Oral lichen planus and HLA A.

Oral lichen planus (OLP) is autoimmune disease that appears on the oral mucosa only, or in combination with skin lesions. The occurrence of squamous cell carcinoma in areas of lichen erosions makes it necessary to follow up patients suffering from chronic lesions. Prevalence in general population varies from 0.9% to 1.2% and varies in different ethnic groups. It is known that cell-mediated immune mechanisms and genetic factors underline OLP pathogenesis. We tested 50 OLP unrelated patients (5 males) for HLA A and compared them to 1089 healthy controls. OLP patients had 3 times less frequently

Lichen planus localized to the nails.

BACKGROUND: Lichen planus may rarely be localized to the nails. OBJECTIVE: A case of ungual lichen planus is reported that was misdiagnosed as onychomycosis. METHODS: A 52-year-old woman had progressive nail dystrophy for 13 months that was unresponsive to oral terbinafine. A nail biopsy was performed. RESULTS: Histopathologic findings were consistent with lichen planus. CONCLUSION: Onychomycosis may be confused clinically with other causes of nail dystrophy. It is important to confirm the diagnosis of onychomycosis with appropriate laboratory or histologic analysis.

Cytologic findings in lichen sclerosus.

OBJECTIVE: To describe the findings in gynecologic smears from 29 patients with biopsy-proven vulvar, vaginal or perineal lichen sclerosus. STUDY DESIGN: The computerized database of the surgical pathology division of the Hospital of the University of Pennsylvania was searched for the diagnosis of lichen sclerosus from the years 1989-1992. Once these patients were identified, a search for all associated gynecologic material, including cytologic smears, biopsies and resections, was conducted. All the corresponding slides were obtained from the files and reviewed. RESULTS: Parakeratosis was pr

Tenascin expression in lichen sclerosus.

In this study, the distribution of tenascin immunoreactivity in 44 cases of vulvar lichen sclerosus (LS) was investigated. To assess the epithelial basement membrane structure, immunostaining with an antibody to type IV collagen was performed. Ten selected cases were also analyzed by in situ hybridization with a tenascin RNA probe to study the cellular distribution of tenascin mRNA synthesis in LS. Strong tenascin immunoreactivity could be found in LS, especially in areas with subepithelial edema and marked inflammation. By in situ hybridization, signals for tenascin mRNA could be found in bas

Diffuse and bilateral lichen striatus.

A 5-year-old girl had a papulo-squamous dermatosis arranged in a Blaschko line pattern of 1 month's duration. The skin lesions were asymmetrically distributed bilaterally on the face, neck, trunk, and limbs. Histologically there was a lichenoid infiltrate with foci of spongiosis. The dermatosis cleared spontaneously with hypopigmentation 16 months later. Lichen striatus usually occurs in a solitary and unilateral fashion. Its diffuse and bilateral distribution is an uncommon presentation.

Lichen amyloidosus: a consequence of scratching.

BACKGROUND: Lichen amyloidosus (LA) is generally said to be a pruritic type of amyloidosis of unknown cause. Histopathologically, it is characterized by epidermal changes of lichen simplex chronicus and by deposits of amyloid in the papillary dermis that are derived from keratin peptides of necrotic keratinocytes. Chronic scratching is responsible for the development of lichen simplex chronicus and may lead to necrosis of individual keratinocytes. OBJECTIVE: Our purpose was to evaluate whether chronic scratching may also be responsible for the formation of amyloid in LA. METHODS: We studied

Anogenital lichen sclerosus in women.

A study of 350 women with lichen sclerosus, originally made to elucidate the relationship between lichen sclerosus and autoimmunity, led to the amassing of a considerable amount of clinical material. Our review is confined to those with anogenital lesions (342), supplemented by some new cases (15), giving a total of 357 women with biopsy proven lichen sclerosus. It demonstrates the wide age range of the condition, the association with morphoea and lichen planus and the occurrence of squamous cell carcinoma in some cases. It also shows that inappropriate surgery has continued to be carried out

Isolated lichen planus of the lip.

Oral lichen planus (LP) is a relatively common disorder of unknown aetiology. Oral LP occurs most often on the buccal mucosa, but the gingivae, tongue, floor of the mouth, retromalar pads and lips may also be affected. Usually, patients have multiple sites of involvement. We report a 44-year-old patient with a 3-year history of isolated swelling of the lower lip, erosions and crusting. Histology of a biopsy from the lip revealed features of LP. Oral treatment with acitretin and low-dose steroid led to complete resolution of the lesions within 10 weeks. This is the first well-documented case of
