Delayed union of fractures of the trapezoid and body of the hamate are rare and difficult to treat. The fracture fragments may include the neighboring metacarpals with the insertions of the wrist extensors. These two cases occurred after direct blows to the bases of the 2nd and 5th metacarpals, respectively. Fractures in the hand, as elsewhere, usually fall into identifiable patterns. With prompt diagnosis and appropriate management, most fractures can be treated satisfactorily by closed means. However, a small percentage of fractures in the hand do require treatment by open means. Early recognition and early treatment of these injuries will usually avoid significant complications.

Previously described radiological signs of Turner's syndrome were evaluated in X-rays of the left hand from 17 patients with Turner's syndrome (age 10.4--15.3 years) and 17 age-matched girls with constitutional short stature. A case was presented in which the late reconstruction of left ring and little metacarpal bone defects was carried out using a lengthening device and a double-barrel osteocutaneous free parascapular flap. With this flap, up to 14 cm of nearly straight bone of a suitable width and length for the reconstruction of two metacarpal bones was raised safely. However, it required 3 months of lengthening to create enough space to insert the grafted bone because of the severe resistance of scar contracture and fibrous tissues.

A new vascularized bone graft from the base of the second metacarpal was used to treat scaphoid non-union and Kienböck's disease. In two patients with scaphoid non-union, the procedure promoted healing, even when the proximal segment was poorly vascularized. In one patient with Kienböck's disease, the graft held stable and wrist pain was markedly reduced. Further clinical experience may establish this procedure as an option for carpal bone fractures or disease.
