Regional trends in perceptions of American Shoulder and Elbow Surgeons towards barriers to access for patients with Medicaid: limited perioperative service access, low patient engagement, and decreased reimbursement are regionally consistent obstacles.

BACKGROUND: Medicaid is a means-tested health insurance program for low-income adults (∼30%), children (∼40%), individuals with disabilities (15%) and some elderly patients that are dual Medicare and Medicaid eligible (15%). It is jointly funded by the federal and state governments but administered by individual states. Allowing states to oversee the administration of Medicaid has led to regional variability in eligibility criteria, types of services covered, and reimbursement. The purpose of this study was to evaluate current perceptions of shoulder and elbow surgeons surrounding practice pat

Demographic and Socioeconomic Determinants Are Associated with Poor Preoperative Patient-Reported Pain and Function in Primary TKA: A Cohort Study of 14,079 Patients.

BACKGROUND: Pain and function, as reflected by patient-reported outcome measures (PROMs), can influence improvement after total knee arthroplasty (TKA) and can reflect the extent of patient access to orthopaedic surgical care. We aimed to (1) categorize patients according to pain and function PROM phenotypes, (2) identify patient characteristics associated with poor preoperative pain and function, and (3) assess relationships between baseline characteristics and PROM phenotypes. METHODS: A prospective cohort of 14,079 TKAs was enrolled. Demographics, comorbidities, surgical details, and preop

An analysis from the Quality Outcomes Database, Part 1. Disability, quality of life, and pain outcomes following lumbar spine surgery: predicting likely individual patient outcomes for shared decision-making.

OBJECTIVE Quality and outcomes registry platforms lie at the center of many emerging evidence-driven reform models. Specifically, clinical registry data are progressively informing health care decision-making. In this analysis, the authors used data from a national prospective outcomes registry (the Quality Outcomes Database) to develop a predictive model for 12-month postoperative pain, disability, and quality of life (QOL) in patients undergoing elective lumbar spine surgery. METHODS Included in this analysis were 7618 patients who had completed 12 months of follow-up. The authors prospectiv

Epidemiological and demographic patterns of pediatric trigeminal neuralgia: nationwide trends in diagnosis and treatment over a decade.

OBJECTIVE: Trigeminal neuralgia (TN) is a rare, debilitating craniofacial pain disorder that is uncommon in children, representing 1%-1.5% of cases. While adult TN is well-documented, data on pediatric presentations, treatment patterns, and healthcare disparities remain limited. This study characterizes demographic trends, procedural interventions, and inpatient cost trends for pediatric patients with TN using a national dataset. METHODS: The authors conducted a retrospective cohort study using the National Inpatient Sample from 2011 to 2020 to identify patients with TN younger than 18 years.

After ACL Injury in Children and Adolescents, Where in the Preoperative Timeline do Disparities in Timing and Clinical Course Originate?

BACKGROUND: Disparities in the timing of pediatric ACL reconstruction (ACLR) have been described along the lines of insurance and race. However, it is unclear where exactly in the preoperative timeline these inequities originate. Effective interventions to mitigate disparities cannot be designed without understanding this. This study aims to identify differences in the course and timing at each stage of the pre-ACLR process with respect to insurance and race or ethnicity. Such data can be used in the development of strategies to reduce delays for pediatric patients with ACL injuries. QUESTION

Incidence and factors associated with reoperation after rotator cuff repair in Korea: A nationwide cohort study.

INTRODUCTION: With the growing interest in rotator cuff repair (RCR), substantial evidence on factors associated with reoperation is crucial for decision-making. This retrospective longitudinal cohort study aimed to investigate the rate of rotator cuff reoperation after repair in Korea and identify factors associated with reoperation while considering the medical services implemented in Korea using the Korean National Health Insurance Database from 2011 to 2021. METHODS: Reoperation rates in patients who underwent RCR during a 3.5-year follow-up period were analyzed using Kaplan-Meier analysi

Epidemiology, treatment and outcomes of gastroenteropancreatic neuroendocrine neoplasms.

To investigate incidence, treatment patterns and outcomes of gastroenteropancreatic neuroendocrine neoplasms (GEP-NEN) in the United States. The 2019 National Cancer Database was searched for adult GEP-NEN patients. Main outcomes included overall and site-specific incidence, treatment patterns, and overall survival (OS). Overall survival was evaluated using averaged Cox regression. 86,324 GEP-NEN patients were included (6.33% of all GEP malignancies). From 2004 to 2016, annual GEP-NEN cases increased (n = 4,010 to n = 9,379), largely driven by low-stage, low-grade disease. Most patients receiv

The costs and outcomes of treating a deep pressure ulcer in a patient with quadriplegia .

The cost of pressure ulcers, especially Stage III and Stage IV ulcers, is substantial. A 27-year-old man with a 6-year history of quadriplegia developed an ischial pressure ulcer. Twelve months of treatment with wet-to-dry dressings were followed by admission to several facilities and 15 months of care with biological dressings and negative pressure wound therapy (NPWT). When admitted to the authors' wound care center, the wound measured 4.5 cm x 3.2 cm with exposed bone. A review of his insurance records showed that paid claims totaled $242,350, including $52,992 for NPWT ren

Avoiding frenotomies and improving health equity: Findings from a paradigm shift newborn feeding difficulties model.

IMPORTANCE: Newborn feeding difficulties are increasingly attributed to ankyloglossia, leading to a surge in frenotomy procedures despite limited high-level evidence supporting its effectiveness. Disparities in access to appropriate feeding assessments contribute to potential surgical overtreatment and inequities in care. OBJECTIVE: To evaluate the impact of a paradigm shift newborn feeding difficulties model on clinical care patterns and health equity. DESIGN, SETTING, AND PARTICIPANTS: A retrospective cohort study of infants (<6 months) with ankyloglossia-related ICD-9/ICD-10 codes at a te

The necessity and risk factors of subsequent fusion after decompression alone for lumbar spinal stenosis with lumbar spondylolisthesis: 5 years follow-up in two different large populations.

BACKGROUND/CONTEXT: Although decompression without fusion is a reasonable surgical treatment option for some patients with lumbar spinal stenosis (LSS) secondary to spondylolisthesis, some of these patients will require secondary surgery for subsequent fusion. Long-term outcome and need for subsequent fusion in patients treated with decompression alone in the setting of lumbar spondylolisthesis remains controversial. PURPOSE: The aim of this study was to examine the rate, timing, and risk factors of subsequent fusion for patients after decompression alone for LSS with spondylolisthesis. STUD

The Impact of Social Determinants of Health on Severe Colorectal Complications.

Complications following colorectal surgery remain a persistent issue despite advances in surgical technique, development of enhanced recovery, and improvements in perioperative care. Social determinants of health (SDOHs) at the individual, interpersonal, institutional, community, and policy levels have been associated with the risk of patients experiencing a complication. At the individual level, race and ethnicity are associated with complications, with the Black race having the most robust data demonstrating increased risk for complication. Additional individual-level factors associated with

Preoperative biliary drainage associated with biliary stricture after pancreaticoduodenectomy: a population-based study.

BACKGROUND: The rate of preoperative biliary drainage for pancreaticoduodenectomy has been increasing despite most recent evidence that favors avoiding it. Only a few studies have focused on late surgical complications - biliary stricture after pancreaticoduodenectomy and have produced only inconclusive results. We evaluate the role of preoperative biliary drainage in the formation of biliary stricture after pancreaticoduodenectomy. METHODS: The Taiwan National Health Insurance Program is a mandatory health care plan that covers nearly the entire population of 23 million in this country. A re

Superficial Temporal Artery to Anterior Cerebral Artery Hemi-bonnet Bypass Using Radial Artery Graft for Prevention of Complications after Surgical Treatment of Partially Thrombosed Large/Giant Anterior Cerebral Artery Aneurysm.

BACKGROUND: Partially thrombosed large/giant aneurysm of the anterior cerebral artery is still challenging because this complex aneurysm requires arterial revascularization in the deep operation field. Therefore, direct neck clipping is often impossible. We describe our experiences with extracranial-intracranial bypass as an insurance bypass prior to clipping of partially thrombosed anterior cerebral artery aneurysms, and discuss the microsurgical technique and strategy. CLINICAL PRESENTATION: Consecutive, single-surgeon experience with the surgical treatment of partially thrombosed anterior
