Progressive cochleo-vestibular labyrinthitis.

OBJECTIVE: We report a rare case of sudden deafness and benign paroxysmal positional vertigo (BPPV) corresponding to a canalithiasis of the ipsilateral left posterior semicircular canal (PSC). CASE REPORT: The initial examination showed intact lateral semicircular canal (LSC) function. Cerebral MRI results were normal. However evoked otoacoustics emissions were absent in the left ear. The patient developed, in the course of one week, symptoms and signs of an acute unilateral vestibulopathy in the same ear. However there were no change in hearing loss after it. CONCLUSION: The association of

Experimental cytomegalovirus infection: viremic spread to the inner ear.

The entry route of viruses into the inner ear is still controversial. Guinea pig cytomegalovirus (CMV) has been used to create an animal model of systemic infection as well as labyrinthitis. In this study, seronegative guinea pigs were administered intracardiac inoculations of varying doses of guinea pig CMV. After three or eight days, the animals were killed and necropsy specimens examined for viral antigen. In the specimens examined three days after inoculation, no label was seen in the cochlea. However, label was found in the spleen with doses of 100 microL, and also in the liver in animals

[Stapedovestibular otospongiosis beginning in the cochlea. Apropos of 3 cases].

Stapedovestibular ankylosis developed after a prolonged course of pure perception deafness in 3 patients, suggesting possible secondary fixation of the stapes from otospongiosis restricted to the cochlear up to that time. A precise diagnosis of pure cochlear otospongiosis is always difficult to make, its presence usually being supposed rather than confirmed in spite of clinical, audiometry and radiology data, except in those few cases where secondary ankylosis of the foot of the stapes occurs. These findings suggested the need for regular audiometric surveillance of all patients with perceptio

An Overview of Central Vertigo Disorders.

Head motion recorded by the vestibular labyrinths is conveyed to specific brainstem and cerebellar structures that relay velocity information to eye muscles to stabilize vision, and to the axial and limb muscles necessary to stabilize balance. Neural networks enhance and extend the primary vestibular signal, and create adaptation to movement when appropriate. Pathological lesions to one or more of these structures may cause central vertigo and imbalance, and may be localized by specific forms of nystagmus and other abnormal neurological signs. Vertigo treatment may be directed to the underlyin

Middle fossa vestibular neurectomy: an update.

Middle fossa section of the vestibular nerve is a neurotologic skull-base procedure that has found a successful, albeit limited, role in the surgical management of Meniere's disease. Incapacitating vertigo can be controlled in 94% of the cases, while serviceable hearing is maintained in 70%. Tinnitus and aural pressure are reduced in 60% and 80% of cases, respectively. The morbidity of this approach is similar to that of other destructive procedures of the labyrinth when careful patient selection and precise surgical technique are employed. In the incapacitated vertiginous patient with service

[Sudden deafness and virus infection (author's transl)].

A survey of the literature concerning sudden deafness caused by virus infection is followed by a discussion of 46 patients with the history of acute loss of hearing, who were subjected to virological-serological examination. In 9 patients the titre movement of the complement fixation reaction was regarded to imply a viral cause of the acute loss of hearing. This was corroborated in 5 cases by the history of recent viral infection. Pathogenetically, the significance of the viral endolymphatic labyrinthitis was underlined. The success of traditional therapy did not differ from the results in cas

Bilateral endolymphatic hydrops in Menière's disease: review of temporal bone autopsies.

The wide range in incidence rates reported for bilateral Meniere's disease results mainly from the lack of established clinical criteria for bilaterality, and from follow-ups of varying duration. In order to help determine a reasonable bilateral involvement rate, we examined histologic reports of temporal bones with endolymphatic hydrops. We reviewed the literature on temporal bone autopsies from 1938 to 1988, and selected 72 cases for histologic examination. Of the 72 cases, 67 (93%) showed endolymphatic hydrops, 2 (2.8%) showed collapse, and 3 (4.2%) showed membranous labyrinths of normal ap

The vestibular aqueduct and endolymphatic sac and duct in endolymphatic hydrops.

The histologic features of the endolymphatic sac and duct in 23 serially sectioned temporal bones with idiopathic or secondary endolymphatic hydrops were blindly compared with 22 randomly selected, normal temporal bones. In idiopathic hydrops, the pars rugosa of the endolymphatic sac extended out of the vestibular aqueduct into the dura in 29% of bones, compared with none of normal bones (P less than .01). In the other 71%, the pars rugosa in the vestibular aqueduct was surrounded by dura more commonly than normal. Functional studies are required to assess the relationship of these findings to

IL-10/HMOX1 signaling modulates cochlear inflammation via negative regulation of MCP-1/CCL2 expression in cochlear fibrocytes.

Cochlear inflammatory diseases, such as tympanogenic labyrinthitis, are associated with acquired sensorineural hearing loss. Although otitis media is extremely frequent in children, tympanogenic labyrinthitis is not commonly observed, which suggests the existence of a potent anti-inflammatory mechanism modulating cochlear inflammation. In this study, we aimed to determine the molecular mechanism involved in cochlear protection from inflammation-mediated tissue damage, focusing on IL-10 and hemoxygenase-1 (HMOX1) signaling. We demonstrated that IL-10Rs are expressed in the cochlear lateral wall

The central nervous system in deaf adolescents.

Studies were made on 100 pupils, recruited from occupational rehabilitation schools for the deaf in Kraków. They underwent general medical as well as specialist examinations. Deafness was found up to the age of 3 years in 75% of those examined. In 30% of the subjects, their vestibular system was paralysed, ophthalmological disorders were found in 95% of cases, neurological disturbances in 15% and pathological changes of the central nervous system in 65% of cases were recorded by EEG. We did not look for evidence that deafness causes anatomical changes in the central nervous system, but the dis

[Vestibular function in female transport workers with cervical osteochondrosis].

A complex study was carried out on the vestibular disorders in 60 transport workers with cervical osteochondrosis in order to determine their incidence and character. The workers' age was from 20 to 60 and a length of service as drivers of transport vehicles - from 3 to 16 and even more years. The vestibular system was studied with regard to spontaneous nystagmic manifestations, the state of stato-kinetics, via experimental rotation and caloric provocations. Changes were observed in space-time perception after the provocation, rheographic hemodynamic changes and serum-biochemical deviations in

Endolymphatic hydrops and Ménière's disease: a lesion meta-analysis.

OBJECTIVE: To determine whether the distribution of membrane lesions associated with Ménière's disease is random, as might be expected from a pervasive process such as hydrops, or orderly, as might be expected if membrane resistance is graded. METHOD: A meta-analysis of temporal bone reports on 184 specimens demonstrating endolymphatic hydrops was undertaken to determine membrane lesion evolution and distribution. RESULTS: Lesion distribution was found to be orderly and cochleocentric. No random scattershot lesions were reported in any study. Disease always started in the cochlear apex, even

[Surgical therapy in Menière's disease. Historical development and today's state of the art].

After P. Menière's first description of the typical symptoms in 1861 it took more than 40 years before the first otosurgical procedures were performed to cure Menière's disease. Various surgical methods were established during the twentieth century, which still are employed in the treatment of intractable Menière's disease, especially saccotomy and vestibular neurectomy but also intoxication of the labyrinth by intratympanic application of gentamicin. Despite the good results of such therapeutic regimens the basic pathological mechanism is still not fully understood. Since the description of a

Histopathological findings in the inner ear caused by measles.

An otopathological analysis of three cases of viral labyrinthitis was performed. Six temporal bones cut in serial sections were available for this study. According to the degree of degenerative changes in various parts of the inner ear two types of morphologically distinct labyrinthitis after measles are presented: the first one with the port of entrance through the internal auditory meatus and characterized by, first and most significantly, changes in the spiral ganglion cells; and the second, as described previously by Lindsay, with the port of entrance of the virus in the inner ear through
