Knee dislocation and associated injuries: an analysis of the American College of Surgeons National Trauma Data Bank.

PURPOSE: Knee dislocations (KDs) are potentially devastating injuries, leading to loss of function or limb in often young patients. This retrospective database review aims to determine the relative incidence and risk factors for KDs presenting to North American Level I and II trauma centers. METHODS: The National Trauma Data Bank (NTDB) was retrospectively interrogated using ICD-9-CM codes to identify KDs between 2010 and 2014 to derive KD incidence. KDs were stratified by age, sex, Injury Severity Score (ISS), Glasgow Coma Scale (GCS), drug and alcohol use, injury mechanism, open vs. closed

Is the all-arthroscopic tibial inlay double-bundle PCL reconstruction a viable option in multiligament knee injuries?

BACKGROUND: All-arthroscopic tibial inlay double-bundle (DB) posterior cruciate ligament (PCL) reconstruction avoids an open dissection and the "killer turn" while maintaining the advantage of an anatomic graft. However, clinical data on the viability of this surgical technique in multiligamentous knee injuries are lacking. QUESTIONS/PURPOSES: At greater than 2 years of followup, we evaluated (1) validated outcomes scores; (2) range of motion; and (3) side-to-side stability on PCL stress radiographs of a small group of patients who underwent all-arthroscopic tibial inlay DB PCL reconstruction…

Treatment options in overuse injuries of the knee: patellofemoral syndrome, iliotibial band syndrome, and degenerative meniscal tears.

Overuse injuries of the knee, particularly those of the extensor mechanism and iliotibial band, are commonplace in primary care and musculoskeletal practices. Effective treatment requires identification of all pertinent biomechanical factors contributing to overload and implementing measures to correct them.

Popliteal arterial injuries associated with fractures or dislocations about the knee as a result of blunt trauma.

A total of 73 patients with popliteal arterial injuries due to blunt trauma were treated at the Los Angeles County/University of Southern California Medical Center between January 1975 and January 1986. The injuries resulted from high-energy trauma (70% were motor-vehicle-related), and were associated with a variety of fractures and dislocations about the knee. Forty-eight of the patients (66%) had open injuries, 27% of which became infected; the infection rate for all injuries was 22%. The average time from injury to anastomosis was 14.6 hours. The overall amputation rate in the series was 15

Rethinking knee injury prevention strategies: joint-by-joint training approach paradigm versus traditional focused knee strengthening.

Knee injuries remain a significant challenge in competitive sports, impacting athletic performance, long-term joint health, and healthcare resource utilization. Traditional prevention strategies often focused primarily on strengthening the knee itself. However, emerging evidence supports a joint-by-joint training approach (JBJTA), emphasizing the interconnected kinetic chain, particularly the trunk, hips, ankles, and feet. This commentary explores the potential of JBJTA in knee injury prevention by enhancing hip and ankle mobility, trunk and foot stability, and proprioception. The JBJTA posits…

Is knee laxity change after ACL injury and surgery related to open kinetic chain knee extensor training load?

OBJECTIVE: The purpose of this study was to evaluate whether knee anterior laxity changes after anterior cruciate ligament injury and surgery are related to aspects of thigh muscle resistance training during rehabilitation. DESIGN: Forty-nine subjects (13 females) diagnosed with an anterior cruciate ligament-deficient knee or who had undergone anterior cruciate ligament reconstructive surgery participated in this study. The subjects trained their knee extensors in the open kinetic chain during a 6-wk program, and the relationship of aspects of training (for example, absolute resistance load)

Distal posterolateral corner injury in the setting of multiligament knee injury increases risk of common peroneal palsy.

PURPOSE: The purpose of this study was to identify if the location of posterolateral corner (PLC) injury was predictive of clinical common peroneal nerve (CPN) palsy. METHODS: A retrospective chart review was conducted of patients presenting to our institution with operative PLC injuries. Assessment of concomitant injuries and presence of neurologic injury was completed via chart review and magnetic resonance imaging (MRI) review. A fellowship-trained musculoskeletal radiologist reviewed the PLC injury and categorized it into distal, middle and proximal injuries with or without a biceps femor

Transcriptomic comparison of the infrapatellar fat pad between young patients with traumatic knee injury and older patients with knee osteoarthritis.

PURPOSE: The infrapatellar fat pad (IFP) is increasingly recognized as an active contributor to knee osteoarthritis (KOA) pathogenesis. However, age-related transcriptional changes in the IFP remain poorly understood, and whether observed differences are purely age-dependent or confounded by disease status is unclear. This study aimed to compare transcriptomic profiles of the IFP between young patients with traumatic knee injury and older patients with KOA. METHODS: IFP samples were obtained from three young trauma patients (21-28 years, KL grade 0) and three older patients with KOA (64-72 ye

Bilateral Open Knee Dislocation Following an Agricultural Injury: A Case Report and Review of the Literature. Bilateral Open Knee Dislocation.

Bilateral open knee dislocation is an extremely rare injury, with only one previously documented case in the literature. A 65-year-old male sustained bilateral open knee dislocations in an agricultural accident. The left knee was treated with early multiligamentous reconstruction, achieving full recovery. The right knee, complicated by infection and extensive bone loss, required multiple debridements, soft tissue coverage with a medial gastrocnemius flap, and primary arthrodesis. At 18 months, the patient walked independently, with full function in the left knee and a stable, painless right kn

Bone contusion progression from traumatic knee injury: association of rate of contusion resolution with injury severity.

BACKGROUND: Bone contusions are frequently encountered in magnetic resonance imaging (MRI) evaluation of knee anterior cruciate ligament (ACL) injuries. Their role as indicators of injury severity remains unclear, primarily due to indeterminate levels of joint injury forces and to a lack of preinjury imaging. PURPOSE: The purpose of this study was to 1) quantify bone contusion pathogenesis following traumatic joint injuries using fixed imaging follow-ups, and 2) assess the feasibility of using longitudinal bone contusion volumes as an indicator of knee injury severity. STUDY DESIGN: Prospect
