessed using potential renal acid load (

Swedish national cohort of children living with long-term respiratory support (DISCOVERY-P): cohort profile.

PURPOSE: Children living with respiratory support rely on medical technology, either fully or partially, throughout the day to meet their breathing requirements. Although children and young people living with respiratory support at home undergo long-term treatments and make extensive use of health and social care services, there is a notable absence of comprehensive outcome data on this group. The establishment of the first nationwide Course of DISease reported to the Swedish CPAP Oxygen and VEntilator RegistrY paediatrics cohort aims to investigate the disease trajectory, clinical and socioec

AYA Considerations for Aggressive Lymphomas.

PURPOSE OF REVIEW: Lymphoma is the one of the most common cancer diagnoses among adolescents and young adults (AYAs) aged 15-39. Despite significant advances in outcomes observed in older adults and younger children, improvements in AYAs have lagged behind. The reasons for this are likely multifactorial including disparities in access to health insurance, low rates of enrollment to clinical trials, potential differences in disease biology, and unique psychosocial challenges. Here we will review Hodgkin lymphoma (HL) and primary mediastinal B cell lymphoma (PMBCL), two of the most common aggres

[Guideline 'Diagnosis and treatment of inflammatory bowel disease in adults'. II. Special situations and organisation of medical care].

The Dutch national practice guideline 'Diagnosis and treatment of inflammatory bowel diseases (IBD) in adults' describes the multidisciplinary approach for adult patients with (suspected) IBD, recommended following analysis of the literature according to the principles of evidence based guideline development. Ulcerative colitis and Crohn's disease are associated with temporary or permanent extra-intestinal disorders: reactive inflammatory conditions, associated conditions, and those arising as a consequence of long standing, inflammatory, intestinal disease. Treatment is aimed first at the IBD

All-cause healthcare resource utilization and costs among community-managed adults with long-COVID in France, 2020-2023.

BACKGROUND: The clinical and economic burden of long COVID is poorly understood. We aim to assess all-cause healthcare resource utilization (HCRU) and costs in the primary care setting among adults with long COVID in France. METHODS: A retrospective cohort study using the electronic healthcare records (EHRs) of confirmed and/or probable COVID-19 patients from The Health Improvement Network (THIN) data between March 2020 and December 2022 was conducted. Long COVID was identified per World Health Organization (WHO) definition as suggestive symptoms present ≥3 months following acute SARS-CoV-2 i

A Chest Tube Inadvertently Inserted into Tracheal Lumen.

Social determinants of health (SDOH)-conditions where individuals live, learn, work, and socialize-significantly influence health disparities and patient care outcomes. SDOH can be categorized into five key domains: economic stability, access to quality education, access to quality healthcare, neighborhood environments, and social support. Structural barriers such as economic inequality, limited transportation, and discrimination directly impact patients' willingness and ability to access care. Transportation barriers, notably among low-income and elderly populations, often result in delayed o

Anesthetic management of traumatic brain injury.

The management of TBI remains an important and frustrating component of the practice of anesthesiology and critical care medicine. The difficulties in management of TBI as well as the poor response rates to medical therapy after TBI are not new. The following passage appeared in the introductory chapter of a text on TBI from 1897: "The manner of treatment is of importance in only a minority of cases, since many subjects of intracranial injury are fated to die whatever measures may be adopted for their relief, and a still greater number are destined to recover though left entirely to the resour

Long-Term Wound Healing Dynamics and Tissue Remodeling After Minimally Invasive Voice Preservation Techniques With Implications for Functional Outcomes Racial Disparities and Access in Professional Singers: A Literature Review.

Minimally invasive voice-preservation techniques, including microlaryngoscopy, phonosurgery, and office-based regenerative interventions, have revolutionized the management of benign and early-stage laryngeal disorders by minimizing fibrosis and preserving phonatory function. However, the long-term (>6 months) wound healing and extracellular matrix (ECM) remodeling dynamics that determine sustained vocal outcomes remain poorly understood, particularly among professional singers who experience high vibratory demand. Moreover, African American and minority singers continue to face significant ba

Charting A Path Forward For Medicare Advantage: Expert Consensus On Policy Priorities And Trade-Offs.

More than half of Medicare beneficiaries are enrolled in Medicare Advantage (MA), yet concerns persist about overpayments to MA plans, inadequate plan oversight, and limits on beneficiaries' access to care. To identify MA reform options, we conducted a two-round e-Delphi panel with thirty-nine experts, including researchers, industry representatives, patient advocates, and former policy makers. Panelists rated fifteen policy reform ideas on their impact on beneficiaries, the federal government, and MA plans. Experts agreed that six policy ideas would benefit beneficiaries, three would benefit

The Impact of Nurse Home-Visiting for Pregnant and Parenting Individuals with Previous Live Births.

The Nurse-Family Partnership (NFP) benefits first-time parents and their children; however, its effectiveness for families with previous children (multiparous) is not known. This quasi-experimental study used electronic health record data from three NFP sites affiliated with large health systems to evaluate the impact of NFP on pregnancy, birth, and health care utilization outcomes among multiparous families compared with propensity score-matched Medicaid-insured families who did not receive NFP. The study population included 639 multiparous pregnant individuals enrolled in NFP between 2017 an

Efficacy of Diagnostic In-Office Shoulder Ultrasound in the Surgical Treatment of Full-Thickness Rotator Cuff Tears.

BACKGROUND: Early surgical management of full-thickness traumatic rotator cuff tears (RCTs) may optimize functional outcomes, prioritizing timely diagnoses. Ultrasound and magnetic resonance imaging (MRI) are highly sensitive and specific modalities for RCT diagnosis, yet MRI remains the gold standard diagnostic tool despite increased costs and potential delays in care. Ultrasound can provide same-day diagnosis, thus possibly expediting care. HYPOTHESIS: The use of diagnostic shoulder ultrasound alone in the orthopaedic surgeon's office could increase the efficiency of diagnosis and care of t

Application of a new type of double-lumen endotracheal tube in preterm infants with respiratory distress syndrome: study protocol for a non-inferiority randomised controlled trial (NISA).

BACKGROUND: Non-invasive ventilation combined with pulmonary surfactant (PS) therapy is recognised as a method for treating neonatal respiratory distress syndrome (NRDS). Among the administration, methods of PS, INtubation-SURfactant-Extubation (InSurE) and less invasive surfactant administration (LISA) have been widely discussed.LISA technique prevents patients from exposure to invasive positive pressure ventilation (PPV), thus improving the long-term outcomes of the respiratory system, but it faces challenges in resource-limited areas due to complexity and cost. The InSurE technique remains

Disparities in Headache Care and Research in Children and Adolescents: A Narrative Review.

PURPOSE OF REVIEW: Headache disorders are highly prevalent in children and adolescents and represent a significant source of disability during developmental years. Despite this, disparities in pediatric headache care and research remain insufficiently characterized. This narrative review synthesizes current evidence on inequities across epidemiology, diagnosis, treatment access, and research participation. RECENT FINDINGS: Migraine and tension-type headache contribute substantially to years lived with disability in the pediatric population, with rising absolute burden over time. Important gaps
