A prospective cohort study of late sequelae of pediatric allogeneic hematopoietic stem cell transplantation.

As survivors of pediatric allogeneic hematopoietic stem cell transplantations (HSCTs) increase in number, it is increasingly important to evaluate their well-being. We conducted this prospective cohort study to evaluate the cumulative incidence and risk factors for late sequelae of HSCT. Comprehensive surveillance tests were performed annually on every participant, regardless of signs and symptoms, to obtain accurate information on the time-of-onset of each late event to allow hazard function analyses. All participants included in this report had been followed for at least 3 years after HSCT.

Optimizing treatment approaches in seborrheic dermatitis.

Seborrheic dermatitis is a chronic, recurring, cutaneous condition that causes erythema and flaking, sometimes appearing as macules or plaques with dry white or moist oily scales. In adults, it commonly occurs in areas with high concentrations of sebaceous glands. The face and scalp are the most frequently affected areas, and involvement of multiple sites is common. Dandruff is regarded as a mild noninflammatory form of seborrheic dermatitis. There is a high incidence of seborrheic dermatitis among persons with human immunodeficiency virus infection or Parkinson's disease. The cause of seborrh

Spontaneous transverse colon volvulus in a patient with Duchenne muscular dystrophy: An unreported complication.

A 22-year-old male patient with Duchenne muscular dystrophy (DMD) and chronic constipation presents to the emergency room with severe abdominal pain and hive closed to feces and gas. Contrast-enhanced computed tomography of the abdomen demonstrates mechanical ileus due to volvulus of the transverse colon: torsion of the transverse mesocolon is confirmed and subtotaly colectomy is performed, revealing multiple ischemic areas with focal perforations. DMD is frequently associated with gastrointestinal motility disorders, including chronic constipation and life-threatening conditions like intestin

Multiple bilateral pulmonary "fungus balls" in an immunocompetent patient unsuitable for surgical treatment: efficacy of azoles treatment.

Data guiding management of pulmonary mycetomas are based on uncontrolled trials and case reports. Surgical resection represents a definitive treatment associated with high mortality and sometimes not feasible due to clinical conditions. We report a case of an immunocompetent patient with multiple pulmonary mycetomas, suggestive for probable chronic aspergillosis, in which surgery was contraindicated. The patient experienced a good response to long-term oral voriconazole therapy with remarkable clinical and radiological improvement at three-month follow-up. In cases of probable chronic aspergil

[Multiple subcutaneous Trichophyton rubrum abscesses. Pathomorphosis of a generalized superficial tinea due to impaired immunological resistance].

A patient with chronic polyarthritis is reported who was treated with corticosteroids over several years and also received azathioprine. The patient developed extensive tinea pedum and corporis which was suddenly complicated by multiple subcutaneous abscesses caused by the same organism, Trichophyton rubrum. This type of dermatophytosis has only rarely been described. It shares some characteristics with the maladie dermatophytique of Hadida and Schousboe. Chronic granulomatous dermatophytoses such as granuloma trichophyticum Majocchi or tinea granulomatosa nodularis cruris or acute suppurative

The quest for sustained multiple morbidity reduction in very low-birth-weight infants: the Antifragility project.

OBJECTIVE: Can a comprehensive, explicitly directive evidence-based guideline for all therapies that might affect the major morbidities of very low-birth-weight (VLBW) infants help a neonatal intensive care unit (NICU) further improve generally favorable morbidity rates? Can Antifragility principles of provider adaptive growth from stressors, enhanced infant risk assessment and adherence to effective therapies minimize unproven treatments and reduce all morbidities? STUDY DESIGN: Prospectively planned observational trial in VLBW infants: control group born October 2011 to September 2013 and s

A prospective cohort study examining medical and social factors associated with engagement in life activities following total hip replacement.

OBJECTIVES: Studies show limited improvement in the frequency of engaging in life activities after joint replacement. However, there is a paucity of research that has examined factors, including other life events, which influence engagement following total hip replacement (THR). This research sought to identify factors associated with engaging in life activities following THR. METHODS: A prospective cohort study was conducted with 376 people who had a THR for osteoarthritis (OA). Data were collected pre-surgery and 1 year post-surgery. The primary outcome was change in frequency in engagement

The role of chronic disease in the disparity of influenza incidence and severity between indigenous and non-indigenous Australian peoples during the 2009 influenza pandemic.

BACKGROUND: The 2009 H1N1 influenza pandemic (influenza A(H1N1)pdm09) disproportionately impacted Indigenous peoples. Indigenous Australians are also affected by a health gap in chronic disease prevalence. We hypothesised that the disparity in influenza incidence and severity was accounted for by higher chronic disease prevalence. METHODS: We analysed influenza data from Western Australia, South Australia, the Northern Territory, and Queensland. We calculated population prevalence of chronic diseases in Indigenous and non-Indigenous Australian populations using nationally-collected health sur

Machine learning prediction of premature death from multimorbidity among people with inflammatory bowel disease: a population-based retrospective cohort study.

BACKGROUND: Multimorbidity, the co-occurrence of 2 or more chronic conditions, is important in patients with inflammatory bowel disease (IBD) given its association with complex care plans, poor health outcomes, and excess mortality. Our objectives were to describe premature death (age < 75 yr) among people with IBD and to identify patterns between multimorbidity and premature death among decedents with IBD. METHODS: Using the administrative health data of people with IBD who died between 2010 and 2020 in Ontario, Canada, we conducted a population-based, retrospective cohort study. We describe

Accelerating discovery in pain science: the Acute to Chronic Pain Signatures program.

Chronic pain is a complex and multifaceted disease, with biological causes distributed across tissues and bodily systems and influenced by psychological and social factors. A major aim of pain research is to find better ways to measure signals associated with chronic pain and thus predict, track, and treat pain conditions. Emerging biological measures related to the metabolic, immune, and nervous systems hold promise for identifying the physiology underlying pain and developing new treatments. Developing such measures will require multimodal datasets from large samples in combination with meth

Juvenile Idiopathic Arthritis: A Focus on Pharmacologic Management.

Juvenile idiopathic arthritis is a chronic condition that affects many pediatric patients. It is a prevalent disease and has become the most common rheumatologic disease of childhood. The condition encompasses multiple different forms of chronic arthritides classified based on the location and number of joints affected as well as the presence or lack of a number of different inflammatory markers. The exact etiology is unknown but is thought to be multifactorial with genetic, humoral, and environmental factors playing a key role. Many pharmacologic agents are available for use in the treatment

Syndemic conditions predict lower levels of physical activity among African American men who have sex with men: A prospective survey study.

African American men are disproportionately affected by, not only HIV/AIDS, but also chronic non-communicable diseases. Despite the known benefits of physical activity for reducing chronic non-communicable diseases, scant research has identified factors that may influence physical activity in this population. A growing literature centers on the syndemic theory, the notion that multiple adverse conditions interact synergistically, contributing to excess morbidity. This secondary data analysis examined two primary questions: whether syndemic conditions prospectively predicted physical activity,

The Role of Lipopolysaccharide-Induced Cell Signalling in Chronic Inflammation.

Lipopolysaccharide (LPS) is the main structural component of the outer membrane of most Gram-negative bacteria and has diverse immunostimulatory and procoagulant effects. Even though LPS is well described for its role in the pathology of sepsis, considerable evidence demonstrates that LPS-induced signalling and immune dysregulation are also relevant in the pathophysiology of many diseases, characteristically where endotoxaemia is less severe. These diseases are typically chronic and progressive in nature and span broad classifications, including neurodegenerative, metabolic, and cardiovascular

Measuring internalized health-related stigma across health conditions: development and validation of the I-HEARTS Scale.

BACKGROUND: Health-related stigma and its internalization among individuals with chronic health conditions contribute to impaired mental and physical health and quality of life. Research on health-related stigma has been siloed, with disease-specific measures that may not capture the experiences of individuals with multiple health conditions and that prevent comparisons across health conditions. The current study aimed to develop and test a transdiagnostic measure of internalized health-related stigma for use among adults with different physical health conditions. METHODS: An existing measure

HTLV-1-associated myelopathy as a translational model of progressive neurodegeneration.

Human T-lymphotropic virus type 1 (HTLV-1)-associated myelopathy/tropical spastic paraparesis (HAM/TSP) and progressive forms of multiple sclerosis are chronic CNS diseases characterized by persistent inflammation and progressive neurodegeneration. Although triggered by distinct upstream events [persistent HTLV-1 infection in HAM/TSP and autoimmune mechanisms in multiple sclerosis, with increasing evidence implicating Epstein-Barr virus (EBV) in multiple sclerosis initiation], both conditions converge on shared inflammatory and neurodegenerative cascades. In HAM/TSP, a hierarchical Tax-MAP3K8-

An exploration of the experience of lesbians with chronic illness.

An exploration of the challenges facing lesbians with chronic conditions and their coping strategies was investigated by examining the experiences of participants who were clients of a volunteer organization serving chronically ill lesbians. This article reports the results associated with those challenges, with its ultimate goal being and to assess the effectiveness of current services. Using the participant observation method, as employed by O'Toole (2000), the analysis was based on multiple data sources and 10 years experience within the volunteer organization, including 3 years in direct c

Syndemic interactions of chronic pain, mental health problems, and opioid misuse and their association with academic performance and quality of life among college students.

BACKGROUND: College students experience multiple stressors impacting academic performance and quality of life (QoL), including chronic pain, mental health issues, and opioid misuse, which can co-occur and exacerbate one another. Guided by syndemic theory, this study examined the associations between these three health conditions and academic performance and QoL. METHODS: We analyzed cross-sectional data from 334,957 students from the ACHA-NCHA III survey (2019-2022). Lifetime chronic pain and mental health problems were self-reported based on provider diagnosis or symptoms. Opioid misuse was

Epstein-Barr virus and its association with disease - a review of relevance to general practice.

BACKGROUND: General practitioners encounter the vast majority of patients with Epstein-Barr virus-related disease, i.e. infectious mononucleosis in children and adolescents. With the expanding knowledge regarding the multifaceted role of Epstein-Barr virus in both benign and malignant disease we chose to focus this review on Epstein-Barr virus-related conditions with relevance to the general practitioners. A PubMed and Google Scholar literature search was performed using PubMed's MeSH terms of relevance to Epstein-Barr virus/infectious mononucleosis in regard to complications and associated co

PET Evaluation of Microglial Activation in Non-neurodegenerative Brain Diseases.

PURPOSE OF THE REVIEW: Microglial cell activation is an important component of neuroinflammation, and it is generally well accepted that chronic microglial activation is indicative of accumulating tissue damage in neurodegenerative conditions, particularly in the earlier stages of disease. Until recently, there has been less focus on the role of neuroinflammation in other forms of neurological and neuropsychiatric conditions. Through this review, we hope to demonstrate the important role TSPO PET imaging has played in illuminating the pivotal role of neuroinflammation and microglial activation
