Left Ventricular Diastolic Function: Comparison of Slow Coronary Flow Phenomenon and Left Ventricular Hypertrophy in the Absence of Obstructive Coronary Disease.

Background An interplay of myocardial structural abnormalities and coronary arterial dysfunction underlies the worsening left ventricular co … eft ventricular mass (146.9 ± 17.7 g/m2 vs. 126.1 ± 121.5 g/m2, p=0.027). Conclusions Patients with microvascular angina represent a diverse population. The echocardiographic parameters of left ventricular relaxation (e') and end-diastolic pressure (E/e') are abnormally altered in the population with left ventricular hypertrophy compared to SCFP. The delayed epicardia …

Microvascular Resistance Reserve vs Coronary Flow Reserve to Assess Age-Related Trends in Coronary Microvascular Dysfunction.

BACKGROUND: Advancing age is associated with epicardial atherosclerosis and coronary microvascular dysfunction (CMD), complicating reliable assessment of CMD using coronary flow reserve (CFR). Whether prevalence of functional and structural CMD varies with age remains unclear. OBJECTIVES: The authors sought to evaluate the prevalence of CMD endotypes by age strata and compare CFR with microvascular resistance reserve (MRR) for diagnosis and stratification. METHODS: Data from 1,704 patients (2,283 lesions) with stable angina in the ILIAS Registry (Inclusive Invasive Physiological Assessment i

Impact of Abnormal Coronary Reactivity on Long-Term Clinical Outcomes in Women.

BACKGROUND: Currently as many as one-half of women with suspected myocardial ischemia have no obstructive coronary artery disease (CAD), and abnormal coronary reactivity (CR) is commonly found. OBJECTIVES: The authors prospectively investigated CR and longer-term adverse cardiovascular outcomes in women with and with no obstructive CAD in the National Heart, Lung, and Blood Institute-sponsored WISE (Women's Ischemia Syndrome Evaluation) study. METHODS: Women (n = 224) with signs and symptoms of ischemia underwent CR testing. Coronary flow reserve and coronary blood flow were obtained to test

Coronary Computed Tomography (CT) Angiography as a Predictor of Cardiac and Noncardiac Vascular Events in Asymptomatic Type 2 Diabetics: A 7-Year Population-Based Cohort Study.

BACKGROUND: Type 2 diabetics are at increased risk for vascular events, but the value of further risk stratification for coronary heart disease (CHD) in asymptomatic subjects is unclear. We examined the added value of coronary computed tomography angiography over clinical risk scores (United Kingdom Prospective Diabetes Study), and coronary artery calcium in a population-based cohort of asymptomatic type 2 diabetics. METHODS AND RESULTS: Subjects (n=630) underwent baseline clinical assessment and computed tomography angiography (64-slice scanner). Plaque site, volume, calcific content, and ar

The role of plaque fissures in unstable angina: fact or fiction?

Several factors may contribute to the development of unstable angina. These include underlying atherosclerosis, mural platelet thrombosis, dynamic stenosis, occlusive spasm and microvascular dysfunctions. Of these, thrombosis is the most visible event, but its actual causes remain elusive. Plaque fissure is widely assumed to be the cause of unstable angina. However, studies have shown no evidence of plaque fissure in about 40% of patients dying from acute coronary syndromes and, conversely, fissured plaques have been found in 10-25% of individuals dying of non-cardiac causes. An inflammatory p

Coronary sinus narrowing therapy: A "Reducer" for angina and beyond.

Refractory Angina (RA) is an increasingly common clinical diagnosis, in which patients unsuitable for further percutaneous or surgical procedures experience anginal symptoms, despite receiving optimal medical therapy. This clinical condition challenges the everyday activities and diminishes the quality of life of these patients. A wide variety of novel therapies for this type of angina are being investigated for clinical use. One of them is coronary sinus narrowing, which is performed as a percutaneous interventional procedure using catheter-delivered device, the Reducer. The device is implant

Coronary Microvascular Dysfunction: Pathophysiology, Clinical Phenotypes, and Mechanism-Guided Diagnostic Strategies.

Coronary microvascular dysfunction (CMD) is a prevalent and clinically important cause of myocardial ischemia across the spectrum of coronary artery disease, including patients with angina and nonobstructive coronary arteries, persistent symptoms after revascularization, and acute coronary syndromes. Despite its prognostic significance, CMD remains underrecognized due to the limitations of angiography-centered evaluation and the heterogeneity of underlying biological mechanisms. This review presents a physiology-based framework for the assessment of CMD, emphasizing the role of invasive corona

Predictive ability of microvascular resistance reserve for periprocedural myocardial injury in patients with stable coronary artery disease undergoing percutaneous coronary intervention.

Periprocedural myocardial injury (PMI) has shown an association with worse prognosis after percutaneous coronary intervention (PCI). The microvascular resistance reserve (MRR) has been introduced as an index to evaluate microvascular vasodilatory capacity, independent of epicardial disease. Given the unclear association between MRR and PMI, this study aimed to determine the relationship between PMI and coronary microvascular dysfunction, including index of microcirculatory resistance (IMR) and MRR. This study identified 68 patients with stable angina pectoris who underwent PCI after measuremen

Association of monocyte-to-lymphocyte ratio with coronary endothelial dysfunction and cardiovascular events in patients with angina and nonobstructive coronary artery disease.

BACKGROUND: Coronary endothelial dysfunction (CED) is regarded as an early stage of atherosclerosis. Inflammation plays a key role in the mechanism and progression of atherosclerosis, but its clinical utility in the initial phase is less clear. METHODS: Patients with angina and nonobstructive coronary artery disease who underwent coronary reactivity testing at the Mayo Clinic between 1997 and 2021 were enrolled. Endothelium-dependent microvascular dysfunction was a <50 % change in coronary blood flow, and endothelium-dependent epicardial dysfunction was a < -20 % change in epicardial arterial

Microvascular ischemia in patients with successful percutaneous coronary intervention: effects of ranolazine and isosorbide-5-mononitrate.

OBJECTIVE: About one-third of patients undergoing percutaneous coronary interventions (PCIs) for flow-limiting coronary stenosis continue to develop signs of myocardial ischemia (MI) during exercise stress test [EST], despite successful coronary revascularization. Coronary microvascular dysfunction is a likely major cause of the persistence of EST-induced MI in these patients. PATIENTS AND METHODS: We studied 15 patients (14 men, age 67±5 years) fulfilling the following strict inclusion criteria: (1) recent PCI (<6 months), with drug-eluting stent, of coronary artery stenoses for stable angin

Magnetocardiography in the diagnosis of suspected ANOCA: a case series.

Patients with angina pectoris with non-obstructive coronary artery disease (ANOCA) often present to cardiology outpatient clinic or emergency departments with manifestations mimicking acute coronary syndrome (ACS). As a working diagnosis defined by angina pectoris in the absence of obstructive coronary arteries, ANOCA requires a stepwise diagnostic process-one that incorporates coronary functional assessments and provocative tests. Magnetocardiography (MCG) is a diagnostic modality with the potential to distinguish mild myocardial ischemic in patients with suspected ANOCA from nonischemic card

Coronary microcirculatory dysfunction after percutaneous coronary intervention: pathogenesis, diagnosis, and therapeutic strategies.

Coronary microvascular dysfunction (CMD) can lead to a variety of severe adverse cardiovascular events. CMD represents the primary cause of recurrent angina pectoris following percutaneous coronary intervention (PCI). The etiology of post-PCI CMD is complex and largely occult, which significantly impairs the therapeutic efficacy of PCI. This article reviews the physiological functions of the coronary microcirculation, as well as the latest research progress on the pathogenesis, diagnosis, and treatment of CMD after PCI. Finally, it highlights the scientific issues that urgently need to be addr

The effect of nicorandil in patients with cardiac syndrome X: A meta-analysis of randomized controlled trials.

BACKGROUND: The prevalence of cardiac syndrome X (CSX) is considerable. Some patients show recurrent angina attacks and have a poor prognosis. However, the knowledge of CSX pathophysiological mechanism is still limited, and the treatment fails to achieve a satisfactory suppression of symptoms. Nicorandil has a beneficial effect on improving coronary microvascular dysfunction (CMD). This study aims to evaluate the clinical effects and safety of nicorandil on CSX patients. METHODS: The Cochrane Library, Pubmed, EMBASE, ClinicalTrials.gov and 4 Chinese databases were searched to identify relevan

Investigation of MTHFR gene C677T polymorphism in cardiac syndrome X patients.

BACKGROUND: Definition of Cardiac Syndrome X (CSX) refers to groups of patients with positive exercise stress test and normal epicardial coronary arteries on coronary angiography accompanied by chest pain. Although the etiology of CSX is not completely understood, there is a common consensus that its pathophysiology may be associated with endothelial dysfunction resulting in impaired coronary flow. Some polymorphisms observed on the MTHFR gene cause inactivation of the MTHFR enzyme, leading to hyperhomocysteinemia and homocysteinuria, which are prominent risk factors of cardiovascular and cere

Basis for sex-specific expression of Takotsubo cardiomyopathy, cardiac syndrome X, and spontaneous coronary artery dissection.

Takotsubo cardiomyopathy, cardiac syndrome X, and spontaneous coronary artery dissection are cardiovascular syndromes with a predilection for women. A complex interplay between neurohormonal factors, genetic influences, anatomic alterations, and other factors together affect cardiovascular function. Specifically, a high, variable, or deficient estrogen state leads to vasomotor instability with propensity toward vasoconstriction and endothelial dysfunction that predispose women to myocardial impairment, microvascular dysfunction, and coronary arterial wall instability. As the predominant sex ho

Relation between stress-induced myocardial perfusion defects on cardiovascular magnetic resonance and coronary microvascular dysfunction in patients with cardiac syndrome X.

OBJECTIVES: The purpose of this study was to investigate whether a direct relation can be demonstrated between myocardial perfusion defects detected during dobutamine stress test (DST) by cardiovascular magnetic resonance (CMR) and impairment of coronary microvascular dilatory function in patients with cardiac syndrome X (CSX). BACKGROUND: Despite the fact that coronary microvascular dysfunction has been shown in most patients with CSX, the ischemic origin of CSX remains debated. No previous study assessed whether a strict relation exists between abnormalities in myocardial perfusion and coro
