Middle segment surgery: indications, techniques, and future directions.

PURPOSE OF REVIEW: Middle segment surgery (MSS) refers to ophthalmic surgical intervention through the pars plana to prevent or manage anterior segment abnormality and surgical challenges/complications. It has gained interest in recent years due to its expanding indications and utility across ophthalmic subspecialties. Although MSS has been employed for decades, its integration into contemporary ophthalmic surgery requires a multidisciplinary understanding of both anterior and posterior segment diagnostics, treatment options and surgical techniques. RECENT FINDINGS: Although the nomenclature

The eye as a window to a rare disease: ectopia lentis and homocystinuria, a Pakistani perspective.

Non-traumatic ectopia lentis has been associated with genetic diseases in a European population; however, no data are present in regards to this in a Pakistani demographic. In third world countries such as Pakistan, due to the lack of screening tests, this disease has the potential to remain undiagnosed till a later age, at which point the eye through the finding of ectopia lentis has potential to lead to the right diagnosis. Our purpose was to investigate Pakistani patients presenting with ectopia lentis who have underlying homocystinuria and establish a relationship between the two. Addition

Canine Secondary Glaucomas.

Secondary glaucomas are common in dogs, and occur due to obstruction of aqueous humor flow at the pupil, iridocorneal angle, or trabecular meshwork by numerous mechanisms. Secondary glaucoma is suspected based on examination findings, or presence of elevated IOP in an animal with a signalment inconsistent with primary glaucoma. Animals with secondary glaucoma require more diagnostic testing than animals with primary glaucoma. Management is challenging, and treatments used for primary glaucoma may be ineffective or even detrimental. Prognosis for vision and/or globe retention may be better than

[Corneal Endothelial changes after insertion of iridocapsular (Two-loop) lenses (author's transl)].

The corneal endothelial damage in extracapsular extraction and insertion of a two-loop lens (Binkhorst) using the deep chamber technique was investigated by specular microscopy. The endothelial cell loss in 25 patients was 15%. During a mean postoperative period of 15 months a further endothelial cell loss of 5% occurred. The effect of a direct contact between an intraocular lens and the corneal endothelium is demonstrated by a case of luxation of a two-loop lens into the anterior chamber.

Pseudophakic bullous keratopathy. A clinical-pathologic analysis.

Of 226 patients who had an intraocular lens implanted over a three year period, 6.2% developed bullous keratopathy compared with 2.2% of 90 patients who had a standard cataract extraction performed by the same surgeon. An additional 4.0% of the lens implant patients developed peripheral or intermittent corneal edema. The factors related to the higher incidence of this complication in lens implant patients included corneal touch during implantation, prolonged postoperative iritis and, in cases of mild or peripheral corneal edema without bullous keratopathy, recurrent lens dislocation. The most

Post-traumatic iridodialysis, crystalline dislocation and vitreous haemorrhage: how to manage.

A 66-year-old man, while taking care of his horse, sustained a blunt, non-penetrating injury to the right side of his face, which damaged his eye. On slit lamp examination, iris dialysis and crystalline dislocation in the vitreous chamber were observed. On presentation, his best correct visual acuity was hand motion. A 23 G vitrectomy and subsequently an iris reconstruction and a glued intraocular lens implant were performed. Visual acuity reached +0.1 logMAR 1 month after surgery and remained stable after 12 months' follow-up.

Management of subluxed and dislocated intraocular lenses in patients with uveitis: a practical approach.

Purpose: To describe practical approaches to the management of subluxed or dislocated intraocular lenses (IOL) in patients with uveitis.Patients and methods: Retrospective case series from a specialist uveitis clinicResults: Fifteen IOLs in 13 patients were subluxed inferiorly (12) or dislocated into anterior chamber (2) or vitreous (1) at a mean delay of 12 years after cataract surgery. Six eyes required vitrectomy and seven IOL explantation. A dislocated IOL was repositioned by scleral fixation in one, and a new IOL was implanted in three (two scleral-sutured, one iris-claw). Eight were obse

[Recurrent spontaneous pneumothorax revealing Marfan's syndrome].

INTRODUCTION: Because of its rarity and lack of specific symptoms, the diagnosis of Marfan's syndrome can be delayed. Though cardiovascular, skeletal and ophthalmological manifestations are the most frequent features, the respiratory system can also be involved. OBSERVATION: We report the case of a 35-year-old patient who presented with a large, right sided, encysted pyopneumothorax. The history revealed two episodes of spontaneous pneumothorax, cataract surgery ten years before his admission, and a similar case in the family. Clinical examination showed skeletal abnormalities of the wrist an

Iridoschisis associated with nanophthalmos and bullous keratopathy.

Iridoschisis is a rare condition in which the iris stroma splits into anterior and posterior layers. Iridoschisis is associated with anterior segment abnormalities including keratoconus, lens subluxation and angle-closure glaucoma. We describe the first case in which iridoschisis has occurred in association with nanophthalmos. We also report the rare complication of irido-corneal touch and subsequent corneal decompensation. Nanophthalmos and irido-corneal touch are sight-threatening associations of iridoschisis and should be excluded in all patients with this rare condition.

[Variability in the clinical presentation of the Brown-McLean syndrome].

CASE REPORT: We report two aphakic patients with Brown-McLean syndrome. DISCUSSION: One patient was affected by Marfan syndrome, after having undergone lens subluxation surgery and aphakia 23 years previously. The other patient was aphakic due to cataract surgery with complications three years before. Our cases demonstrate that this syndrome can show a variety of clinical characteristics, but peripheral corneal edema is always present. A full understanding of the clinical signs of presentation is of great importance in order to detect this syndrome.

Multicenter survey on implantable collamer lens dislocation.

This study aimed to investigate the incidence, patient background, and postoperative prognosis of implantable collamer lens (ICL) dislocation. We retrospectively reviewed all cases of ICL dislocation at four major refractive surgery centers in Japan until December 2019. The incidence, patient background, cause of dislocation, complications of repositioning surgery, and postoperative visual function were investigated. Seven ICL dislocations [0.072% of total ICL-implanted eyes (9775 eyes)] occurred at an average of 28.6 months (11-82 months) postoperatively. All patients were male. Five eyes wer

Out-of-the-bag intraocular lens dislocation: outcomes of posterior chamber intraocular lens exchange, risk factors, and prevention.

BACKGROUND: Dislocation of posterior chamber intraocular lens is one of the most common complications of intraocular lens implantation. Lens exchange is an effective solution to this unsatisfactory status. This study was conducted to analyze the possible predisposing factors for out-of-the-bag posterior chamber intraocular lens dislocation and to study the outcomes of lens exchange surgery. METHODS: Thirty-six consecutive patients (36 eyes) with out-of-the-bag intraocular lens dislocation who underwent posterior chamber intraocular lens exchange in Zhongshan Ophthalmic Center of Sun Yat-sen U

Adenoma of nonpigmented ciliary epithelium with smooth muscle differentiation.

A 35-year-old man underwent successful iridocyclectomy for a ciliary body mass that was subluxating the lens and causing a secondary cataract. Histopathologically the mass proved to be a spindle cell tumor, but leiomyoma and melanoma were initially considered to be diagnostic possibilities. However, further studies and immunohistochemical studies revealed that the tumor probably arose from the nonpigmented ciliary epithelium. The spindle cells represented smooth muscle differentiation within the mass. The final diagnosis was adenoma of the nonpigmented ciliary epithelium with smooth muscle dif

Aniridic Fibrosis Syndrome in a Child with Ahmed Glaucoma Valve: Report of a Case and Review of the Literature.

PURPOSE: To report a case of aniridic fibrosis syndrome (AFS) after Ahmed glaucoma valve (AGV) surgery. METHODS: Case report. RESULTS: Two years post-AGV surgery in both eyes, a 3-year-old aniridic female presented with grayish discoloration of the right eye for the past 3 months. A slit-lamp examination of the right eye revealed an aniridic, hypotonic eye with a totally edematous cornea. A tube of the AGV was in place with a mature cataract subluxed superiorly. In addition, a thick, whitish vascularized membrane originating from the rudimentary iris to the inferior part of the subluxed lens

Isolated sulfite oxidase deficiency: review of two cases in one family.

OBJECTIVE: The authors describe two cases of isolated sulfite oxidase deficiency found in one family. This is a rare autosomal-recessive disorder presenting at birth with seizures, severe neurologic disease, and ectopia lentis. It can be easily missed with metabolic screening; however, the finding of lens subluxation stresses the importance of ophthalmic assessment in making the diagnosis. DESIGN: Two observational case reports. INTERVENTION/METHODS: Ophthalmic assessment, biochemical assay for specific urinary and plasma metabolites, magnetic resonance imaging, and gene sequencing were used…

Mandibulo-facial dysostosis--the eye signs of a case study.

The mandibulo-facial dysostosis syndrome (Treacher-Collins syndrome) was first described in 1889. It is a syndrome with multiple presentations, the classification for which was devised by Franceschetti and Zwahlen in 1944. The eye signs are an important part of this syndrome. In addition to the main ocular features of colobomata of the lower eyelids and an anti-mongoloid slant, many other eye signs have been reported. An 18-year-old Indian male was found to have features not previously described. These are high myopia, dermolipoma, lens subluxation and secondary glaucoma.
