Open peritoneal drainage as effective treatment of advanced peritonitis.

In 13 patients with advanced peritonitis (postoperative suture line breakdown in eight and spontaneous intestinal perforation in five) treatment consisted of widely opening the peritoneal cavity and exposing the contaminated viscera. Eight of the patients underwent diverting enterostomy, and the remaining five underwent an additional drainage procedure to evacuate a residual collection. Clinical improvements were observed in all except one who died without favorable response. Some of the clinical manifestations indicative of organ failures subsided in nine of the ten patients. In five patients

Aortoenteric Fistula through a Thrombosed Graft.

BACKGROUND Aortoenteric fistula is a dreadful and uncommon complication after abdominal aortic aneurysm repair. Continuous friction against the intestine and the aortic graft along with local inflammation is thought to be the major cause of aortoenteric fistula formation, although it is unexpected to have fistula formation with a thrombosed aortic graft. CASE REPORT Here, we report a case of an aortoenteric fistula between a thrombosed aortoiliac bypass graft and the duodenum in a 75-year-old male patient who presented with a 2-month history of melena. In this case, the aortoduodenal fistula w

[One-stage reconstructive jejunogastroplasty after previous multiple abdominal surgeries for left-sided diaphragm rupture complicated by gastric incarceration and necrosis].

Acute gastric necrosis is a rare event requiring organ resection. Delayed reconstruction is advisable in patients with peritonitis and sepsis. The most common complication of gastrectomy with reconstruction is failure of esophagojejunostomy and duodenal stump. In case of severe esophagojejunostomy failure, appropriate surgical approach and timing of reconstructive stage should be analyzed. We report one-stage reconstructive surgery in a patient with multiple fistulas after previous gastrectomy. Surgery included reconstructive jejunogastroplasty with jejunal graft interposition. The patient und

Management of urinary complications in Crohn's disease.

Among the 127 patients who underwent surgical treatment for Crohn's disease at Tohoku University Hospital, urinary complications were noted in 13 patients (10.2%), including urolithiasis in 6 patients (4.7%), a ureteral obstruction in 4 (3.1%), and urinary fistula in 3 (2.4%). In patients with urolithiasis, conservative therapy was effective. An ureteral obstruction was detected on the right side in all 4 of these cases because of the inflamed terminal ileum. In 2 of the 4 cases, the symptoms improved by either preoperative total parenteral nutrition or elemental diet therapy. A resection of t

Secondary aortoduodenal fistula following endovascular repair of inflammatory abdominal aortic aneurysm due to Streptococcus anginosus infection: A case report and literature review.

INTRODUCTION: Aortoenteric fistula is a rare but very serious complication of both surgical and endovascular abdominal aortic reconstruction. Since the advent of endovascular abdominal aortic aneurysm repair (EVAR), 20 cases of aortoduodenal fistula associated with aortic stent grafts have been reported.(1) However, only a handful has been reported following inflammatory abdominal aortic aneurysm repair. It most commonly presents with bleeding, usually from the upper gastro-intestinal tract. With recent advances in the screening, diagnosis and management of abdominal aortic aneurysms either su

Ileovesical fistula secondary to chemotherapy for follicular non-Hodgkin lymphoma: a case report and review of the literature.

Enterovesical fistulas are rare complications of malignancies, diverticulitis, inflammatory bowel diseases, radiotherapy, and traumas involving the colorectal and pelvic region. In this study, an ileovesical fistula that occurred during chemotherapy for non-Hodgkin lymphoma is presented. The patient had acute abdomen and multiple comorbidities, and ileovesical fistula was diagnosed during the operation. The affected intestinal segment was resected, and an end-to-end anastomosis was performed with a primary bladder repair. This is a reliable treatment method for such cases.

Enterovesical fistula presenting as life-threatening normal anion gap metabolic acidosis.

Enterovesical fistula is a rare complication of a variety of inflammatory and neoplastic diseases. It usually presents with pneumaturia, fecaluria, urinary tract infections, or irritable bladder symptoms in the setting of either diverticulitis or malignancy. For the first time, we describe a patient with an enterovesical fistula who presented with a life-threatening normal anion gap metabolic acidosis. The direction of flow through the fistula, ie, bladder to intestine, was contingent on a spastic bladder and was responsible for the atypical presentation.

Enterovesical fistula due to non-Hodgkin lymphoma in AIDS.

Enterovesical fistula associated with lymphoma is exceedingly rare. We report three patients with the acquired immunodeficiency syndrome who presented with fecaluria and pneumaturia. Non-Hodgkin lymphomas involving the intestine and the urinary bladder creating an enterovesical fistula were found at surgery in two patients and at autopsy in the third. Extranodal lymphomas are becoming more common in AIDS patients, so that the possibility of lymphoma should be considered in the differential diagnosis of enterovesical fistulas in these patients.

Enterovaginal fistulas associated with Crohn's disease.

Our experience in the management of 16 enterovaginal fistulas complicating Crohn's disease leads us to believe that such fistulas usually arise from active rectal disease and, despite being evident clinically, are difficult to confirm endoscopically or radiologically. Patients with few symptoms can be managed conservatively. Surgical repair protected by diversion may have a place in management, but most with severe symptoms will require excision of the intestine from which the fistula is arising.

Penile and scrotal lymphedema as an unusual presentation of Crohn's disease: case report and review of the literature.

Crohn's disease is an inflammatory intestinal disease that primarily causes abdominal pain and diarrhea. We report a male patient who presented with penile and scrotal lymphedema and inguinal fistulas as the first manifestations of Crohn's disease. Extraintestinal or metastatic Crohn's disease initially presenting as genital lymphedema with fistula formation is rare. Skin lesions in extraintestinal Crohn's disease typically show non-caseating, sarcoidal granulomas with numerous foreign body- and Langhans-type multinucleated giant cells, which are separated from intestinal involvement by normal

Strategy to small intestine obstruction caused by Crohn's disease on the basis of transnasal ileus tube insertion.

BACKGROUND: Previous studies reported that transnasal ileus tube was a new and useful method for rapid relief of small intestinal obstruction. However, no study reported the impacts of the transnasal ileus tube for Crohn's disease combined with intestinal obstruction. We aimed to describe the strategy to the small intestine obstruction caused by Crohn's disease on the basis of transnasal ileus tube insertion. METHODS: From November 2019 to November 2021, the data of 6 hospitalized patients with CD, diagnosed and conservatively treated in The Second Hospital of Nanjing, were not relived and re
