We have investigated the influence of antithyroid drugs (methimazole and propylthiouracil) and sodium iodide on the expression of major histocompatibility (MHC) class II antigen expression in human and rat thyroid cells. While methimazole and propylthiouracil significantly inhibited lectin-induced MHC class II (HLA-DR) antigen in crude human thyroid monolayer preparations these drugs had no influence on gamma-interferon induction of class II antigens in similar cell preparations. A case of triiodothyronine (T3) toxicosis associated with hypokalemic periodic paralysis is reported. Thyrotoxic manifestation was minimal except for mild struma. Serum T3 levels were moderately elevated, though other thyroid function tests were within the normal range. The patient was treated with methimazole, and paralytic attacks ceased. This is the first report of this combination and the necessity of careful thyroid function tests in sporadic periodic paralysis is emphasized.

The antithyroid drugs mainly include thioimidazole (carbimazole, methimazole=thiamazole) and propylthiouracil. After absorption, carbimazole is rapidly metabolized to methimazole and thus switching between these two drugs should not be considered in case of side effects. This study scrutinizes the correlation between serum free triiodothyronine (FT3) to free thyroxine (FT4) ratios and the eventual outcome of antithyroid drug (ATD) therapy in patients with Graves' disease.
