Large Language Model and Natural Language Processing Approach to Identify Cancer Recurrence From Pathology Reports.

PURPOSE: Cancer recurrence is a critical outcome for patients and physicians. Retrospective cancer recurrence data can evaluate recurrence-directed treatment and generate novel interventions targeting recurrent cancers. However, large cancer databases do not provide recurrence-related information, stymying study at scale, and consequently require significant manual record review. Automated evaluation of records may allow for the rapid generation of easily analyzed data sets, accelerating the evaluation of recurrence altogether. METHODS: Patients treated with radiation therapy at one tertiary

Human and large language model judgments of cognitive impairment from language: An explainable artificial intelligence approach.

INTRODUCTION: We examine how non-expert humans (young adults unfamiliar with dementia) and large language models (LLMs) perceive dementia in transcribed texts-recognizing signs that may indicate cognitive decline. Human perception is important, as it is often the driver for seeking medical evaluation. LLM perception is equally interesting given their potential as screening tools. METHODS: Humans and LLMs intuitively judged whether transcribed picture descriptions came from dementia patients or healthy controls. We represented texts using high-level, expert-guided features and used logistic re…

Relation-aware dynamic knowledge subgraph enhanced large language models for medical multiple-choice question answering.

Medical multiple-choice question answering requires models to simultaneously understand question semantics, candidate-answer differences, and structured relations among medical entities in complex clinical contexts. To address the limitations that existing knowledge-enhanced methods often adopt question-level unified modeling and thus struggle to characterize local relational differences among different candidate options, while conventional low-rank adaptation lacks relation-pattern adaptivity, this paper proposes a knowledge-enhanced large language model framework that combines option-level k

ASAS-NANP SYMPOSIUM: MATHEMATICAL MODELING IN ANIMAL NUTRITION: Retrieval Augmented Generation for Large Language Models.

The rapid growth of scientific literature and other unstructured materials has made agricultural decision support increasingly complex. Students, extension agents, inspectors, and private sector researchers increasingly rely on large language models (LLM). These models can produce incomplete, overly cautious, or erroneous responses. We outline how LLM offers a possible opportunity for the animal science discipline, describe what LLM do well and where they fall short, and explain how retrieval-augmented generation reduces fabrication and improves relevance. We present a case study that builds a…

Leveraging computerized adaptive testing for cost-effective evaluation of large language models in medical benchmarking.

Large language models (LLMs) are increasingly applied in healthcare, yet their evaluation relies predominantly on static benchmarks that are costly, contamination-prone, and lack calibrated measurement properties for fine-grained performance tracking. We developed and validated a computerized adaptive testing (CAT) framework grounded in item response theory to enable scalable, psychometrically rigorous assessment of standardized medical knowledge in LLMs. A two-phase study comprising Monte Carlo simulations and empirical evaluation of 38 LLMs was conducted between July and September 2025. The …

Flexible Targeted Adversarial Alignment with Frequency-based Visual Focus for Attacking Large Vision-Language Models.

Although Large Vision-Language Models (LVLMs) have shown great capabilities across various downstream tasks, they are proven to be vulnerable to adversarial examples, thus resulting in critical safety issues. Existing adversarial attacks against LVLMs predominantly rely on uniform, image-wise perturbations, while largely overlooking how these models internally perceive and prioritize visual structures during multimodal alignment. Moreover, most prior LVLM attack methods are designed for either white-box or black-box settings, heavily depending on full-model gradients or sophisticated transfer

Sex and gender bias in large language models: an old problem at a new scale.

Patients are increasingly turning to large language models for health information, which makes the consistency of these systems' outputs across patient groups a public health concern. Demographic bias is harder to see than fabrication because it escapes accuracy benchmarks and shows up instead in the language the models produce. Gender bias is the form most consistently reported, with effects on clinical documentation and reasoning that often compound with ethnicity, socioeconomic status and other attributes. Drawing on evidence from gender medicine, we argue that it calls for sex- and gender-…
