Normalization of maxillary sinus mucosa after functional endoscopic sinus surgery in pediatric chronic sinusitis.

OBJECTIVES: To observe the recovery of maxillary sinus mucosa after endoscopic sinus surgery in children with chronic maxillary sinusitis. Functional endoscopic sinus surgery (FESS) was used to perform a middle meatal antrostomy, leaving the antral mucosa intact for later observation. METHODS: Between January 1998 and December 2003, 43 children with chronic sinusitis were enrolled in the study. Pre-operative patient profiles, including a history of symptoms, signs and allergies, were collected. Phidiatap test was used to check allergy. Saccharine transit time tests were performed for each sid…

Correlation of plain radiological diagnostic features with antral lavage results in chronic maxillary sinusitis.

To justify the continued relevance of plain radiography of maxillary sinus in establishing diagnosis and planning treatment of chronic maxillary sinusitis, its specificity and predictive values must have a high degree of confidence. The objective of this study was to examine different plain radiographic features and correlate them with antral lavage results in patients suffering from chronic maxillary sinusitis. Air-fluid level and antral opacity have specificity of 92.3% each and positive predictive value of 87.5% and 96.0% respectively. Gross mucosal thickening and haziness showed low specif…

Immune response to Fusobacterium nucleatum and Prevotella intermedia in patients with chronic maxillary sinusitis.

Our objective was to study the immune response to 2 anaerobic bacteria in patients with chronic maxillary sinusitis. Immunoglobulin G class antibody titers to Fusobacterium nucleatum and Prevotella intermedia were measured by enzyme-linked immunosorbent assay. Serum levels in the patients were determined on the day sinus aspiration for culture was done, and 75 to 90 days later, after completion of antimicrobial therapy, Prevotella intermedia was isolated from 16 aspirates, and F. nucleatum from 12. Median antibody levels at day 1 for F. nucleatum and P. intermedia were significantly higher in …

Ostial and nasal patency in chronic maxillary sinusitis. A long-term post-treatment study.

In a long-term follow-up study (mean 3.5 years) after treatment of chronic maxillary sinusitis of either rhinogenous or dental origin, 72 sinuses in 66 patients were examined with respect to maxillo-ostial and nasal resistance. In sinusitis of rhinogenous etiology, the mean equivalent ostial diameter was pathologically narrowed (0.89 mm, in patients treated conservatively) whereas it was normal (2.51 mm) in sinusitis of dental etiology (dental treatment in combination with local sinus surgery). In the former group, all sinuses were diseased, compared with 3 out of 23 in the dental group. In or…

A case report of acute pyelonephritis secondary to maxillary sinusitis caused by implant displacement.

BACKGROUND: With the advancement of dental implant technology, implant placement in the maxillary posterior region has become a primary method for restoring missing teeth. However, due to factors such as significant maxillary sinus pneumatization and insufficient alveolar bone volume, implant displacement into the maxillary sinus occasionally occurs. Retained implants in the maxillary sinus may induce maxillary sinusitis and secondary systemic diseases such as pyelonephritis. The relevance of which has not yet been fully recognized and thoroughly studied. CASE PRESENTATION: A 47-year-old male…

Nonsurgical endodontic management of maxillary sinusitis of endodontic origin: a CBCT-guided case report.

BACKGROUND: Maxillary sinusitis of endodontic origin (MSEO) is a recognized complication of odontogenic infection, arising from the close anatomical relationship between the posterior maxillary dentition and the maxillary sinus floor. Failure to identify the odontogenic etiology of sinus pathology risks misdiagnosis, delayed treatment, and unnecessary surgical intervention. This case report presents the diagnostic approach and clinical management of a maxillary second molar associated with sinus mucosal thickening, emphasizing the role of advanced imaging and conservative endodontic therapy in…

Impact of Zinc Oxide on the Development of Aspergillus-Induced Maxillary Sinusitis Rabbit Model.

Aspergillus fumigatus is commonly found in the airway and is associated with airway inflammatory diseases. Zinc oxide (ZO) is known to be an essential microelement that facilitates fungal survival, growth, and proliferation. This study aimed to investigate the impact of ZO on A. fumigatus-induced fungal sinusitis in rabbits. Twenty-eight New Zealand white rabbits were divided into four groups for this study. Group 1 (6 sides) was treated with intramaxillary phosphate buffer saline (PBS) served as the negative control, Group 2 (6 sides) received intramaxillary PBS and ZO, Group 3 (8 sides) was

Does Middle Meatal Antrostomy Prevent the Onset of Maxillary Sinusitis After Zygomatic Implant Placement?

PURPOSE: To evaluate the role of middle meatal antrostomy in preventing the onset of maxillary sinusitis after placement of zygomatic implants. We hypothesized that middle meatal antrostomy might reduce the incidence of this complication. MATERIALS AND METHODS: A prospective longitudinal study was performed of patients who had undergone placement of 4 zygomatic implants and concurrent middle meatal antrostomy with an endoscopic approach at the maxillofacial surgery unit of Verona University. The radiological results of the maxillary sinuses were evaluated on computed tomography scans performe…

Odontogenic sinusitis is a common cause of unilateral sinus disease with maxillary sinus opacification.

BACKGROUND: Unilateral sinus disease (USD) carries a broader differential diagnosis than bilateral sinus disease, including various inflammatory and neoplastic conditions. Odontogenic sinusitis (ODS) is a common cause of unilateral maxillary sinusitis, but few studies have examined its incidence among all USD etiologies. The main purposes of this prospective study were to determine the incidences of ODS and other etiologies causing USD with complete maxillary sinus opacification on computed tomography (CT), and to compare CT features of ODS and non-odontogenic USD patients. In addition, clinic…

Bacterial colonization and granulocyte activation in chronic maxillary sinusitis in asthmatics and non-asthmatics.

The impact of bacterial colonization on the severity and pattern of chronic inflammation in rhinosinusitis is not clear. In this study, it was hypothesized that bacterial colonization of the sinus mucosa would have a greater impact on inflammatory response modulation in asthmatic patients than in non-asthmatic patients with chronic rhinosinusitis. In order to test this hypothesis, granulocyte activation was measured and related to bacteria identified in the sinus lavage. Lavages from the maxillary sinuses of 21 asthmatic and 19 non-asthmatic patients with chronic rhinosinusitis (CRS) were micr

Primary-care-based randomised placebo-controlled trial of antibiotic treatment in acute maxillary sinusitis.

BACKGROUND: The value of antibiotics in acute rhinosinusitis is uncertain. Although maxillary sinusitis is commonly diagnosed and treated in general practice, no effectiveness studies have been done on unselected primary-care patients. We used a randomised, placebo-controlled design to test the hypothesis that there would be an improvement associated with amoxycillin treatment for acute maxillary sinusitis patients presenting to general practice. METHODS: Adult patients with suspected acute maxillary sinusitis were referred by general practitioners for radiographs of the maxillary sinus. Thos…
