Above-Knee Replantation Following Traumatic Bilateral Amputation: Sciatic Nerve Transplantation.

A 12-year-old boy who was overrun by a train, sustained traumatic bilateral above-knee amputation and a rupture of the symphysis. The left leg had multiple fractures and soft tissue injuries and amputation was necessary. The right one, although severely crushed, at the amputation site and with a MESS of 9, was replanted accepting some shortening and a soft tissue defect at the amputation site, employing saphenic vein grafts from the amputate (left leg) and an early free latissimus dorsi-flap. Septic complications at the amputation site were managed, and an autologous sciatic nerve graft was pe

Hip Abductor Strength Asymmetry: Relationship to Upper Extremity Injury in Professional Baseball Players.

BACKGROUND: Hip strength is an important factor for control of the lumbo-pelvic-hip complex. Deficits in hip strength may affect throwing performance and contribute to upper extremity injuries. HYPOTHESIS: Deficits in hip abduction isometric strength would be greater in those who sustained an upper extremity injury and hip strength would predict injury incidence. STUDY DESIGN: Prospective cohort study. LEVEL OF EVIDENCE: Level 3. METHODS: Minor League baseball players (n = 188, age = 21.5 ± 2.2 years; n = 98 pitchers; n = 90 position players) volunteered. Hip abduction isometric strength w

Traumatic injuries of the popliteal and tibial arteries: limb ischemia and problems of surgical management.

This paper analyzes the results of examination of 106 patients with traumatic injuries of the popliteal (n=36) and leg arteries (n=70) including gunshot (n=48), punctured and incized (n=32), contused-crushed (n=17) wounds and blunt (n=9) traumas. Sixty-two patients were admitted within 8 hours of injury and 20 within 8 to 24 hours, 24 patients within 1 to 15 days. Sixty-five patients had been rendered first aid at other medical institutions. It is to be noted that the prehospital diagnostic and tactic errors had been made in 67.7% of cases. In the patient group with injuries of the leg arterie

The impact of lateral ankle ligament injuries on ankle stability and talar cartilage stress: a finite element analysis of combined injury mechanisms.

OBJECTIVE: To investigate the differential biomechanical effects of injuries to the superior and inferior fascicles of the anterior talofibular ligament (ATFL) and the calcaneofibular ligament (CFL) on ankle stability and talar cartilage stress, providing a basis for the Fascicle-specific diagnosis and treatment of chronic ankle instability. METHODS: A finite element model incorporating bones, articular cartilage, and nine sets of ligament spring elements was created based on CT data from the ankle of a 26-year-old male volunteer. Six conditions were simulated: 1. intact model; 2. superior AT

Apligraf as an Alternative to Skin Grafting in the Pediatric Population.

Split-thickness skin grafting and healing by secondary intention are the most common options for the treatment of full-thickness skin injuries. This case explores Apligraf (Organogenesis Inc., Canton, Massachusetts) as an alternative treatment for full-thickness skin injuries in the pediatric population. Apligraf, a bioengineered living cell construct, is an advanced wound care modality that is commonly used to treat chronic, nonhealing venous leg ulcers and diabetic foot ulcers. This case demonstrates Apligraf as a viable, if not superior, treatment option for full-thickness skin injury.

Refractory Lateral Leg Pain in a 25-Year-Old Competitive Runner: A Report of a Case of Fascial Herniation of the Peroneus Brevis With Complete Resolution After Surgical Decompression.

Fascial herniations of the leg occur when an intracompartmental leg muscle protrudes through weaknesses in the overlying fascial sheath. These fascial defects may be congenital or acquired from trauma involving penetrating injuries to the fascia. Increases in intracompartmental pressure, often resulting from muscular hypertrophy, can lead to muscular herniation through the weakened fascia. This may present as a leg mass which is often misdiagnosed as a hematoma, varicosity, or soft-tissue mass, leading to significant delays in treatment. We present a case of a peroneus brevis herniation in a 2

[Contralateral replantation after bilateral traumatic lower leg amputation. Case report with 6 year follow-up].

A 66-year-old patient attempted suicide by jumping in front of a train. The lower extremities were amputated at different levels. On the right side, there was a complete amputation within the distal third of the lower leg. Proximal to the amputation site, there was an extensive soft-tissue and bone defect. On the left side, there was a crush injury of the tarsal and mid-tarsal bones. The left lower leg showed only few injuries. An ipsilateral (anatomical) replantation was not possible. In order to save one lower extremity, we decided to carry out a cross-over (contralateral) replantation of th

Survey of the effects of aerobic dance on the lower extremity in aerobic instructors.

The rate of aerobic dance injuries has been high for two decades. To determine the types of lower-extremity injuries to aerobic instructors, a questionnaire was sent to 18 fitness centers in the Sydney, Australia, metropolitan area requesting information on the number and types of injuries, frequency of activity levels, footwear worn, and treatments sought. The reported rate of injury was 77%. The leg was the most common site of injury, reported by 52.9% of respondents, followed by the foot and ankle (32.8%), and the knee (20%). These figures are comparable to previous studies. Further investi

[Traffic multiple trauma patient with head injuries: a study of 146 cases].

146 cases of traffic multiple trauma associated with head injuries were studied. There was a significant difference in the ratio of associated injuries in various parts of the body. The three most encountered parts were chest, shoulder and leg. Death were mainly due to severe head injury, chest injury and shock. Atypical ushing syndrome was due to severe blood loss, even intracranial hypertension existed. The blood pressure decreased rapidly after operation. Since comatous patients were unable to provide chief complaint, misdiagnosis occurred during emergency stage.

Shin-splints: common exercise-related syndromes affecting the lower leg.

Lower leg pain is a common complaint of athletically active individuals, often limiting physical activities. As such, the group of lower leg conditions related to athletic pursuits and physical exercise confer considerable operational implications for the military. Whilst acute injuries to the lower limb are commonly encountered and are clearly of significance, this article focuses instead on chronic conditions related to physical activity. These include insults to bone such as stress fractures and medial tibial stress syndrome, and those related to the soft tissues such as chronic exertional

[Clinical application of medial leg skin flap for the repair of defects of the extremities].

A total of fifteen medial leg skin flaps with the posterior tibial artery as an axial vessel have been applied successfully to reconstruct extremity defects, such as chronic ulcers, scar contracture, high-voltage electric injuries and hot crush, etc. This skin flap can be elevated as a free flap, or vascular island flap, or cross-leg vascular pedicle flap to resurface the defects on the extremities. It is especially suitable for foot repair because of its long vascular pedicle and wide rotational arc as an island skin flap. The paper describes and discusses the indications, design and operatin

A Storm, An Explosion, and Flying Rocks - An Unusual Injury due to a Lightning Strike in the Mountains.

BACKGROUND: Most injuries observed in victims of lightning strikes can be explained by electrothermal phenomena. Blast penetrating injuries caused by a lightning-strike-induced explosion of a nearby structure are rarely reported. CASE PRESENTATION: Here reported is the case of a patient with numerous mixed injuries caused by a lightning strike, including deep lacerations of both hips and thighs with rock fragments embedded in the wounds. Surgical removal of rock fragments from deep areas of the right hip and right lower leg was necessary. The cause of the formation of rock missiles was the li

Ankle injuries in the growing skeleton. A review of the recent literature.

Ankle injuries arise with significant frequency in the pediatric population, although diagnosis is rarely straightforward. Contact sports, such as football are widely practiced in Saudi athletes, little is carried out to educate them of measures regarding playing conditions and prophylactic steps like foot wear, warming up, and similarly. The usual potential complications associated with trauma to mature skeleton, as well as immature selection includes post-traumatic arthritis, stiffness, reflex sympathetic dystrophy, as well as those seen with physeal damage including leg length discrepancy,

[Short-Term Cryotherapy--Putting Dynamic Foot and Ankle Stability at Risk?].

BACKGROUND: The application of ice or other forms of cooling represent a common method to treat acute musculoskeletal injuries during sporting events in order to reduce pain. Often athletes return to competition immediately after cooling. It is not known if short-term cryotherapy in the form of ice spray has an influence on the joint's dynamic stability. The aim of this study was to investigate if application of ice spray to the ankle has an effect on the dynamic stability of the joint in healthy participants. METHODS: A randomized controlled single-blind pilot study with crossover-design was

Feet injuries in rock climbers.

While injuries of the upper extremity are widely discussed in rock climbers, reports about the lower extremity are rare. Nevertheless almost 50 percent of acute injuries involve the leg and feet. Acute injuries are either caused by ground falls or rock hit trauma during a fall. Most frequently strains, contusions and fractures of the calcaneus and talus. More rare injuries, as e.g., osteochondral lesions of the talus demand a highly specialized care and case presentations with combined iliac crest graft and matrix associated autologous chondrocyte transplantation are given in this review. The

Transposition of muscle flaps for covering exposed bone in the leg.

A muscle transposition technique for reconstruction of soft tissue loss and skin defects with exposed bone in the leg is described. Reconstruction with muscle flaps and freee skin grafts is a reliable and convenient one-stage operation. The gastrocnemius and soleus muscles were used to cover the upper and middle third of the leg. The method was used in 13 patients, and there were 3 failures in the series because of the impaired vascularity of the injuried leg. In 7 cases, including one failure, the soft tissue injury was associated with compound fractures. No functional disability was observed

Posttraumatic torsional injury as an indirect cause of fibular intraneural ganglion cysts: case illustrations and potential mechanisms.

Trauma has been loosely associated with the development of fibular intraneural ganglion cysts. Sporadic reports of direct trauma to the proximal lateral leg in the region of the superior tibiofibular joint (STFJ) capsule support the development of a capsular rent and the subsequent egress of joint fluid from the articular joint. This report provides evidence to suggest that indirect trauma from torsion can link concomitant ankle injury and fibular nerve palsy (foot drop) and fibular intraneural ganglion cysts. We present two cases to illustrate different potential mechanisms. One patient susta
