[Application of occlusal guide plate combined with intermaxillary fixation screw in mandibular defect repair with free fibular flap].

OBJECTIVE: To investigate the clinical value of occlusal guide plate combined with intermaxillary fixation screw in mandibular defect repair with free fibular flap. METHODS: Between August and December 2011, 7 patients with mandibular tumor were treated, including 5 cases of ameloblastoma and 2 cases of gingival cancer. Of 7 patients, 4 were males and 3 were females, aged 32-65 years (median, 50 years). Occlusal guide plate was prepared and the implanted position of intermaxillary fixation screws was determined preoperatively. Hemimandibulectomy was performed in 5 cases, half mandibular segme

Mandibular metastasis of pulmonary adenocarcinoma: How unexpected could it be?

OBJECTIVE: Metastatic tumours of bone must be considered in all patients with unexplained bone pain and particularly in patients who present with a known cancer, localised pain at multiple sites, and radiographic findings suggestive of metastasis. The purpose of this report was to present a case of a pathological fracture of the mandible as a consequence of metastatic pulmonary adenocarcinoma. MATERIALS AND METHODS: In July 2018 a 68-year-old male patient was hospitalised because of pulmonary adenocarcinoma and attended our department for an oral maxillo-facial evaluation. He complained of pa

Tumor-induced hypophosphatemic osteomalacia caused by a mesenchymal tumor of the mandible managed by a segmental mandibulectomy and microvascular reconstruction with a free fibula flap.

BACKGROUND: Tumor-induced osteomalacia is a rare paraneoplastic syndrome in which patients develop hypophosphatemia and osteomalacia. METHODS AND RESULTS: Here, we report a unique case of a 42-year-old man who presented to our institution with a 1-year history of pain in his ribs, hips, lower back, and feet. Radiologic examination revealed a decrease in bone density and multiple insufficiency fractures. Laboratory evaluation revealed hypophosphatemia, low serum 1,25 dihydroxy vitamin D3 , and elevated fibroblast growth factor 23 (FGF23). A positron emission tomography/CT scan showed increased

Malignant transformation of ameloblastic fibroma to ameloblastic fibrosarcoma: case report and review of the literature.

INTRODUCTION: Ameloblastic fibrosarcoma is a rare malignant odontogenic tumour and is regarded as the malignant counterpart of the ameloblastic fibroma. The epithelial component remains benign, but the mesenchymal component becomes malignant. The diagnosis is made by histopathology. PATIENT: The case of a 26-year-old man who underwent curettage of an ameloblastic fibroma and died of an ameloblastic fibrosarcoma is presented, and the course of malignant transformation is analysed retrospectively. CONCLUSION: One-third of ameloblastic fibrosarcoma cases seem to have developed from recurrent am

[Establishment and application of a digital workflow in the treatment of condylar osteochondroma and secondary dentofacial deformities using image-guided endoscopic navigation combined with orthognathic surgery].

Objective: To establish a digital workflow in the treatment of mandibular condylar osteochondroma with secondary dentofacial deformities using navigation and endoscope combined with orthognathic surgery. Methods: Thirty-six patients with unilateral condylar osteochondroma were analyzed retrospectively. Preoperative planning and simulation were carried out on the digital three-dimansional (3D) model in all patients. With the aid of image-guided endoscopic navigation, osteochondroma resection and condylectomy were accurately performed. Secondary dentofacial deformities were simultaneously correc

[Aneurysmal cysts located in the maxilla or mandible (author's transl)].

Aneurysmal cysts of the mandible and maxilla are rare conditions, though not exceptional, which are seen in particular in the child and adolescent. A part from facial asymmetry and tumefaction, it is rare to find other clinical manifestations. No neurological signs were seen in the present study. An aneurysmal cyst gives a radiological appearance of non-characteristic lysis compatible with a large range of diagnoses. Histological study permits early diagnosis but must always involve the whole specimen with careful examination to detect any other associated lesion. The pathogenesis of the condi

Metastatic recurrent giant orbital ameloblastoma: A rare case report and literature review.

RATIONALE: Orbital ameloblastoma is a rare benign tumor with metastatic potential, typically exhibiting follicular or plexiform histopathological patterns. Ameloblastoma commonly occurs in the jaws and rarely metastasizes; when it does, the lungs and lymph nodes are the most frequent secondary sites. In the world's reported literature, there are 32 cases of maxillary ameloblastoma metastasizing to the orbit, with only 4 cases of mandibular ameloblastoma metastasizing to the orbit. The mystery lies in its high recurrence rate and aggressive malignant potential despite being classified as a beni

Quantitative Assessment of Bone Density at the Borders of Radiolucent Mandibular Lesions Using Cone-Beam Computed Tomography: Correlations With Lesion Aggressiveness.

INTRODUCTION: Radiolucent mandibular lesions present diagnostic challenges owing to their diverse nature, ranging from benign cysts to aggressive neoplasms, with varying impacts on the surrounding bone. This study aimed to evaluate bone density variations at the borders of these lesions using cone-beam computed tomography (CBCT) and to correlate these findings with histopathological indicators of lesion aggressiveness to identify quantitative imaging markers for improved diagnosis and treatment planning. MATERIALS AND METHODS: A retrospective cross-sectional study was conducted, involving 125

[Metastatic lung cancer origin from osteosarcoma of mandible invading tracheal lumen].

A 52-year-old woman underwent the surgical treatment for osteosarcoma of the left mandible in 2003 and was followed up afterward. She suffered from dry cough and bloody sputum, and was admitted to our hospital in April 2007. Computed tomography (CT) revealed several nodules in bilateral lung, and bronchofiberscopy showed the endobronchial tumor obstructing in the right main bronchus. The metastatic tumor progressed in the right main bronchus from the right S6 lung segment. The tumor rapidly progressed in the right bronchus in comparison with the CT findings in about 2 weeks, and the possibilit

Recurrent Primordial Odontogenic Tumor: Epithelium-Rich Variant.

Primordial odontogenic tumor (POT) is a rare, mixed odontogenic neoplasm composed of spindled and stellate-shaped cells in myxoid stroma resembling dental papilla, surfaced by cuboidal-to-columnar odontogenic epithelium. Most POTs present in the posterior mandible as a well-demarcated radiolucency associated with a developing tooth in children and adolescents. POT is treated conservatively with no recurrences documented to-date. To describe the clinicopathological features of a recurrent POT. A 19-year-old female presented with an asymptomatic swelling, and panoramic radiograph revealed a mult

Parapharyngeal branchial cleft cyst extending to the skull base: a lateral transzygomatic-transtemporal approach to the parapharyngeal space.

Second branchial cleft cysts (BCCs) can occur anywhere from the tonsillar fossa to the supraclavicular area. Second BCCs usually lie on the great vessels of the neck, deep in the sternocleoidomastoid muscle or along its anterior border at the mandibular angle. Parapharyngeal second BCCs are an uncommon neoplasm and rarely extend to the skull base. We report the case of a 45-year-old male with symptoms of conductive hearing loss. Temporal bone CT scan showed effusion in the left middle ear cavity. Magnetic resonance imaging (MRI) revealed a cystic mass that was located at the left parapharyngea

A retrospective histopathological review of oral squamous cell carcinoma in a Nigerian teaching hospital.

This study was undertaken to describe the demographic pattern of oral squamous cell carcinoma (OSCC) in Ibadan, Nigeria and compare our findings with that of other countries. It involved a retrospective review of OSCC diagnosed at the University College Hospital Ibadan, Nigeria between 1990 and 2008. A review of studies from other countries was done with respect to the three most frequently affected sites by OSCC, as well as mean ages and gender ratios. OSCC comprised 181 (43.7%) of the 414 malignant neoplasms diagnosed in the oral cavity within the study period. The most frequently affected s

Segmental mandibulectomy and microvascular reconstruction with fibula free flap: Comparison between intraoral and extraoral approaches.

BACKGROUND: Segmental mandibulectomy and reconstruction of resulting defect can be performed via intraoral approach (IOA) or extraoral approach (EOA). Both approaches have advantages, disadvantages, indications, and contraindications to consider during their selection. OBJECTIVE: To compare IOA vs EOA of segmental mandibulectomy and microvascular reconstruction with fibula free flap (FFF). METHODS: We conducted a retrospective cohort study in which 51 patients who underwent segmental mandibulectomy and microvascular reconstruction with FFF from 2020 to 2024 were included, especially 17 patie

Postradiotherapy Prosthetic Management of Mandible Reconstructed With Fibular Graft.

Intraoral defects secondary to resection of mandible leading to mandibular discontinuity are very complicated to treat as the muscles exert forces on the residual portion of the mandible. Primarily this leads to disfigurement of the face. These forces also make mastication difficult due to imbalance. Ultimately it leads to poor quality of life for the patient. Apart from that the clinical outcome and prognosis of prosthetic rehabilitation of such patients is very poor. The composite fibular flap is the preferred donor site for reconstruction of most complex orofacial-mandibular defects. The ad
