OBJECTIVE: To evaluate the long-term outcomes of trans-mastoid plugging of superior semicircular canal dehiscence (SSCD), focusing on complicated cases.
METHODS: In this cohort study, we included all patients who underwent trans-mastoid plugging of SSCD between 2009 and 2019. We evaluated the symptoms (autophony, sound-/pressure-induced vertigo, disequilibrium, aural fullness and pulsatile tinnitus) before and 1 year after surgery in the medical records. We systematically assessed the current symptoms 6.2 ± 3 years postoperative (range 2.2-12.3 years) using questionnaires sent by post and val…

INTRODUCTION: Poor access to the difficult areas in the middle ear and mastoid cavity is considered as the major reason for failure in mastoid surgery. Wide field visibility, visualization of nooks and corners by an endoscope could contribute to better clinical control of the disease in these patients that cannot be accessed by the operating microscope. The study was done to assess and clean postoperative canal wall down mastoidectomy cavities with endoscope and compare with oto-microscopy.
METHODS: This was a descriptive cross-sectional study, done in Kathmandu Medical College from January t…

An inverted papilloma is a benign neoplasm that occurs exclusively in the sinonasal cavity; an inverted papilloma involving the mastoid cavity is extremely rare. We present the case of a patient with an inverted papilloma of the mastoid cavity secondary to cholesteatoma surgery. No case that occurred after acquired cholesteatoma has been reported in literature. A 39-year-old female who was diagnosed with cholesteatoma was treated with a modified radical mastoidectomy in 1988. After recurrence, the patient underwent a canal wall down mastoidectomy in 2006. Four years later, the patient complain…

Inverted papilloma or Schneiderian papilloma arising primarily from middle ear and mastoid is a very rare entity. It is a benign and locally aggressive condition, with increased risk of recurrence and malignant transformation. Surgery is the primary treatment. Radiation therapy should be considered in case of malignant transformation, recurrent or inoperable disease. Here we are reporting a case of recurrent inverted papilloma with dysplastic changes arising from mastoid cavity in a 60-year-old male. The patient was treated with surgery followed by radiotherapy.

OBJECTIVES: We present the surgical techniques and outcomes of mastoid surgery under local anesthesia in patients who were unfit for general anesthesia.
METHODS: Five tertiary-referred patients with multiple comorbidities and failed conservative treatment for chronic otitis media were operated on under local anesthesia. No sedation was administered. The principles of cholesteatoma surgery were observed, but the technique was adapted to keep surgical time to a minimum.
RESULTS: None of the patients had perioperative problems, and all have dry, waterproof ears with preservation of hearing afte…

Postauricular cutaneous mastoid fistula secondary to chronic suppurative otitis media (CSOM) is an unusual complication. Bilateral manifestation along with primary acquired cholesteatoma and atelectasis of the pars tensa as an end-stage complication in the natural course of atticoantral CSOM is rare. This kind of complication has a very morbid effect on the ear, and it poses a therapeutic challenge in terms of eradicating disease and restoring function. In this article, we describe the unusual course of an atticoantral CSOM that (1) began as a primary acquired cholesteatoma simultaneously in b…

OBJECTIVES: To present the effects of basic fibroblast growth factor on intractable cavity problems and discuss the mechanisms of its effects.
METHODS: We treated three ears with postoperative open cavities. All three cases had suffered from chronic discharge of the ear for 7, 10, and 30 years, respectively; 100 microg/ml of trafermin (genetic recombination) solution, as basic fibroblast growth factor, was dropped into the open cavity once daily. If bacterial and/or fungal infection was observed, antibiotics and/or antifungal agents were administered locally twice daily.
RESULTS: The cavitie

A 6-year review of complications of mastoid surgery between June 1995 and June 2001 revealed five cases with serious iatrogenic complications from mastoid surgery, of which four were facial nerve palsy and two were labyrinthine fistula. One of these patients had concomitant facial nerve palsy and labyrinthine fistula. There were two cases of complete facial nerve palsy (House Brackmann grade VI) and two cases of incomplete palsy (House Brackmann grades IV and V). The second genu was the site of injury in three of the four cases. Of the four cases with facial nerve palsy, two patients had full …

Obliteration of old mastoids and wet middle ears with autologous abdominal fat seems to be a reliable technique to render chronically discharging mastoid cavities or open middle ears dry and closed. This paper is the third in the series and looks at the intermediate results at five years. Of the 16 patients (one bilateral) 94.1 per cent of the ears are still dry and uninfected with closed external meati. Recurrent cholesteatoma was found in two patients at implantation and removed.

Two-stage methods for reconstruction of congenital microtia have been widely utilised. To obtain a desirable auriculocephalic angle and provide a nutrient support to the constructed auricle, elevation of reconstructed ears using a costal cartilage graft, the anteriorly based mastoid fascial flap transfer and a skin graft was performed as the second operation for nine microtia patients. In this procedure, the mastoid fascial flap was used instead of the temporoparietal fascial flap. Following the elevation of the reconstructed ear the anteriorly based mastoid fascial flap was harvested. A carve…

A 34-year-old man presented with tinnitus and conductive hearing loss. CT demonstrated an expansile, air-containing cavity contiguous with mastoid air cells, narrowing the external auditory canal. This is a case of symptomatic pneumocele resulting in an air collection beneath external canal lining, possibly related to an abnormality in mastoid fusion.
