The PRECIsE study: a prospective, multicenter study of shape-sensing robotic-assisted bronchoscopy with 2 years of follow-up.

RATIONALE: Guidelines recommend nonsurgical biopsy for indeterminate pulmonary lesions >8 mm. Shape-sensing robotic-assisted bronchoscopy (ssRAB) is growing in use to biopsy small pulmonary nodules; however, current literature is limited to single-center studies with limited follow-up. OBJECTIVES: This study, PRECIsE, assessed safety and performance of the first ssRAB iteration across multiple sites and nascent users. METHODS: This was a prospective, multicenter, observational study evaluating ssRAB without cone-beam computed tomography guidance in patients with nodules 10 to 30 mm located i

Short-term quantitative CT trajectories of pulmonary Ground-Glass nodules during immune checkpoint inhibitor Therapy: A matched surveillance cohort study.

BACKGROUND: The short-term CT behavior of pulmonary ground-glass nodules (GGNs) during immune checkpoint inhibitor (ICI) therapy remains uncertain. We compared longitudinal CT changes in GGNs observed during ICI therapy with those in a matched surveillance cohort of untreated GGNs. METHODS: In this retrospective exploratory study, the ICI cohort was identified from 1,930 patients with malignant tumors and synchronous pulmonary nodules. After eligibility screening, 98 patients with one target GGN per patient were included, and 80 patients entered the patient-level matched analysis. The compara

Impact of Commercial Artificial Intelligence on Radiologist Reading Time for Pulmonary Nodule Evaluation at Chest CT.

Background Chest CT is a primary method for identifying pulmonary nodules, yet interpreting scans remains time-intensive and demanding. Currently, artificial intelligence (AI) is expected to reduce reading times, but the effect of AI on reporting times in this setting is unknown. Purpose To evaluate the impact of a commercial AI software on radiologists' reading time for pulmonary nodule assessment on chest CT scans within a real-world clinical setting. Materials and Methods This retrospective study included patients who underwent chest CT examinations at a tertiary medical center between Sept

A Novel CBGT Model for Anxiety and Depression in Patients with Pulmonary Nodules.

OBJECTIVE: With the widespread use of computerized tomography (CT) examinations, the annual detection rate of pulmonary nodules has been increasing. However, early non-surgical patients often experience anxiety and depression during follow-up. This study aims to develop a novel intervention model to address these psychological issues and evaluate its clinical efficacy. METHODS: Our research team designed a psychological intervention model tailored to pulmonary nodule patients with anxiety and depressive symptoms. A total of 160 participants were randomly divided into two groups: an experiment

Incidence and risk factor analysis of moderate-to-severe pain after preoperative CT-guided hook-wire puncture localization of pulmonary nodules.

BACKGROUND: Pain is a relatively common complication after hook-wire puncture localization. However, the problem of pain occurrence following this localization procedure has not been sufficiently examined. In this prospective study, we aimed to investigate the incidence and risk factors associated with acute pain after preoperative CT-guided hook-wire puncture localization of pulmonary nodules. METHODS: Clinical data were collected prospectively from patients with small pulmonary nodules who underwent preoperative CT-guided hook-wire puncture localization between May 2022 and May 2023 at the

An Artificial Intelligence-Based Chest X-ray Model on Human Nodule Detection Accuracy From a Multicenter Study.

IMPORTANCE: Most early lung cancers present as pulmonary nodules on imaging, but these can be easily missed on chest radiographs. OBJECTIVE: To assess if a novel artificial intelligence (AI) algorithm can help detect pulmonary nodules on radiographs at different levels of detection difficulty. DESIGN, SETTING, AND PARTICIPANTS: This diagnostic study included 100 posteroanterior chest radiograph images taken between 2000 and 2010 of adult patients from an ambulatory health care center in Germany and a lung image database in the US. Included images were selected to represent nodules with diffe

[Chest CT: spectrum of normal findings].

A fundamental issue in the interpretation of chest CT lies in the ability to determine normality. Technical advances have resulted in an increasing number of submillimeter sections which in turn has resulted in the identification of a large number of minor abnormalities with no significant pathophysiological consequence. These images should be properly interpreted in order to avoid unnecessary follow-up examinations and radiation exposure. Often they are due to respiratory or cardiac motion artifacts. Others are explained by aging, anatomic variants, physiological phenomenon or tobacco use. Th

High-resolution CT of nontuberculous mycobacteria pulmonary infection in immunocompetent, non-HIV-positive patients.

PURPOSE: The objective of this study was to demonstrate that nontuberculous mycobacteria (NTM) pulmonary infections are not so infrequent and that the diagnosis may be suggested on the basis of the high-resolution computed tomography (HRCT) pattern alone. MATERIALS AND METHODS: We retrospectively reviewed HRCT scans of 29 patients (9 men, 18 women; mean age 63 years, range 38-88 years) with positive culture from bronchial wash. Mycobacterium avium complex (MAC) was present in all (with the exception of one in whom the NTM was indistinct). In six patients, MAC was associated with M. chelonae,

Pulmonary metastasis in nevoid basal cell carcinoma syndrome.

Nevoid basal cell carcinoma syndrome is a relatively uncommon clinical entity. It is essentially characterized by the early onset of multiple basal cell carcinomas of the skin generally above wrist-line level, by the presence of jaw cysts, musculoskeletal and other anomalies. Metastatic disease secondary to ordinary basal cell carcinoma is indeed rare. What is even more rare is pulmonary metastases from basal cell carcinoma. Very few cases have been reported in which pulmonary resection was performed for metastatic basal cell carcinoma. We were unable to find a reported case in which pulmonary

Multiple pulmonary atypical carcinoids presenting with long-standing Cushing syndrome masked by pulmonary tuberculosis.

Pulmonary neuroendocrine tumors, especially small cell carcinoma and typical carcinoids, are the most common cause of ectopic Cushing syndrome (CS). Sometimes these adrenocorticotropic hormone (ACTH)-producing tumors are too small to localize, and it may take a long time to identify them. This report describes the case of a 27-year-old-man with CS. This CS was due to the ectopic ACTH produced by the pulmonary atypical carcinoids, which were recognized at the disseminated stage 5 years after the initial presentation of CS. Microscopically, multiple whitish nodules (up to 1.0 cm in diameter) of

[A case of multiple pulmonary leiomyomatous hamartoma].

A 45-year-old woman with multiple pulmonary leiomyomatous hamartoma was reported. Multiple round shadows were observed in the chest X-ray film of mass examination. Retrospectively chest X-ray films revealed an increase in size and number of pulmonary nodules in three years. Metastatic pulmonary tumors were suspected. Primary site was unclear. Calcified myoma uteri was recognized. The pathological diagnosis under diagnostic thoracotomy was multiple leiomyomatous hamartoma. After ten months of medical castration using superactive analogue of LHRH (buserelin) by intranasal spray, chest X-ray film

Pulmonary hyalinizing granuloma: an unusual association with multiple sclerosis.

Pulmonary hyalinizing granuloma (PHG) is a rare entity included in the differential diagnosis of pulmonary nodules of unknown origin. The pathologic entity may represent a peculiar form of pulmonary immune reaction. We report the case of a 40-year-old white woman who had nodular lesions in both lung bases after a 10-year history of multiple sclerosis. An open lung biopsy was required to make the diagnosis. The association of pulmonary hyalinizing granuloma with multiple sclerosis could be coincidental, but since there is a possibility of immune reaction in the pathogenesis of both diseases, th
