Speech results after bilateral osteotomy surgery for cleft palate: a review of 413 patients.

Patients records of those who had received the bilateral osteotomy surgical procedure for cleft palate were studied. There were 413 records available for review. Patients evaluated by a speech/language pathologist numbered 226. The other 137 patients were evaluated either by a school speech/language pathologist or by the plastic surgeon. An additional 50 records could not be used. Patients were 18 to 24 months through 35 years of age. The results revealed that 81.5 percent of the patients demonstrated normal or near normal resonance quality. Two additional positive aspects of the procedure are

Three-dimensional voxel-based evaluation of surgical accuracy and one-year stability after segmented Le Fort I osteotomy for anterior open bite correction.

This two-center retrospective study evaluated the surgical accuracy and one-year stability of the anterior maxillary segment after three-piece segmented Le Fort I osteotomy for anterior open bite correction using a validated voxel-based three-dimensional assessment protocol. Patients treated between January 2016 and December 2021 were included. The anterior maxillary segment was assessed in three translational (left/right, anterior/posterior, intrusion/extrusion) and three rotational (roll, pitch, yaw) dimensions by comparing the virtual surgical plan with the early postoperative position and

Major vascular complications of orthognathic surgery: false aneurysms and arteriovenous fistulas following orthognathic surgery.

False aneurysms and arteriovenous fistulas are rare complications of orthognathic surgery. The vessel most commonly involved with false aneurysms following mandibular surgery is the internal maxillary artery, and this vessel, especially the sphenopalatine branch, may also be involved following maxillary surgery. An unusual factor in the presentation of false aneurysms following Le Fort I osteotomies is an initial episode of epistaxis occurring greater than 2 weeks postoperatively. Arteriovenous fistulas following orthognathic surgery are more apt to involve large vessels, especially the intern

Trans-sinusal maxillary distraction for correction of midfacial hypoplasia: long-term clinical results.

Maxillary distraction osteogenesis is indicated in severe angle class III malocclusions, and severe maxillary hypoplasia among some cleft patients and other craniofacial deformities. Twenty patients, aged 8-48 years (mean 17.8+/-10.5 SD) with maxillary and midfacial hypoplasia were treated. The follow-up period was 13-65 months (mean 35+/-16.3 SD). A trans-sinusal maxillary distractor was placed intraorally at each side of the maxilla. The distraction vector was predicted using specialist software, and was transferred to the patients using stereolithographic models and individual templates. A

Nasoantral windows: an experimental study in rabbits.

An experimental study was designed to compare the effects of enlargement of the maxillary ostium to inferior meatal antrostomy. In 10 rabbits, the sinus ostium was enlarged (osteoplasty group), and in 10 other animals, a window of the same size was created far from the ostium (antrostomy group). The control groups were 7 rabbits in which no surgery was performed and 6 rabbits in which only the surgical approach was performed. At reexploration 14 and 84 days after surgery, it was observed that rabbits in the osteoplasty group had significantly more infections of the sinuses than the antrostomy

Late management of secondarily grafted clefts.

34 patients (40 sides) received alveolo-palatal bone grafts for closure of the residual cleft, thus guiding a lateral incisor or canine into the arch. Long-term follow-up shows that in 41% of the patients uninterrupted arches were achieved with a normal relationship by orthodontic treatment only. 38% needed segmental osteotomies to eliminate the edentulous space, and in only 20% were bridges made to restore the dental arch. 9 (25%) patients still required a Le Fort I advancement osteotomy, despite optimal orthodontic treatment. The use of segmental osteotomies for eliminating edentulous spaces

The influence of tissue redraping after Le Fort I type osteotomy.

Elongation of the upper lip, in particular the philtrum, and nose widening are common postoperative changes after a Le Fort I osteotomy. These changes can be induced by the transection of soft tissue and loosening of the underlying musculature. A methodology for soft tissue redraping was developed to limit these undesirable nasolabial changes. This study evaluates the effectiveness of the technique and influence of maxillary translocation on the nasolabial form. Anthropometric measurements, lip, philtrum length, and nose width, were taken two weeks prior to, and one year after, surgery. The me

Intraoperative ultrasonography for the reposition of the condylar process during bilateral sagittal osteotomy of the mandible-A case report.

INTRODUCTION: Ultrasonography (USG), being a safe, quick and relatively cheap examination, is used to diagnose various pathologic conditions. The application of ultrasound to assess the position of the condyle during bilateral sagittal split osteotomy (BSSO) may improve the treatment results. CASE PRESENTATION: A 33-year-old patient, who was operated for a skeletal defect of maxilla and mandible with BSSO and Le Fort I maxillary osteotomy is discussed in this case report. The procedure was complicated with a mandibular head dislocation. The split segment was repositioned under ultrasound guid…

Mid-facial sensation following craniofacial surgery.

During mid-face advancement and correction of orbital dystopia, the infraorbital nerves are always stretched and displaced by the orbital floor osteotomies and bone displacement, and are at risk of damage. We have found no published work that has investigated the function of the infraorbital nerves following elective craniofacial procedures. In this study the cheek sensation and tooth sensibility in 20 patients who have had either mid-face advancement or correction of orbital dystopia was assessed. The function of the infraorbital nerve is largely undisturbed by the osteotomies and bone displa

Factors predictive of pterygoid process fractures after pterygomaxillary separation without using an osteotome in Le Fort I osteotomy.

OBJECTIVE: This study demonstrated pterygomaxillary disjunction patterns and elucidated factors related to unfavorable pterygomaxillary junction fractures in Le Fort I osteotomy without using an osteotome. STUDY DESIGN: Clinical and anatomical data obtained from computed tomography images (100 sides) were analyzed for their ability to predict patterns of pterygomaxillary disjunction. RESULTS: Separation of the pterygomaxillary junction was most frequently performed at the maxillary tuberosity (48.0%). Twenty-eight pterygoid plates were fractured. Male gender, increased age, thickness of the

Maxillary-midface distraction in children with cleft lip and palate: a preliminary report.

A miniature system of distraction devices has been employed for maxillary-midface advancement in two children with cleft lip and palate, class III malocclusion, and associated midfacial hypoplasia. The devices are made with commercially available palatal expansion screws linked to rigid fixation plates. A midfacial osteotomy is used, and distraction is begun on the third postoperative day. In the first child, a 7-year-old boy, the midface was distracted 11 mm sagittally and 4 mm inferiorly. In the second patient, a 4 1/2-year-old girl with unilateral cleft lip and palate and midfacial retrusio

Do Patients With Cleft Lip and/or Cleft Palate Require More Postoperative Opioids Following Le Fort I Osteotomy?

BACKGROUND: Le Fort I (LFI) osteotomies in patients with cleft lip and/or palate (CL/P) present unique challenges. It is unclear if these patients require distinct postoperative pain management. PURPOSE: The purpose of this study was to measure and compare postoperative opioid consumption between patients with and without CL/P undergoing LFI osteotomies. STUDY DESIGN, SETTING, AND SAMPLE: This retrospective cohort study included patients who underwent LFI osteotomies at Boston Children's Hospital (2015 to 2024). Patients with chronic pain, psychiatric disorder, or developmental delay were ex

Is skeletal stability after bimaxillary surgery for skeletal class III deformity related to surgical occlusal contact?

A stable occlusion at the time of surgery is considered important for post-surgical stability after orthognathic surgery. The aim of this study was to determine whether skeletal stability after bimaxillary surgery using a surgery-first approach for skeletal class III deformity is related to the surgical occlusal contact or surgical change. Forty-two adult patients with a skeletal class III deformity corrected by Le Fort I osteotomy and bilateral sagittal split osteotomy with a surgery-first approach were studied. Dental models were set and used to measure the surgical occlusal contact, includi

The use of a bite fork to help mobilize the maxilla after downfracture during the Le Fort I osteotomy. A technical note.

INTRODUCTION: The Le Fort I osteotomy is indicated to reposition the maxilla in the surgical correction of dentofacial deformities. Although it is a known surgical step, like any surgical procedure, it can have complications and difficulties; among these, the initial mobility of the jaw is very common after completing all corticotomies. METHODS: The authors present a technique in which the bite fork is used as an auxiliary tool in maxilla downfracture, especially in cases of maxillary impaction where vertical space is created on the walls of the corticotomy. FINDINGS: As the bite fork acts a
