Prominent osteolysis in the maxilla: case report of an odontogenic fibroma mimicking a cyst.

BACKGROUND: Odontogenic fibroma (OF) is a rare benign odontogenic tumor of ectomesenchymal origin, mostly affecting the tooth-bearing portions of the jaws in middle-aged patients. Whilst small lesions tend to be clinically asymptomatic, varying unspecific clinical symptoms occur with an increase in size and may mimic odontogenic or other maxillofacial bone tumors, cysts, or fibro-osseous lesions of the jaws. CASE PRESENTATION: A 31-year-old female patient presented with a hard, non-fluctuating protrusion in the vestibule of the upper right maxilla. It was visualized on cone beam computed tomo…

Polyetheretherketone (PEEK) Implant for the Reconstruction of Severe Destruction in the Maxilla: Case Report.

Polyetheretherketone (PEEK) implants are being increasingly used to reconstruct defects in the oral and maxillofacial region. This article reports a special case of a patient with major destruction in his maxilla due to a war injury. The resultant defect was reconstructed with a 3D-printed, patient-specific, PEEK implant, restoring acceptable function and aesthetics. The patient followed up for 13 months and showed no technical or biological complications, proving the reliability of this treatment option for recreating severe maxillofacial deformities, and benefiting from the advantage they of…

[Extensive polyostotic form of fibrous dysplasia with involvement of the right maxilla and mandible].

Fibrous dysplasia (FD) is classified as fibro-osseous and osteochondromatous changes in the new WHO classification. In FD, the regular bone is replaced by a mixture of disorganized and moderately calcified woven bone and connective tissue. A distinction is made between a monostotic and a polyostotic type. In the monostotic form, only one bone or region is affected, primarily the femur and the craniofacial skeleton. In the polyostotic form, several regions are affected. In the cranial region, FD leads to clinical symptoms such as asymmetries, tooth displacement, malocclusion, nasal obstruction,

Fibula Free Flap Reconstruction of the Maxilla Leading to Extracapsular Ankylosis of the Mandible.

After en bloc resection of the maxilla or mandible, surgeons may choose to replace the missing bone and soft tissue with a fibula free flap (FFF). One of the complications that may arise during the healing of an FFF is heterotopic ossification along the free flap pedicle. Heterotopic ossification is most often noted incidentally on postoperative radiographs and rarely creates a functional deficit. Subperiosteal dissection of the pedicle from the bone during the harvest of the FFF is believed to be the main contributing factor that leads to this formation of bone along the flap soft tissue. Pai…

Effects of different types of maxillary protraction on maxilla with finite element analysis.

Although it is well documented in animal research that an early exposure to general anesthetics during critical stages of synaptogenesis disturbs normal brain development ultimately leading to cognitive and affective impairments, it is less clear whether and how surgical interventions and/or underlying systemic inflammation impact the detrimental effects of general anesthetics. Some emerging evidence suggests that aseptic systemic inflammation preceding exposure to the commonly used general anesthetics worsens anesthesia-induced neuroapoptosis and activates inflammasome pathways while resultin

Surgical Ciliated Cyst of the Left Maxilla - A Case Report of Unusual Pathogenesis.

INTRODUCTION: Surgical ciliated cyst (SCC) is a type of true epithelial lined antral cysts, which mostly occurs in maxilla from the sinonasal mucosa and gland, comprising 20% of all oral cysts in Japan. It is also called as postoperative maxillary cyst or postoperative paranasal cyst. The etiology is believed to be post trauma or surgery in the region of maxillary antrum. PATIENT CONCERNS: Patient's concern is less evident in this cystic pathology, because this cyst even though an aggressive cyst, presents with fewer symptoms that can be uncomfortable to the patient or push them to seek medic…

The Alveolar Ridge Splitting Technique on Maxillae: A Biomechanical Human Cadaveric Investigation.

The alveolar ridge splitting technique (ARST) offers an alternative to classic ridge augmentation techniques for successful insertion of dental implants. However, the buccal lamella is at risk of fracturing during ARST distraction. To better understand the fracture mechanisms and displacement limits of the split lamella, this study conducted biomechanical tests on human cadaveric maxilla specimens having extremely atrophied alveolar ridges treated with ARST. A total of 12 standardized alveolar splits were prepared on the maxillae of 3 elderly female donors using an oscillating piezoelectric sa…

Potential neurovascular damage as a result of dental implant placement in the anterior maxilla.

Introduction A patient requested dental implant therapy to replace his missing upper left central incisor. Pre-operative cone beam computed tomography (CBCT) imaging revealed a complex neurovascular supply to the anterior maxilla.Discussion Imaging demonstrated accessory neurovascular canals around the nasopalatine foramen which directly communicate with canalis sinuosus. They are, therefore, most likely to carry branches of the anterior superior alveolar nerve and vessels. One of these canals was directly in the path of proposed dental implant placement. A review of the dental literature sugg…

Mucormycotic Osteomyelitis Involving the Maxilla: A Rare Case Report and Review of the Literature.

Osteomyelitis is an inflammatory process of bone and marrow contents. These changes in bone are primarily seen in soft tissue followed by calcified tissue. It is an opportunistic infection due to the complication of some other conditions rendering the host susceptible to disease. Consequences of this infection range from draining tract to malignant transformation. Various etiological factors are involved in origin of the disease; among them, fungal origin is rare. Specific feature in fungal osteomyelitis is the involvement of maxillary sinus with a complaint of sinusitis associated with diabet…

Sinus Disventilation and Atrophy of the Upper Maxilla: A Combined Surgical Approach Is Possible?

This study aims to assess the effectiveness and reliability of a new combined approach to treat the maxillary atrophy and chronic rhinosinusitis (CRS) and other rhino-sinusal pathologies. This 1-step procedure consisting functional endoscopic sinus surgery (FESS) and maxillary sinus-lift with simultaneous implant placement for patients with a severe reabsorbed maxillary bone and sinuses' disease. In case of disventilation and maxillary sinuses flogosis, the mucociliary clearance and physiological ventilation have to be aided, in order to make a safer and predictable sinus-lift surgery. This ne…

Bilateral molariform supernumerary teeth in the anterior maxilla: a report of two cases.

Supernumerary teeth are additional teeth besides the normal series and can be found in any region of the dental arch. Supernumerary teeth can be classified according to their form and locations. Early diagnosis and treatment of patients with supernumerary teeth may prevent or minimize complications. The treatment options depend on the type and position of the supernumerary tooth and its effect on the adjacent structures. Two male patients were referred to our clinic due to swelling in their maxillae and interrupted eruption of teeth. Upon radiological examination of the patients, impacted supe…

Surgical management in a rare case of basaloid squamous cell carcinoma of the maxilla.

Basaloid squamous cell carcinoma (BSCC) is a rare and aggressive version of squamous cell carcinoma (SCC) that preferentially occurs in the upper aerodigestive tract. Since the first description by Wain SL et al., in 1986, only 21 cases with BSCC in the nasal cavity or in the paranasal cavity have been reported in the English literature. We present a case of BSCC arising in a paranasal sinus, a 51-year-old male patient with four months history of right cheek swelling and unilateral nasal obstruction, who underwent an operation and postoperative radiotherapy. Clinical, pathological and surgical

Modifications of the deep circumflex iliac artery free flap for reconstruction of the maxilla.

BACKGROUND: The deep circumflex iliac artery (DCIA) free flap remains underused in maxillectomy reconstruction. A number of surgical techniques have been described however, maxillary defects vary greatly and modifying techniques to account for such variation can be challenging. PURPOSE: This article presents the first standardized approach to DCIA free flap modification for maxillary reconstruction where graded modifications are made to a standard procedure based on defect grade. A review of 11 cases that underwent maxillectomy reconstruction with this technique is presented. METHODS: Defect…

Assessment of migrated foreign bodies in the maxillae by x-ray fluorescence spectrometry.

Two cases of foreign bodies that were incidentally observed on orthopantomographs are reported in this study. The foreign bodies were analyzed using an x-ray fluorescence spectrometer to identify what the migrated foreign bodies were and what migration pathways they had taken. The removed metallic foreign bodies from the maxillae of 2 patients, who were referred to the Department of Dentistry and Oral Surgery of the University of Fukui Hospital, were analyzed using an x-ray fluorescence spectrometer in the Department of Dental and Biomedical Materials Science, Nagasaki University. The major co…

An unusual presentation of an intraosseous epidermoid cyst of the anterior maxilla: a case report.

INTRODUCTION: Intraosseous epidermoid inclusion cysts are rare benign epithelial inclusion cysts in the bone. They are usually found in the cranium and hand phalanges. They are slow growing lesions, and it is difficult to differentiate them from other inflammatory and cystic lesions. Only a few cases of epidermoid inclusion cyst in the jaw have been reported in the literature. This is the fourth case reported as intraosseous epidermoid cyst of the maxilla in the English literature. CASE PRESENTATION: An asymptomatic 59-year-old Caucasian man was referred to our Oral and Maxillofacial Surgery …

Comparative assessment of periapical radiography and CBCT imaging for radiodiagnostics in the posterior maxilla.

Close proximity of the maxillary roots and the sinus floor makes a dental disease a probable cause of maxillary sinusitis. The aim of this study was to evaluate the ability of periapical radiography and cone beam computed tomography in defining the topographic relationship of maxillary teeth to the sinus floor and detecting apical periodontitis and other odontogenic causes of the maxillary sinusitis. Out of 145 dental records from subjects (mean age 52 years, range 20-75 years; 89 females) referred to the Oral Imaging Centre, KU Leuven, periapical and CBCT images of the posterior maxilla were …

The effect of mandibular anterior teeth on the hypermobile tissue in the anterior part of the maxilla.

The absence of posterior occlusal contacts may result in increased anterior occlusal contacts and forces between natural mandibular dentition and artificial maxillary dentition. The impact of natural mandibular anterior teeth on the development of hypermobile tissues in the anterior part of the edentulous maxillary alveolar ridge was investigated in this study. The study group consisted of 410 patients with maxillary complete dentures and various mandibular dentitions from seven rest homes in Istanbul. The data; including hypermobile tissue in the anterior part of the maxilla, occlusal relatio…
