PURPOSE: The purpose of this study was to perform an evidence-based, expert consensus survey using the Delphi panel methodology to develop recommendations for the treatment of degenerative meniscus tears.
METHODS: Twenty panel members were asked to respond to 10 open-ended questions in rounds 1 and 2. The results of the first 2 rounds served to develop a Likert-style questionnaire for round 3. In round 4, the panel members outside consensus were contacted and asked to either change their score in view of the group's response or argue their case. The level of agreement for round 4 was defined …

PURPOSE: Anterior cruciate ligament (ACL) reconstruction can reduce the risk of developing osteoarthritic knees. The goals of ACL reconstruction are to restore knee stability and reduce post-traumatic meniscal tears and cartilage degradation. A chronic ACL insufficiency frequently results in medial meniscus (MM) injury at the posterior segment. How ACL reconstruction can reduce the deformation of the MM posterior segment remains unclear. In this study, we evaluated the form of the MM posterior segment and anterior tibial translation before and after ACL reconstruction using open magnetic reson…

BACKGROUND: Proper functioning of the meniscus depends on the composition and organization of its fibrocartilaginous extracellular matrix. We previously demonstrated that the avascular inner meniscus has a more chondrocytic phenotype compared with the outer meniscus. Inhibition of the Rho family GTPase ROCK, the major regulator of the actin cytoskeleton, stimulates the chondrogenic transcription factor Sry-type HMG box (SOX) 9-dependent α1(II) collagen (COL2A1) expression in inner meniscus cells. However, the crosstalk between ROCK inhibition, SOX9, and other transcription modulators on COL2A1…

BACKGROUND: Discoid lateral meniscus of the knee is common in Chinese population. There has been considerable debate about the best treatment for discoid lateral meniscus tears. The purpose of this study was to observe the effect of arthroscopic meniscectomy for the treatment of discoid lateral meniscus tears.
METHODS: Between July 1999 and December 2004, arthroscopic meniscectomy was performed on 62 menisci of 57 patients with discoid lateral meniscus tears (52 unilateral, 5 bilateral). 41 menisci were "complete discoid meniscus", 21 "incomplete". According to the extent of the meniscus tear…

We retrospectively reviewed the long-term results of total meniscectomy performed in seventeen knees (fourteen children) to treat a discoid lateral meniscus. The mean duration of follow-up was 19.8 years (range, 12.5 to 26.0 years). On the basis of the rating system of the International Knee Documentation Committee, seven knees were normal (grade A), six were nearly normal (grade B), three were abnormal (grade C), and one was severely abnormal (grade D) at the latest follow-up evaluation. Ten of the seventeen knees had clinical symptoms of osteoarthrosis. Radiographs were available for fifteen…

The hypermobile lateral meniscus is an uncommon cause of posterolateral knee pain and mechanical symptoms that often escapes standard imaging and is best confirmed by arthroscopy. A clinical stress maneuver combining knee flexion, valgus stress, and external rotation is described in this Technical Note as a simple adjunct to reproduce symptoms related to posterior horn instability and support early suspicion of hypermobile lateral meniscus. This Technical Note also describes a minimally invasive arthroscopic stabilization technique using sequential all-inside vertical sutures. The procedure be…

Recent studies have highlighted the role of micrometer-scale, three-phase interfacesbetween an electrode, an electrolyte, and a gas phasein confining and activating chemical reactions. Here, we combine scanning electrochemical cell microscopy (SECCM) to create a localized air|water|indium tin oxide (ITO) meniscus and interference reflection microscopy (IRM) to image nanoscale electrochemical processes occurring in its vicinity. This approach reveals that, under the conditions used here (20 μm diameter KCl electrolyte meniscus, ca. 40-60% relative humidity, ITO partial wetting properties, and…

Meniscus repair is often limited by poor intrinsic healing, an adverse oxidative-inflammatory microenvironment, and insufficient mechanical continuity across the tear interface. Here, we engineered a solvent-free injectable polyurethane adhesive (PUA) loaded with connective tissue growth factor (PUA@CTGF) for meniscus tear repair that combines rapid wet interfacial stabilization, compliant defect filling, and prolonged CTGF release. The adhesive showed rapid in situ curing, high apparent initial wet lap-shear strength, and stable short-cycle compressive behavior in a 100-cycle loading-unloadin…

Medial meniscus posterior root tears are debilitating injuries that can lead to the rapid progression of osteoarthritis (OA) if left untreated. Medial meniscus root tears commonly occur in older patients (≥55 years old) and are usually degenerative-type tears, rather than acute traumatic injuries (like those most commonly seen with lateral meniscal root tears). These degenerative-type tears are functionally equivalent to a total medial meniscectomy. Repair is becoming the gold standard for treatment, but laxity to probing indicates a nonfunctional repair, and progression of OA can still occur.…

BACKGROUND: The menisci are critical for knee stability and cushioning and tears of the menisci have been linked to the acceleration of osteoarthritis. Horizontal cleavage tears split the meniscus into a top and bottom portion and are notoriously difficult to repair due to their localization in the avascular zone of the meniscus. Repair augmentation techniques, including fibrin clot and bone marrow venting, have been reported to increase meniscus healing.
INDICATIONS: The use of a fibrin clot is indicated when there is a complex horizontal cleavage tear of the meniscus. There must be a clear …

Meniscal tears are the most common and most treated injuries of the knee. In the recent past, meniscectomy was considered the treatment of choice for meniscal lesions. However, this practice is progressively being discontinued in favor of meniscal repair, which has shown high success rates. Several techniques are currently used individually or in association to treat bucket-handle meniscal tears. The inside-out technique has been considered the gold standard for this kind of lesion, especially if the posterior horn of the meniscus is involved. This article aims to describe a reproducible surgi…

Medial meniscus injury is one of the most prevalent knee disorders, with posterior root tears occurring in approximately 10% to 21% of affected individuals. A posterior root tear disrupts the meniscus's continuous annular structure, compromising its ability to absorb pressure and protect the knee's articular cartilage. If left untreated, this can result in increased stress on the cartilage, leading to conditions such as varus deformity and accelerated joint degeneration. Partial removal (resection) of the medial meniscus further exacerbates these issues, often leading to quicker joint deterior…
