Fibrous dysplasia is an uncommon benign bone disorder of unknown origin. It rarely affects the temporal bone. We report a case of fibrous dysplasia of the temporal bone with cholesteatoma in the external auditory canal, the first complaint of which was left otorrhea. Fibrous dysplasia invaded the external ear canal, stenosis of which causes a cholesteatoma. Mastoidectomy, tympanoplasty, and meatoplasty were performed. We discuss the characteristics of this condition, its differential diagnosis and its treatment.

Ectopic meningiomas located within the external auditory canal without any intracranial connection are an extremely rare finding. A report is made of a 38-year-old woman presenting with a conductive hearing loss due to an aural polyp, which was subsequently diagnosed as a meningioma. Computer-aided tomography (CT) and magnetic resonance imaging (MRI) showed the tumour to be entirely extracranial. Complete tumour excision was achieved using a modified radical mastoidectomy approach.

25 serially sectioned temporal bones from Wittmaack's temporal bone collection (ENT Hospital, University of Hamburg) have been examined in which radical mastoidectomy for cholesteatoma has been performed. The microtopographical and pathohistological analysis of postoperative results shows that recurrent cholesteatoma and inflammation act together as to complications such as labyrinthitis, meningitis and brain abscess. The subepithelial layer of granulation tissues between the matrix of the cholesteatoma and the underlying bone causes bone destruction via osteitis.

Patients who have had middle-ear or mastoid surgery are at an increased risk of developing cerebrospinal fluid (CSF) otorrhoea. The CSF leak is usually from defects in the tegmen or posterior cranial fossa. We present a patient with CSF otorrhoea following a modified radical mastoidectomy seven years ago. There was an unusual communication between the internal auditory meatus (IAM) and the middle ear. Radiologic imaging like the MRI is useful in identifying the site of leak.

We prefer the intact canal wall technique for tympanoplasty with mastoidectomy, but use a canal wall down procedure in up to 25% of cases. Creating a round cavity with a large meatus is imperative if one is to obtain a trouble-free ear. The five types of canal wall down procedures are defined, as are our indications for using them. Results in 19 only hearing ears and 19 labyrinthine fistula cases are presented, along with 64 tympanoplasty cases.

We report a case of infection of a mastoid cavity after mastoidectomy had been performed for chronic otitis media with cholesteatoma. The infection was caused by Staphylococcus intermedius after a pet dog had licked the patient's ears. Bacterial strains from the dog's saliva and the otorrhea in the patient were confirmed to be identical by pulsed-field gel electrophoresis analysis. The possibility of an oral transmission route of S. intermedius from pets to humans should be noted.

Mastoiditis still occurs as a nonmeningeal complication of otitis media. Fever, postauricular swelling, tenderness of the ear pinna, severe otalgia and ear drainage are all typical clinical findings. Most commonly, the causative organisms are the same aerobic bacteria that are responsible for otitis media; however, anaerobic organisms must also be considered. Treatment for acute mastoiditis includes appropriate intravenous antibiotics and myringotomy for culture and drainage. Mastoidectomy is indicated if medical therapy fails.

Although the incidence of labyrinthine fistula appears to be similar in most reported series, the number of patients reported to have a positive response to the fistula test do not. This may be due to a lack of uniformity in the performance of the test. The method of the author, the direction of eye movement and its diagnostic significance are described. The occurrence and mechanism of severe sensorineural hearing loss in 5 out of 20 patients in whom the matrix was removed accidently from the fistula during mastoidectomy is discussed.

Subperiosteal abscess was the indication for surgery in every child with acute mastoiditis who underwent mastoidectomy at our institution since 1972. Nineteen such patients are discussed herein. Positive bacteriologic findings included Streptococcus pyogenes in three patients, Streptococcus pneumoniae in three patients, Mycobacterium tuberculosis in two patients, and enterococcus in the one patient with cholesteatoma. Cultures were more often positive for causative bacteria when taken directly from the abscess before or shortly after beginning antibiotic therapy.

Tuberculous otitis media, although uncommon today, is still a differential diagnosis of suppurative otitis media. Our patient's otitis progressed from a draining ear with hearing loss to include facial nerve paralysis and meningitis. Several surgical procedures including radical mastoidectomy were insufficient to halt progression of the disease. After culture of acid-fast bacilli from the aural drainage and the cerebrospinal fluid, antituberculous chemotherapy was started and the patient responded well to treatment.

Considering the midterm postoperative ABG alone, the absence of mastoidectomy was a favorable predictive factor. Regarding midterm ΔAC, primary surgery was a predictor of a good hearing outcome. Otitis media initially involves only the mucosa, but if unchecked, may eventually spread to bone, causing bony erosion and severe osteitis that can have life-threatening CNS complications. Placement of a PE tube generally is all that is necessary to eliminate middle ear fluid and restore normal hearing in serous otitis media and adhesive otitis media. Mastoidectomy is required in more advanced chronic ear disease; if disease is not too extensive, hearing may be restored by tympanoplasty.

Surgery remains the mainstay of cholesteatoma management. Through advancement in technique and technology, the available surgical approaches have expanded to include not only the traditional procedures, but also endoscopic procedures, canal wall reconstruction procedures, mastoid obliteration, and retrograde mastoidotomy. Selection of management technique will depend on disease characteristics, patient factors, and surgeon preference.

PURPOSE: Sigmoid sinus anomalies such as dehiscence or diverticula may present with pulsatile tinnitus (PT) and low-frequency hearing loss. Occasionally, these symptoms are severe, necessitating resurfacing of the affected area to restore a normal-appearing sinus wall. This study describes three cases wherein we managed PT attributed to sigmoid sinus anomalies using polymethylmethacrylate (PMMA) bone cement, a novel material.
METHODS: Three patients with PT without any history of illnesses initially underwent cortical mastoidectomy to expose the affected area and resurface the sinus wall. Sub…

A 55-year-old man presented to the otolaryngology department complaining of aural fullness in his left ear after an episode of probable otitis 3 months before. magnetic resonance imaging revealed a soft tissue mass within the mastoid cavity that had destroyed the posterior wall of the middle ear with no apparent middle ear or sinonasal origin. The patient underwent a left canal wall-up tympanomastoidectomy, and the pathology report confirmed an inverted papilloma. Inverted papillomas are uncommon benign epithelial tumors related to a high recurrence rate and high risk of secondary malignant tr…

Objective: To investigate the influences of preoperative secretions of the patients with chronic suppurative otitis media and/or middle ear cholesteatoma on the time to have dry ears after the operations. Method: The study enrolled 228 patients with chronic suppurative otitis media and/or middle ear cholesteatoma who received modified radical mastoidectomies and tympanoplasty. The patients were divided into two groups based on whether there were secreions in the surgical ear before the operations. The time to have dry ears after surgeries were compared between the two groups. Result: The time

Ossicular chain defects are commonly encountered in otologic practice. The search for the ideal ossicular psosthesis is an ongoing debate. In this article, we describe our experience with the use of autologous cartilage tympanoplasty. Septal floor cartilage is harvested via the Killian incision and remodelled to fit the various ossicular defects identified in the middle ear. We describe the procedure from harvesting of cartilage through to ossiculoplasty. Different scenarios are described, as encountered intra operatively. We have not experienced extrusion when using this material. The cartila
