Pediatric palliative care-when quality of life becomes the main focus of treatment.

Pediatric palliative care (PPC) focuses on children and adolescents with life-limiting diseases. It may be initiated at various points of the disease trajectory, if possible early enough to support living with the best possible quality of life despite a limited lifespan. From birth to adolescence, children with a broad spectrum of diseases may benefit from PPC. Since 50% of deaths in childhood occur within the first year of life, PPC is just as relevant to neonatology. Causes of death in the neonate and young infant are due to perinatal conditions such as preterm birth and congenital disorders

Prehospital care providers' understanding of responsibilities during a behavioural emergency.

BACKGROUND: Prehospital emergency care providers are frequently called to assist with the management of mental healthcare users (MHCUs). The Mental Health Care Act no. 17 of 2002 regulates mental healthcare in South Africa, but the act fails to consider the responsibilities of prehospital emergency care providers in the provision of mental healthcare. Rather South African Police Services were given authority over the well-being of a MHCU in the prehospital setting. AIM: To investigate prehospital emergency care providers' understanding of their responsibilities towards MHCUs and the community

Pancreatic cancer-A disease in need: Optimizing and integrating supportive care.

Pancreatic ductal adenocarcinoma (PDAC) is an aggressive malignancy that continues to be challenging to treat. PDAC has the lowest 5-year relative survival rate compared with all other solid tumor malignancies and is expected to become the second-leading cause of cancer-related death in the United States by 2030. Given the high mortality, there is an increasing role for concurrent anticancer and supportive care in the management of patients with PDAC with the aims of maximizing length of life, quality of life, and symptom control. Emerging trends in supportive care that can be integrated into

Determinants of Family Caregiver Well-Being in Home-Based End-of-Life Care: A Qualitative Synthesis.

CONTEXT: Family caregivers are fundamental to enabling home-based end-of-life care, yet their well-being remains inadequately understood. While previous research has documented caregiver burden, a comprehensive understanding of the multidimensional factors shaping caregiver well-being across diverse contexts remains lacking. OBJECTIVES: To synthesize qualitative evidence on the determinants of family caregiver well-being in home and community end-of-life care settings, developing an integrated conceptual understanding of facilitators and barriers across physical, psychological, social, and sp

METHODS: Fifty-one community nurses with recent experience of delivering end-of-life care in the United Kingdom completed a survey about changes to their roles. A purposive sample of 35 respondents participated in focus groups exploring these changes in more depth; thematic analysis was used with constant comparison. RESULTS: As well as two new roles-prescribing and verifying death-many participants talked about a broader expansion of

Uncontrolled DCD: When Should We Stop Trying to Save the Patient and Focus on Saving the Organs?

Uncontrolled donation after circulatory death, which occurs when an individual has experienced unexpected cardiac arrest, usually not in a hospital, generates both excitement and concern. On the one hand, uDCD programs have the capacity to significantly increase organ donation rates, with good transplant outcomes-mainly for kidneys, but also for livers and lungs. On the other hand, uDCD raises a number of ethical challenges. In this essay, we focus on an issue that is central to all uDCD protocols: When should we cease resuscitation and shift to organ preservation? Do current uDCD protocols pr

Anaphylaxis.

Anaphylaxis is a systemic, life-threatening disorder triggered by mediators released by mast cells and basophils activated via allergic (IgE-mediated) or nonallergic (non-IgE-mediated) mechanisms. It is a rapidly evolving, multisystem process involving the integumentary, pulmonary, gastrointestinal, and cardiovascular systems. Anaphylaxis and angioedema are serious disorders that can lead to fatal airway obstruction and culminate in cardiorespiratory arrest, resulting in hypoxemia and/or shock. Often, these disorders can be appropriately managed in an outpatient setting; however, these conditi

A qualitative analysis of best self-management practices: sickle cell disease.

BACKGROUND: Sickle cell disease (SCD) is associated with serious comorbidities resulting in a shortened lifespan, and many clients suffer from frequent pain episodes. However, others successfully manage their disease in the outpatient setting without the need for frequent health care utilization. The purpose of this project was to describe specific strategies used by adult clients with sickle cell disease to achieve optimal physical health. METHOD: A Best Self-management Practices workshop was held in conjunction with the Sickle Cell Disease Association of America meeting. A panel discussion

Brain death.

Mechanical ventilation frequently saves lives that are threatened by temporary respiratory failure; but when breathing stops due to irreversible brain damage ventilation only prolongs the process of dying, as organs serially cease to function. The possibility of extending from a few minutes to many days the interval between final failure of the brain and ultimate cardiac asystole emphasises that death is not an event, but a process.

Traces of spiritual care in nursing records: A qualitative study of cancer care.

AIM: The aim of this study is to explore how spiritual care is expressed in nursing records in cancer care. METHOD: This study adopted a hermeneutic approach, and a qualitative content analysis was used to explore the nursing records of 43 inpatients with cancer from Norway. Ethical approval was obtaine

Better care must be part of the conversation on dying.

As debate around assisted dying continues, the longstanding pressures facing palliative, end-of-life and social care cannot be ignored. With concerns over funding, capacity and access repeatedly raised, the author questions whether healthcare services can properly support people in living well and ensure those approaching the end of life receive the care and dignity they deserve.
